Einleitung: Ein sekundärer Befall des zentralen Nervensystems (SZNSL) ist eine seltene Komplikation (<5%) eines systemischen Non-Hodgkin-Lymphoms (NHL), die mit einer schlechten Prognose einhergeht. Bisher konnte, aufgrund der geringen Anzahl an aussagekräftigen Studien, die optimale Therapie nicht etabliert werden. Die systemische Chemotherapie mit Hochdosis Methotrexat (HD-MTX), und die Konsolidierung mit einer Hochdosistherapie mit autologer Stammzelltransplantation (HD-ASCT) - in Anlehnung an das Therapiekonzept primärer ZNS Lymphome (PZNSL) – gilt am ehesten als erfolgsversprechend.
Methodik: Für die folgende Analyse wurden Daten aus einem internationalen prospektiven Register der Charité für SZNSL Patienten (SZNSL-R) verwendet. Insgesamt konnten Daten von 181 Patienten im Hinblick auf prätherapeutische Merkmale, Therapie und das Ansprechen analysiert werden. Zudem wurde die Therapie auf ihre Toxizität des Grades 3-5 hin evaluiert.
Ergebnisse: Das mediane Alter der Patienten betrug 65 (23-86) Jahre zum Zeitpunkt des ZNS-Befalls, 63 (35%) Patienten waren weiblich und 118 (65%) männlich. Eine Beteiligung des ZNS zeigte sich bei 31 Patienten bereits bei Erstdiagnose des systemischen NHL (Kohorte I) und bei 150 Patienten im Rezidiv (Kohorte II). Die Erstlinientherapie beim ZNS-Befall beinhaltete HD-MTX bei 117 (78%) Patienten und HD-AraC bei 86 (57%); bei 56 (31%) konnte eine konsolidierende HD-ASCT durchgeführt werden. Das mediane progressionsfreie Überleben (PFS) im Gesamtkollektiv betrug 7,9 (95% CI 6,1-9,7) Monate, das mediane Gesamtüberleben (OS) 14,8 (95% CI 7,7-22,0). In der univariaten Analyse zeigte sich ein signifikant verbessertes OS der Kohorte I versus (vs.) Kohorte II: 2-Jahres-OS 67% versus 31% (p<0,001; HR 0,31; 95% CI 0,15-0,64) sowie bei einer Therapie mittels einer HD-ASCT: 2-Jahres-OS 51% vs. 27% ohne HD-ASCT (p<0,001; HR 0,40; 95% CI 0,25-0,63). Die Mortalitätsrate unter der Erstlinientherapie des SZNSL betrug insgesamt 8%. Die häufigste Grad 3-5 Toxizität unter der Induktionstherapie waren Infektionen: bei 42% der Patienten unter HD-MTX und bei 23% unter HD-AraC sowie hämatologische Toxizität bei 48% der Patienten. Unter der konsolidierenden HD-ASCT fanden sich am häufigsten Infektionen in 73% der Patienten sowie eine Mukositis in 48%.
Fazit: Im Vergleich zu historischen Serien war das Outcome der SZNSL in der vorliegenden Analyse verbessert, insbesondere wenn eine konsolidierende HD-ASCT durchgeführt werden konnte, trotz häufiger schwerwiegender Nebenwirkungen.

Introduction: A secondary involvement of the central nervous system (SCNSL) is a rare complication (<5%) of systemic non-Hodgkin's lymphoma (NHL) with a poor prognosis. Due to the paucity of data, the optimal therapy of SCNSL has not been established thus far. Systemic chemotherapy with high dose methotrexate (HD-MTX), and consolidation with high-dose therapy with autologous stem cell transplantation (HD-ASCT) - based on the treatment concept of primary CNS lymphoma (PCNSL) – seems the most promising therapy option.
Methods: For the present analysis, data from an international prospective registry for SCNSL patients (SCNSL-R) performed by Charité University Medicine was used. A total of 181 patients were analyzed for pretherapeutic characteristics, therapy and response. In addition, the therapy was evaluated for its grade 3-5 toxicity.
Results: The median age of patients at the time of CNS involvement was 65 (23-86) years, 63 (35%) patients were female and 118 (65%) male. CNS involvement was diagnosed in 31 patients at initial diagnosis of systemic NHL (cohort I) and in 150 patients at recurrence (cohort II). First-line therapy for CNS involvement included HD-MTX in 117 (78%) patients and HD-AraC in 86 (57%); in 56 (31%) patients consolidating HD-ASCT could be performed. The median progression-free survival (PFS) in the whole collective was 7.9 (95% CI 6.1-9.7) months, and the median overall survival (OS) was 14.8 (95% CI 7.7-22.0). The univariate analysis showed a significantly improved OS in cohort I vs. cohort II: 2-year OS 67% versus 31% (p<0,001; HR 0,31; 95% CI 0,15-0,64) and for HD-ASCT therapy: 2- year-OS 51% vs. 27% without HD-ASCT (p<0,001; HR 0,40; 95% CI 0,25-0,63). The overall mortality rate among SCNSL first-line therapy was 8%. The most common grade 3-5 toxicity among induction therapy was infection: in 42% of patients under HD-MTX and 23% under HD-AraC and haematological toxicity in 48% of patients. Among the consolidating HD-ASCT, infections were most common in 73% of patients and mucositis in 48%.
Conclusion: Compared to historical series, the outcome of SZNSL was improved in the present analysis, especially when a consolidating HD-ASCT could be performed, despite frequent serious side effects.
