Während einer Hypoglykämie kommt es zu einer Herabregulation der Insulinrezeptoren auf isolierten peripheren mononukleären Zellen. Insulin konnte als alleinige Ursache für die Herabregulation der Insulinrezeptoranzahl ausgeschlossen werden. Während einer Hypoglykämie kommt es zu einer ausgeprägten Sekretion der gegenregulatorischen Hormone Epinephrin und Cortisol sowie zur Sekretion des kontrainflammatorischen Zytokins Interleukin-10 (IL-10). Tumornekrosefaktor alpha (TNF) stellt als proinflammarorisches Zytokin einen Gegenspieler zu IL-10 dar. Gesunde Probanden (n=10) erhielten eine Infusion von 2µg/kg x min Hydrocortison über 180 Minuten. Eine zweite Probandengruppe (n=10) erhielt eine Infusion mit 25ng/kg x min Epinephrin über 60 Minuten und 75ng/kg x min Epinephrin in den anschließenden 60 Minuten. Untersucht wurde die Insulinbindung an peripheren mononukleären Zellen, die Epinephrin- bzw. Cortisol-, Glukose- und Insulinkonzentration sowie die IL-10- und TNF-Konzentrationen aus Serum bzw. Plasma oder Vollblutproben. Für Epinephrin und Cortisol konnte, in einer Konzentration, wie sie während einer Hypoglykämie auftritt, kein direkter Effekt auf die Insulinrezeptorbindung bei humanen peripheren mononukleären Zellen beobachtet werden. Epinephrin- nicht jedoch Cortisol- bewirkte, in einer Konzentration, wie sie bei einer Hypoglykämie vorkommt, eine Erhöhung von IL-10 ohne die TNF-Konzentration zu beeinflussen.
Hypoglycemia causes an insulin receptor down regulation on human peripheral blood mononuclear cells (PBMC). However it is known, that insulin is not the only reason of this effect. During hypoglycemia a distinct secretion of counterregulatory hormones appears like the secretion of epinephrine and cortisol and of the contra-inflammatory cytokine interleukin-10 (IL-10). Tumor necrosis factor alpha (TNF) as a pro-inflammatory cytokine is the counterpart of IL-10. Healthy volunteers (n=10) received a continuous infusion of 2µg/kg x min cortisol during 180 minutes. Another group (n=10) received an infusion of 25ng/kg x min epinephrine for 60 minutes followed by 75ng/kg x min epinephrine for further 60 minutes. The insulin receptor binding on PBMC as well as the concentrations of epinephrine, cortisol, glucose, insulin, IL-10 and TNF were determined in serum, plasma or whole blood samples. For epinephrine and cortisol, in concentrations usually seen during hypoglycemia, no direct influence on insulin receptor binding on PBMC could be shown. Epinephrine in concentrations usually seen during hypoglycemia - but not cortisol- enhances IL-10 without altering TNF production.
