Das Kolonkarzinom ist eines der häufigsten Malignome in den westlichen
Ländern. Dabei sind Metastasen für mehr Todesfälle verantwortlich als die
Primärtumoren. Man geht davon aus, dass bereits zum Zeitpunkt des kurativen
chirurgischen Eingriffs oder durch diesen bedingt, bereits einzelne
Tumorzellen im Kreislauf zirkulieren oder schon Mikrometastasen ausgebildet
haben. Die Entwicklung einer adjuvanten Therapie, um die Ausbildung von
makroskopischen Metastasen aus Mikrometastasen zu verhindern ist Gegenstand
aktueller Forschung. Besonders das Konzept der antiangiogenen Therapie scheint
dabei eine vielversprechende Option zu sein. Grundidee dieses Konzeptes ist
es, den Tumor durch Inhibierung der Gefäßneubildung von seiner
Nährstoffversorgung abzuschneiden und somit ein Wachstum der Metastasen zu
verhindern. Andere Konzepte beinhalten die Stimulation des Immunsystems zur
Elimination der Tumorzellen oder die Apoptoseinduktion der Tumorzellen. Die
Induktion der Angiogenese beruht auf einem Ungleichgewicht zwischen pro- und
antiangiogenen Faktoren, welches hervorgerufen wird durch die Bildung von
proangiogenen Faktoren durch Tumorzellen. Dabei hat sich gezeigt, dass
vascular endothelial growth factor (VEGF) einer der wichtigsten proangiogenen
Faktoren ist. Eine Inhibition der Wirkung von VEGF führt daher auch zu einer
geringeren Ausbildung der Tumorvaskularisation. Eine Stimulation des
Immunsystems kann zum Beispiel durch eine vermehrte Rekrutierung von
immunkompetenten Zellen im Tumorgebiet erfolgen. Für diesen Prozess sind
Adhäsionsmoleküle wie ICAM-1, die Immunzellen aus dem Blutkreislauf binden,
essentiell. Eine erhöhte Expression von ICAM-1 im Tumorgewebe korreliert daher
auch mit einer günstigeren Prognose. Für das Enzym Hämoxygenase-1 konnte
gezeigt werden, dass es sowohl die Angiogenese durch Induktion der VEGF-
Synthese fördert und eine Inhibition der Häm-induzierten HO-1 die ICAM-1
Expression erhöht. Eine Inhibition der HO-1 scheint daher für die
antiangiogene Therapie geeignet und fördert gleichzeitig die Rekrutierung
immunkompetenter Zellen in das Tumorgewebe. In der vorliegenden Arbeit konnte
gezeigt werden, dass eine Induktion der HO-1 mit ZnPP zu einer Reduktion der
VEGF-Synthese und einer Erhöhung der Expression des Adhäsionmoleküls ICAM-1
auf Tumorzellen der Kolonkarzinomzelllinie DHD/K12/TRb in-vitro führt. In
einem weiterführenden in-vivo Experiment konnte allerdings kein positiver
Einfluss auf das Tumorwachstum nachgewiesen werden. Die VEGF-Konzentration und
Mikrogefäßdichte zeigten keine Veränderungen zwischen der Vergleichsgruppe und
der Therapiegruppe mit ZnPP. Andere proangiogene Faktoren als VEGF, die nicht
durch HO-1 moduliert werden, scheinen also bei der Ausbildung hepatischer
Metastasen kolorektaler Karzinome von entscheidender Bedeutung zu sein. Die
Inhibition von VEGF mit dem HO-1 Inhibitor ZnPP ist offenbar keine
therapeutische Option in der Prävention makroskopischer Metastasen
kolorektaler Karzinome.

dc.description.abstract
The colonrecctal carcinoma is one of the most often carcinomas in western
countries. Metastases are responsible for more cases of death than the primary
tumors. It`s supposed that already at the time of curative surgery tumor cells
circulate in the blood system or even have built micrometastases. The
development of adjuvant strategies to prevent the growth of macroscopic
metatastases out of micrometastases is part of current investigations.
Especially the concept of anti-angiogenesis seems to be an promising option.
The idea is that through the inhibition of the development of new blood
vessels the tumor cells will be cut off from their supply with oxygen and
nutrients in purpose to suppress growth of meatstasis. The misbalance of pro-
angiogenic and ant-angiogenic factors inducts angiogenesis. It has been shown
that vascular endothelial growth factor (VEGF) is one of the most important
pro-angiogenic factors. The inhibition of VEGF leads to a less marked degree
of tumor vascularisation. The recruitment of immune cells from the blood
circulation into the tumor is mediated by adhesion molecuels like ICAM-1. A
higher expression of ICAM-1 is correlated with a better prognosis.For the
enzyme hemoxygenasis one (HO-1) could be shown that it supports the
angiogenesis by induction of VEGF and an inhibition of HO-1 inducts the
expression of ICAM-1. An inhibition of HO-1 seems to be an appropriate anti-
angiogenic therapy and promotes the recruitment of immunecompetent cells into
the tumor. In this paper it could be shown that that the induction of HO-1
with ZnPP results in a reduction of the VEGF synthesis and an increase of the
expression of ICAM-1 on tumor cells of the colorectal carcinoma cell line
DHD/K12/TRb. In a in-vivo experiment it wasn`t possible to find any postive
influence at the tumor growth. The VEGF concentration and the microvessel
density didn`t show any difference between the investigated groups. Other pro-
angiogenic factors which are not modulated by HO-1 seem to be more crucial in
the develpoment of hepatic metastases of colorectal carcinomas. The inhibition
of VEGF with ZnPP is obviously not therapeutic option in the prevention of
macroscopic metastasis of colorectal carcinomas.
