Der primäre Spontanpneumothorax gehört zu einer der häufigen Erkrankungen weltweit und kann einen potentiell tödlichen Verlauf nehmen. Vor allem junge, groß und schlank gewachsene Männer ohne zugrunde liegende Lungenerkrankung sind betroffen. Das Management in Bezug auf die initiale Therapiemethode und damit verbunden die Vermeidung eines Rezidivs wird immer noch kontrovers diskutiert. In dem Zeitraum von Ende 1999 bis Anfang 2009 wurden 115 Patientenfälle mit der Diagnose „Primärer Spontanpneumothorax“ in die Charité – Campus Benjamin Franklin stationär aufgenommen und behandelt. Die Verlaufs- und Therapieergebnisse wurden in einer Datenbank registriert, statistisch ausgewertet und miteinander verglichen. Zudem wurde ein Fragebogen entwickelt und den Patienten zur Abfrage der Langzeitergebnisse zugesandt. Von den 115 Fällen gaben 45 Patienten bei Aufnahme an bereits unter einem Rezidivpneumothorax zu leiden. Nach Abwägen weiterer Risikofaktoren und der bestehenden klinischen Symptomatik wurde in 62 Fällen primär die alleinige Therapie mittels Thoraxdrainage gewählt und in 52 Fällen von Beginn an eine operative Versorgung geplant. Ein Patient wurde lediglich stationär beobachtet. Von den 62 Patienten wurde in 23 Fällen auf ein operatives Verfahren gewechselt, da es im Verlauf zu Komplikationen oder zum Nichterreichen der Therapieziele kam. Insgesamt kam es unter der Behandlung mittels Thoraxdrainage zur einer Komplikationsrate von 24,53% und unter der operativen Behandlung von 14,67%. In 86,67% wurden im Rahmen der Thorakoskopie makroskopisch Bullae nachgewiesen, in fünf weiteren Fällen bestanden sichtbare unspezifische Parenchymveränderungen. Mit Hilfe des Fragebogens konnte bei knapp der Hälfte des Patientenguts die Erfassung der Langzeitergebnisse erfolgen. Trotz einer großen Anzahl von Studien und Leitlinien herrscht bis heute keine Einigkeit unter den Fachkompetenzen in Hinblick auf eine allgemeingültige Therapieempfehlung. Inzwischen wird von vielen Autoren und Mediziner die videoassistierte Thorakoskopie (kurz VATS) mit Resektion von Bullae-tragendem Lungenparenchym als Methode der Wahl bereits bei Erstereignis angesehen. Vor allem die hohe Rate an Rezidiven nach alleiniger Behandlung mit der Thoraxdrainage spricht für dieses Vorgehen. Letztlich besteht weiterhin Forschungsbedarf, um die aktuell gültigen Leitlinien überarbeiten zu können.
The primary spontaneous Pneumothorax is a common disease worldwide with a potentially fatal outcome. Especially young, tall and slim men without underlying lung disease are affected. The management with regard to the initial treatment and consequently the prevention of recurrence is still controversial. In the period from late 1999 to early 2009 115 cases with a diagnosis of "primary spontaneous pneumothorax" were hospitalized and treated in Charité - Campus Benjamin Franklin. The process and therapy results were registered in a database, analysed statistically and compared with each other. In addition, a questionnaire was developed and sent to the patients to obtain information about the long-term results. At hospital admission 45 of the 115 patients reported to already suffer from a recurrence of pneumothorax. After the evaluation of other risk factors and clinical symptoms the therapy of choice was a single chest tube in 62 cases and in 52 cases a surgical procedure was planned from the outset. Of the 62 patients with planned single chest tube application 23 cases were changed into a surgical procedure, due to complications or non-achievement of the treatment goals during the first days of intervention. Overall, following treatment with a chest tube a complication rate of 24.53% was detectedcompared to 14.67% following surgical treatment. In 86.67% of thoracoscopy, bullae were detected macroscopically, in five other cases visible nonspecific parenchymal changes were found. With the help of the questionnaire it was possible to record the long-term results in almost half of the patient population. Despite a large number of studies and guidelines, there is still no agreement among the experts in regard to a general therapy recommendation. Meanwhile, the video-assisted thoracoscopy with resection of bullae-wearing lung parenchyma is considered the treatment method of choice of primary spontaneous pneumothorax even if it is the first occurrence. In particular, the high rate of recurrence after treatment with single thoracic drainage speaks for this procedure. Ultimately, further research is needed to revise the currently valid guidelines.
