Einleitung: Bezüglich der optimalen Technik der operativen Versorgung von Verletzungen des hinteren Kreuzbandes (HKB) besteht kein wissenschaftlicher Konsens. Begründet liegt dies - unter anderem - in den bislang uneinheitlichen Beschreibungen der korrekten Anatomie, wobei diese primär aus Kadaver-Untersuchungen abgeleitet wurden. Ziel dieser Arbeit war somit die möglichst exakte Analyse der (Insertions-)Anatomie des HKB in vivo zur Optimierung chirurgischer Vorgehensweisen.
Patienten und Methoden: Erstmalig erfolgte die Beschreibung der Anatomie des nativen HKB mittels 3D-MRT. Es wurden nach manueller Segmentierung der 3D-DESS-Sequenzen die Flächen des femoralen (FFP) und des tibialen Insertionsfußpunktes (TFP) quantitativ und deskriptiv bestimmt. Zudem wurden die Insertionsflächen in Relation zu prägnanten anatomischen Landmarken und routinemäßig verwendeten Bohrergrößen gesetzt.
Ergebnisse: Intraligamentäre Bündel konnten nicht identifiziert werden. Die durchschnittliche Fläche betrug für den überwiegend ovalären FFP 107 mm2 (+/- 27,3 mm2) und für den überwiegend trapezförmigen TFP 88 mm2 (+/- 24,6 mm2). Der Mittelpunkt des FFP fand sich im Mittel im „intercondylar notch view“ bei 1:45 Uhr. Der Mittelpunkt des TFP lag in der sagittalen Aufsicht 12,8 mm (+/- 1,3 mm) posteroinferior zur tibialen Gelenklinie. Das durchschnittliche Bandvolumen wurde mit 2,16 cm3 (+/- 0,64 cm3) bemessen.
Diskussion: Diese Studie wies im Gegensatz zu bisherigen anatomischen Deskriptionen signifikant geringere Insertionsflächen (p<0,0001) auf, welche durch Standardbohrer der HKB-Chirurgie zu großen Teilen erfasst werden können. Zudem fielen geringere Volumina des HKB als bislang beschrieben auf. Insgesamt sprechen die Ergebnisse für die Verwendung nur eines tibialen und eines femoralen Bohrtunnels, für welche eine Hilfestellung zur optimalen anatomischen Positionierung definiert wurde.

Introduction: There is no scientific consensus regarding the optimal technique for the surgical treatment of injuries of the posterior cruciate ligament (PCL). One of the reasons for this is the inconsistent description of the correct anatomy, which has primarily derived from cadaveric studies. The aim of this study was therefore to precisely analyze the anatomy of the PCL and its femoral and tibial footprints in order to optimize surgical procedures.
Patients and methods: This is the first time that an analysis of the anatomy of the native PCL was performed by 3D-MRI. 30 knees were included. After manual segmentation of the 3D-DESS sequences, the surfaces of the femoral (FFP) and tibial (TFP) insertions were analyzed quantitatively and descriptively. In addition, the insertion areas were put in relation to anatomical landmarks and drills that are commonly used in surgical PCL-reconstruction.
Results: Intraligamentous bundles could not be identified. The average area of the predominantly oval FFP was 107 mm2 (+/- 27.3 mm2) and of the predominantly trapezoidal TFP 88 mm2 (+/- 24.6 mm2). The centre of the FFP was found on average in the "intercondylar notch view" at 1:45 o'clock. The centre of the TFP in the sagittal view was found 12.8 mm (+/- 1.3 mm) posteroinferiorly to the tibial joint line. The average ligament volume was measured to be 2.16 cm3 (+/- 0.64 cm3).
Discussion: In contrast to previous anatomical descriptions, this study showed significantly smaller insertion areas for both FFP and TFP (p<0.0001). Hence, standard drills in PCL-reconstruction are able to cover large portions of the footprints. In addition, the volumes of the PCL appear to be smaller than previously described. Overall, the results speak for the use of only one tibial and one femoral drill tunnel. For improved placement of the drill tunnels additional relations to anatomical landmarks were defined.
