Die verzögerte Heilung von Frakturen stellt trotz moderner
Osteosyntheseverfahren ein Problem dar. Tibiaschaftfrakturen zeigen bei mehr
als 5-10% der Patienten eine verzögerte Heilung und häufig sind operative
Reinterventionen notwendig.25 Das Ziel dieser Arbeit ist es, ein Tiermodell
für eine biologisch induzierte Pseudarthrose zu etablieren, um die Ursachen
der verzögerten Heilung zu erforschen und Therapien zu testen. Dazu wurde die
rechte Tibia der Ratten osteotomiert und mit einem Titan-Kirschner Draht
intramedullär versorgt. Der Draht war mit der Trägersubstanz Poly(D,L-Laktid)
und dem Angiogenesehemmstoff Fumagillin beschichtet. Die Gruppengröße zu den
Standzeiten 5, 10 und 28 Tage betrug 8 Tiere und zu den Standzeiten 42 und 84
Tagen 16 Tiere, da neben Histologie und Radiologie eine biomechanische Testung
durchgeführt wurde. Als Kontrolle diente eine Versuchsgruppe, deren Implantate
nur mit der Trägersubstanz beschichtet waren.43 Auf den Röntgenbildern zeigte
sich bis zum 84. postoperativen Tag keine knöcherne Überbrückung des
Osteotomiespaltes. Während in der Kontrollgruppe ein hypertropher Kallus zu
erkennen war, erschienen die Knochen der Fumagillingruppe weniger reaktiv und
hypotroph. Der reaktive Kallus und die Fläche des Knorpels waren in der
Fumagillingruppe zu den frühen Zeitpunkten geringer. In der Gefäßdarstellung
zeigten sich unter Fumagillin signifikant weniger Gefäße nach 5 und 10 Tagen.
Sowohl für die Kontroll- als auch für die Fumagillingruppe ergaben sich in der
biomechanischen Testung eine verringerte Steifigkeit und ein verringertes
maximales Drehmoment im Vergleich mit der intakten Gegenseite. Die Werte der
Kontrollgruppe lagen bis auf das Drehmoment an Tag 42 unterhalb der Werte der
Fumagillingruppe. In der histologischen Auswertung zu den späteren Zeitpunkten
war der periostale Kallus gegenüber der Kontrollgruppe verkleinert. Der Anteil
von Knorpel am Kallus nahm in der Kontrollgruppe im Laufe der Zeit zu, während
er in der Fumagillingruppe abnahm. Allerdings wies die Fumagillingruppe zu den
späten Standzeiten einen höheren mineralisierten Knochenanteil am Kallus auf.
Unter dem Einfluss von Fumagillin entstand eine Pseudarthrose mit verringerter
Angiogenese, die wenig reaktiv und hypotroph war. Die Angiogenese scheint von
großer Bedeutung für die Knochenheilung zu sein und dieses Modell kann als
Grundlage für neue Strategien in der Forschung zur Verbesserung der
Knochenheilung dienen.

dc.description.abstract
The delayed union of fractures remains a problem despite modern surgery.
Fractures of the tibia diaphysis show in more than 10 to 15% a delayed healing
and more than 10 % of the patient need a reoperation. (25) Due to these facts
it is the aim of this project to establish a model a biologic induced
pseudarthrosis. Therefore the tibiae of 56 rats were osteomisied and
intramedullär with a Titan Kirschner wire stabilized. The implant was coated
with the angiogenesis inhibitor Fumagillin. As a control the result of a group
were used, that implants were just coated with the carrier substance
poly-D,L-lactid. The animals were randomized divided into different groups
with the different times of 5, 10, 28, 42 and 82 days. Every group contained 8
rats. Furthermore 16 animals were tested biomechanical after 42 and 82 days.
The tibiae were extracted after the different times and prepared for the
analysis. The radiology showed no bone consolidation of the osteotomy gap
until day 82. While the control group developed a hypertrophy callus, the
fumagillin group seemed to produce a less reactive and hypotroph callus. Both
the control and the fumagillin group showed a less stiffness and maximal
torsion moment than the contra lateral tibia. The amount of vessels was
significant lower after 5 and 10 days in the fumagillin group than in the
control group. Furthermore the reactive callus and the area of cartilage were
smaller in the Fumagillin group. At the later points of time like 28, 42 and
82 days the periostal callus was minimalized in comparison to the control
group. The amount of cartilage from the whole callus was increased in the
control group, while it decreased in the Fumagillin group. This group
presented a larger mineralized bone amount of the callus. In both groups the
amount decreased by tendency. The studies show that the influence of
Fumagillin leads to a pseudarthrosis with a decreased angiogenesis. The
results suggest a development of a less reactive, hypotroph pseudarthrosis.
