Pharmakologische Wiederherstellung funktionell eingeschränkter Melanocortin-4-Rezeptoren (MC4R)

Abstract

Einleitung: Die seit Jahrzehnten ansteigende Adipositasprävalenz in der Bevölkerung stellt ein ernstes Gesundheitsproblem dar. Manifestiert sich Adipositas bereits im Kindesalter, führt dies häufig zu multiplen Folgeerkrankungen und einer drastischen Einschränkung der Lebensqualität. Bei bis zu 5 % aller Patient:innen liegen Mutationen im Melanocortin-4-Rezeptor (MC4R) vor, der eine zentrale Rolle in der hypothalamischen Hunger- und Sättigungsregulation spielt. U.a. kann es durch vorzeitige Stopp-Mutationen (PTCs) im MC4R zu einem nahezu kompletten Funktionsausfall kommen, jedoch existieren hierfür derzeit noch keine Therapieoption. Ein Ansatz zur Therapie PTC-vermittelter Erkrankungen ist das Unterdrücken der PTCs durch den Einsatz von Aminoglykosidantibiotika, sog. Translational Readthrough. Ziel ist eine Wiederherstellung der jeweiligen Proteinexpression und -funktion. Für nonsense mutierte MC4R-Varianten zeigten bisherige Studien in non-humanen Zellen vielversprechende Ergebnisse. In dieser Arbeit wurden erstmals in einem humanen System Versuche zur pharmakologischen Wiederherstellung von MC4R-Varianten durchgeführt. Neben einer Charakterisierung der Rezeptorexpression und einer Messung des Gs-Signalwegs wurde insbesondere auch Wert auf die Untersuchung des Gq/11-Signalwegs gelegt. Seit einigen Jahren ist eine Kopplung des MC4R an diesen Weg bekannt, es wurden allerdings bis dato keine Untersuchungen bei MC4R-Stopp-Mutationen durchgeführt. Methoden: Alle Versuche wurden in HEK-293 Zellen durchgeführt. Vor den funktionellen Assays wurde die optimale Geneticin (G418)-Konzentration in nicht-transfizierten Zellen ermittelt. Anschließend erfolgte die Charakterisierung der MC4R-Expression mittels Fluoreszenzmikroskopie und Ermittlung der Oberflächen- und Gesamtexpression mit und ohne G418. Rückschlüsse auf die Gs-Signalisierung und den Einfluss von G418 konnten durch Messung der cAMP-Akkumulation nach Stimulation mit alpha-MSH und Setmelanotide gemessen werden, die Gq/11-Aktivierung wurde mittels Reportergen-Assay bestimmt. Ergebnisse: Die G418-Behandlung führte nur zu minimalen Verbesserungen der Rezeptorexpression oder -funktion. Im Vergleich zum MC4R-Wildtyp waren die MC4R-Expression und -Funktion stark reduziert. Interessanterweise wurde eine signifikante Zunahme der Basalaktivität im Gq/11-Signalweg durch G418 bei den MC4R-Varianten festgestellt. Fazit: In der vorliegenden Arbeit war keine überzeugende Wiederherstellung der Funktion MC4R-Varianten durch G418-Behandlung möglich. Eine Therapie mit Aminoglykosiden scheint daher keine vielversprechende neue Therapieoption für Adipositas hervorgerufen durch MC4R-Stopp-Mutationen zu sein. Trotzdem sollte der Ansatz weiter untersucht werden. Hierbei sollte der Fokus auf ausgewählte Mutationen gelegt werden und diese in einem neuen Modell und mit anderen Readthrough-Substanzen getestet werden.

Introduction: For several decades the prevalence of obesity in the population has steadily increased, which poses a serious health problem. When obesity manifests in childhood, it often leads to multiple comorbidities and a drastic reduction in quality of life. In up to 5 % of all patients, mutations in the Melanocortin-4 receptor (MC4R) are present, which plays a central role in hypothalamic hunger and satiety regulation. Premature stop mutations (PTCs) in the MC4R can result in a nearly complete loss of function, but currently no therapeutic options are available for this condition. One approach to treat PTC-mediated diseases is the suppression of PTCs through the use of aminoglycoside antibiotics, known as translational readthrough. The goal is to restore protein expression and function. Previous studies in non-human cells have shown promising results for MC4R stop mutations. This study represents the first attempt to pharmacologically restore nonsense mutated MC4R variants in a human system. In addition to characterising receptor expression and measuring the Gs signalling pathway, particular emphasis was placed on investigating the Gq/11 pathway. Although coupling of MC4R to this pathway has been known for some years now, no studies have been conducted on nonsense mutated MC4R variants until now. Methods: All experiments were conducted in HEK-293 cells. Prior to functional assays, the optimal Geneticin (G418) concentration was determined in non-transfected cells. Subsequently, MC4R expression was characterised using fluorescence microscopy, and surface and total expression were determined with and without G418. Insights into Gs signalling and the influence of G418 were obtained by measuring cAMP accumulation after stimulation with alpha-MSH and setmelanotide, and Gq/11 activation was determined using a reporter gene assay. Results: G418 treatment only resulted in minimal improvements of receptor expression or function. Compared to the MC4R wild-type, expression and function of the MC4R-stop-variants were significantly reduced regardless of G418-application. Interestingly, G418 was found to significantly increase basal activity of the MC4R-stop-mutations in the Gq/11 signalling pathway. Conclusion: In this study, no convincing restoration of the function of MC4R-variants through G418 treatment was possible. Therefore, at this point aminoglycoside therapy does not appear to be a promising new treatment option for obesity caused by MC4R stop mutations. Nevertheless, the approach should be further investigated, focusing on selected mutations and testing them in a new model with different readthrough-inducing substances.
