Seit Mitte der neunziger Jahre werden Angiotensin-II-Rezeptor Typ 1
(AT1)-Antagonisten, auch Sartane genannt, als neue Wirkstoffklasse in der
Behandlung von Bluthochdruck eingesetzt. Es konnte gezeigt werden, dass
AT1-Antagonisten positive Effekte auf eine Insulinresistenz haben. Die
zugrunde liegenden, molekularen Mechanismen dieser Prozesse sind jedoch
weitgehend unbekannt. Einige Sartane zeigten pleiotrope Effekte bei in-vitro
Untersuchungen in Adipozytendifferenzierung. Es konnte eine agonistische
Aktivität am Peroxisomen Proliferator-Aktivierten Rezeptor gamma (PPARγ)
festgestellt werden. PPARγ ist ein nukleärer Hormonrezeptor, dessen Funktion
als transkriptioneller Regulator eine bedeutende Rolle im Lipid- und
Glukosestoffwechsel spielt. Aktiviert durch synthetische Liganden wie
Thiazolidindione, auch Glitazone genannt, verbessert PPARγ die
Insulinsensitivität. Der Wirkstoff Telmisartan (IUPAC-Name:
4'-[(1,4'-Dimethyl-2'-propyl[2,6'-bi-1H-
benz[d]imidazol]-1'-yl)methyl]-[1,1'-biphenyl]-2-carbonsäure) zeigte sich
unter den Sartanen als potentester PPARγ-Ligand mit partiell agonistischen
Eigenschaften, aus welchem Grund er für diese Arbeit als Leitstruktur für die
Entwicklung neuartiger PPARγ-Agonisten ausgewählt wurde. Diese neue
Wirkstoffgruppe wird nach dem Prinzip der selektiven
Estrogenrezeptormodulatoren als selektive PPARγ-Modulatoren (SPPARγMs)
bezeichnet. Ihnen wird bei gleicher Wirksamkeit ein verbessertes
Nebenwirkungsprofil im Vergleich zu den vollen Agonisten, den Glitazonen,
zugeschrieben. Im Mittelpunkt dieser Arbeit standen die Synthese von
Verbindungen und deren pharmakologische Untersuchung in etablierten in-vitro
Versuchsmodellen. Zunächst wurde eine Struktur-Wirkungs-Untersuchung von
Telmisartan-fragmenten durchgeführt, um relevante Strukturmerkmale zu
identifizieren, welche für die PPARγ-Aktivierung ausschlaggebend sind. Dafür
wurde das Wirkstoffmolekül in vier Bereiche untergliedert: das zentrale
Benzimidazol und die verschiedenen Reste in Position 1, 2 und 6. Aus diesen
ersten Ergebnissen wurde ein Grundgerüst (4'-[(2-Propyl-1H-
benzimidazol-1-yl)methyl]biphenyl-2-carbonsäure) erstellt, das als essentiell
für die Rezeptoraktivierung angesehen werden kann. Anschließend wurden
Modifizierungen und Erweiterungen dieses Grundgerüstes an den genannten
Positionen durchgeführt. Als besonders wichtig für die PPARγ-Aktivierung wurde
die Position 2 identifiziert. Der Methylgruppe in Position 4 wird ein
positiver Einfluss hingehend einer partiellen Rezeptoraktivität zugeschrieben.
Veränderungen an den Positionen 5 und 6 ergaben eine erhöhte Wirksamkeit auf
Seiten der 6-substituierten Verbindungen. Neben der Beobachtung, dass
lipophile Reste in Position 5 und 6 für eine Potenzerhöhung sorgten, zeigte an
diesen Positionen ein H-Brückenakzeptor eine höhere Wirksamkeit als ein
H-Brückendonor. Die Ergebnisse dieser Arbeit bieten eine Grundlage für
zukünftige SPPARγM-Wirkstoffentwicklung, die in der Behandlung des
metabolischen Syndroms einen wichtigen Stellenwert erhalten können.

Since the mid of 1990's angiotensin-II-receptor type 1 (AT1) antagonists, also
known as sartans, are used for treatment of hypertension. They have been shown
to exhibit positive effects on the state of insulin resistance. However, the
underlying molecular mechanism of this process is widley unknown. A subgroup
of sartans demonstrate pleiotropic effects in in-vitro assays like adipocyte
differentiation. They could be identified as Peroxisome Proliferator-Activated
Receptor gamma (PPARγ) agonists. PPARγ is a nuclear hormone receptor, which
plays a central role in lipid- and glucose homeostasis. Activated by synthetic
ligands like thiazolidindiones, also known as glitazones, PPARγ improves
insulin sensitivity. The compound telmisartan (IUPAC name:
4'-[(1,4'-Dimethyl-2'-propyl[2,6'-bi-1H-
benzo[d]imidazole]-1'-yl)methyl]-[1,1'-biphenyl]-2-carboxylic acid) was
identified as the most potent PPARγ activating sartan with partial agonistic
activity, for which reason it was chosen as lead structure for development of
novel PPARγ-ligands. This new class of compounds is termed analogously to the
concept of selective estrogen receptor modulators as selective PPARγ
modulators. They are considered as compounds with an improved side effect
profile, compared to full agonists like glitazones, and maintenance of their
clinical efficacy. The focus of this work was the synthesis of compounds and
their pharmacological testing in established in-vitro assays. Initially,
several fragments of telmisartan are investigated, to identify the structural
components with relevant PPARγ activity. Thus, the molecule was partitioned
into four sections: the central benzimidazole and its different moieties in
position 1, 2 and 6. From the first results a basic scaffold (4'-[(2-Propyl-
1H-benzo[d]imidazole-1-yl)methyl]biphenyl-2-carboxylic acid) was created,
which was considered as the most essential structure for receptor activity.
This was followed by modifications and extensions on above mentioned
positions. Results showed a high impact of position 2 on PPARγ activation. The
methyl group in position 4 positively influenced receptor activity towards
partial agonism. Introducing lipophilic moieties at position 5 and 6 increased
efficacy, meanwhile a shift of the moieties from position 6 to 5 led to a
diminished activity. Further studies showed a preference of an H-bond acceptor
to an H-bond donor at these positions. The data obtained in this structure
activity relationship study provide the basis for the development of new
SPPARγMs, which could be used for the treatment of the metabolic syndrome in
the future.
