Einleitung:
Bei der Rekonstruktion des Ligamentum patellofemorale mediale (MPFL) ist es essenziell, den femoralen Bohrkanal anatomiegerecht zu platzieren. Bei Heranwachsenden muss zudem die nahe gelegene distale Femurepiphysenfuge geschont werden.
Die femorale MPFL-Insertion wird als überwiegend distal gelegen beschrieben. Allerdings ist die bisherige Studienzahl an anatomischen Präparaten klein. Insbesondere die Frage, inwiefern sich die femorale Insertion in Abhängigkeit vom Alter proximalisiert, wird nicht einheitlich beantwortet.
Die Häufigkeitsverteilung der verschiedenen MPFL-Läsionsorte scheint sich zwischen Heranwachsenden und Erwachsenen zu unterscheiden. Dies könnte durch die Insertionsentwicklung bedingt sein. Entsprechende histologische Untersuchungen des MPFLs lassen sich jedoch bisher in der Literatur nicht finden.
Diese Arbeit sollte anhand humaner kindlicher Präparate einerseits radiologisch die anatomische Beziehung zwischen femoraler Insertion und Wachstumsfuge und andererseits histologisch Aufbau und Entwicklung beider Insertionen darstellen.
Material und Methodik:
Es standen 11 Präparate aus MPFL mit insertionsnahen Patella- und Femuranteilen von 6 Kindern bis zum 13. Lebensjahr zur Verfügung.
Nach Markierung des femoralen Insertionszentrums wurden die Präparate geröntgt.
Anschließend wurden histologische Paraffinschnitte angefertigt, nach Masson-Goldner-Trichrom gefärbt und lichtmikroskopisch untersucht.
Ergebnisse:
Radiologisch konnten 7 Präparate von 4 Kindern ausgewertet werden. Bei 5 (71,4 %) lag die femorale Insertion distal, bei 2 (28,6 %) etwa auf Höhe und bei keinem (0 %) proximal der Wachstumsfuge.
Patellar zeigte sich die Entwicklung einer größtenteils fibrokartilaginären, femoral die einer rein periostal-fibrösen Insertion.
Als Folge enchondraler Ossifikation wies die Knochenanlage beider Insertionen beim ältesten Kind keine relevanten Mengen hyalinen Knorpels mehr auf. Patellar begann sich eine Kortikalis zu entwickeln, eine kortiko-kartilaginäre Verzahnung mit der Insertion zeigte sich jedoch noch nicht.
Diskussion:
Diese Arbeit legt nahe, dass die femorale Insertion zumeist distal der Wachstumsfuge liegt. Eine relative Insertionsproximalisierung bei älteren Kindern konnte nicht beobachtet werden.
Weiterhin wurden erstmals Aufbau und Entwicklung der physiologischen MPFL-Insertionen histologisch untersucht. Die Arbeit liefert somit substanziell neue Erkenntnisse, auf welcher strukturellen Grundlage MPFL-Verletzungen resultieren.
An einer fibrokartilaginären Insertion gilt der hyaline Knorpel während der Entwicklung als Schwachstelle und die kortiko-kartilaginäre Verzahnung ist für die Insertionsstabilität elementar.
Daher erklärt die Arbeit ein gehäuftes Auftreten patellarer MPFL-Läsionen bei Heranwachsenden, welches in der Literatur beschrieben wurde, schlüssig. Diese Läsionen scheinen gut auf eine konservative Behandlung anzusprechen.

Introduction
When reconstruction of the patellofemoral ligament (MPFL) is performed it is essential to
place the femoral drill hole anatomically. In addition, in adolescents the close-by distal
femoral physis must not be injured.
The femoral MPFL attachment has been described as being located predominantly distal.
However, there had only been a few studies analysing anatomical specimens so far. In
particular, the question how the attachment could move proximal related to age has not
been answered conclusively.
Comparing adolescents and adults, differences in distribution of MPFL lesion sites seem
to exist. This could be due to attachment development, however, there cannot be found
corresponding histological studies on the MPFL in literature.
Analysing human specimens of children, the aim of this study was to demonstrate the
anatomical relationship between the femoral attachment and the physis radiologically as
well as structure and development of both attachments histologically.
Materials and Methods:
11 specimens of 6 children until the age of 12 years, consisting of MPFL with adjacent
parts of patella and femur, were available.
After marking the center of the femoral attachment the specimens were x-rayed.
Finally, histological paraffin sections stained with Masson-Goldner trichrome were
prepared and analysed using light microscopy.
Results:
7 specimens of 4 children could be analysed radiologically. On 5 (71,4 %) the femoral
attachment was distal, on 2 (28,6 %) approximately on the same height and on none of
them (0 %) proximal to the physis.
The development of a mainly fibrocartilaginous patellar and a purely periosteal fibrous
femoral attachment was observed.
Due to enchondral ossification, the bone system on both attachments of the oldest child
contained no relevant amount of hyaline cartilage anymore.
Patellarly cortical bone began to develop, however, a cortical cartilaginous interlocking to
the attachment could not be seen yet.
Discussion:
This study suggests that the femoral attachment is predominantely located distal to the
physis. A relative proximalization in older children could not be observed.
Furthermore, for the first time structure and development of the physiological MPFL
attachments were analysed histologically providing substantially new knowledge on
which structural basis MPFL lesions occur.
At a fibrocartilaginous attachment hyaline cartilage is regarded to be a failure point during
development and the cortical cartilaginary interlocking is fundamental for attachment
stability.
Therefore, this study conclusively explains frequent occurrence of patellar MPFL lesions
in adolescents, that has been described in literature. Those lesions seem to respond
favourably to conservative treatment.
