Einleitung: Die Therapie von Kindern mit Rezidiven stellt die größte
Herausforderung für die Heilung der B-Zellvorläufer akuten lymphoblastischen
Leukämie im Kindesalter (BCP-ALL) dar. Eine weitere Intensivierung der
konventionellen Chemotherapie ist durch die kumulative Toxizität und eine
zunehmende Therapieresistenz limitiert. Zielgerichteter angreifende Substanzen
wie Proteasom- oder Histondeacetylase-Inhibitoren (HDACi) haben sich als gut
verträglich erwiesen. Ihre Wirksamkeit als Einzelsubstanz ist jedoch begrenzt.
Ziel dieser Dissertation war es zu untersuchen, inwiefern die Kombination
solcher neuer Therapieansätze zu einer gesteigerten Wirksamkeit bei ALL-
Rezidiven führt und welche molekularen Mechanismen daran beteiligt sind.
Darüber hinaus ging es darum zu prüfen, wie solche Kombinationen in etablierte
Chemotherapie-Regime integriert werden könnten. Methodik: Die anti-
proliferative und pro-apoptotische Wirksamkeit der Kombinationsbehandlung mit
dem Proteasom-Inhibitor Bortezomib (BTZ) und HDACi wurde in BCP-ALL-Zelllinien
und in primären Leukämiezellen von Kindern mit ALL-Rezidiv in Mono- und Ko-
Kulturen mit mesenchymalen Stromazellen von ALL-Patienten sowie Xenograft
Mausmodellen untersucht. Mittels Genexpressions-Profilen und Validierung auf
Protein-Ebene wurden Signalwege identifiziert, die wesentlich am
Zustandekommen der Kombinationseffekte beteiligt sind. Ergebnisse: Die
kombinierte Behandlung mit BTZ und HDACi induzierte klar synergistische anti-
proliferative und pro-apoptotische Effekte in BCP-ALL Mono- und Ko-Kulturen
mit mesenchymalen Stromazellen. Der synergistische Effekt erforderte die
gleichzeitige Applikation beider Substanzen. In Gegenwart etablierter
Chemotherapie-Substanzen blieb der synergistische Effekt erhalten. Die
Behandlung mit BTZ und HDACi begrenzte in Xenograft Mausmodellen der BCP-ALL
ein Fortschreiten der Leukämie durch eine verminderte Infiltration von
lymphatischen und extralymphatischen Geweben sowie die Induktion von Apoptose
in Leukämiezellen. Die Analyse der zugrundeliegenden Signalwege ergab eine
differenzielle Regulation von Genen im Zellzkylus-, JUN/MAPK-, PI3K/AKT-,
p53-, ubiquitin-proteasom- und NF-κB Signalweg. Die Einzelbehandlung mit BTZ
aktivierte das NF-κB signaling, während nur nach BTZ/HDACi
Kombinationsbehandlung eine Induktion von Apoptose-relevanten NF-κB-Zielgenen
zu beobachten war. Auf eine mögliche Apoptose-vermittelnde Rolle von NF-κB in
diesem Kontext verwies auch die Beobachtung, dass BCP-ALL-Zellen von
Patienten, die während der Chemotherapie ein Rezidiv erlitten, eine geringere
Genexpression von NF-κB Untereinheiten zeigten, als von solchen, deren
Erkrankung nach Abschluss der Therapie rezidivierte. Schlussfolgerung: Die
synergistische Steigerung der therapeutischen Wirksamkeit ohne Zunahme der
Toxizität sowie günstige Interaktionen mit ausgewählten Chemotherapie-
Substanzen stellen die Grundlagen für eine Integration von BTZ und HDACi in
gegenwärtige Therapieprotokolle der BCP-ALL zur Verfügung. Besondere
Aufmerksamkeit erfordert dabei die Planung der Medikamenten-Gabe, da nur bei
gleichzeitiger Applikation von BTZ und HDACi ein synergistischer Effekt
zustande kam, während die Interaktion mit einzelnen Chemotherapeutika durch
die Reihenfolge der Applikation bestimmt wurde.

dc.description.abstract
Purpose: Relapse of disease remains the major challenge towards the cure of
childhood B-cell precursor acute lymphoblastic leukemia (BCP-ALL). A further
intensification of mostly genotoxic conventional therapy regimens is limited
through cumulative toxicity and emerging resistance. More targeted therapeutic
approaches such as proteasome or histone deacetylase inhibitors (HDACi) have
shown a good tolerability with only limited activity as single agents. The aim
of this thesis was to test the efficacy of combined treatment with such
targeted drugs and to analyze the underlying mechanisms involved in the
combined activity. Furthermore, the focus of my work was to determine how
these combinations could be integrated into established chemotherapy regimens.
Experimental Design: The anti-proliferative and pro-apoptotic efficacy of
combined treatment with the proteasome inhibitor bortezomib (BTZ) and HDACi
was assessed using BCP-ALL cells form cell lines and primary patient samples
in mono- and cocultures with mesenchymal stroma cells from ALL patients, as
well as xenograft mouse models. Gene expression signatures with protein
validation were performed to identify the pathways involved in the efficacy of
combined treatment. Results: Combined treatment with BTZ and HDACi induced
anti-proliferative and pro-apoptotic effects in a clearly synergistic manner
in BCP-ALL mono- and cocultures with stroma cells. The synergistic effect
required concomitant application of both drugs and was still observed in the
presence of conventional ALL chemotherapeutics. In xenograft mouse models of
BCP-ALL, combined BTZ/HDACi treatment limited the expansion of leukemic
disease through a reduced infiltration of lymphatic and extra-lymphatic
tissues and induction of leukemia cell apoptosis. Analysis of the underlying
mechanisms revealed the regulation of genes involved in cell cycle, JUN/MAPK-,
PI3K/AKT-, p53-, ubiquitin-proteasome and NF-κB pathways. BTZ single treatment
induced an activation of NF-κB signaling, whereas only after combined
BTZ/HDACi treatment also an induction of apoptosis-related NF-κB target genes
was observed, suggesting a possible role of NF-κB as pro-apoptotic mediator in
this context. This might be further supported by the observation that ALL
cells from patients who developed a relapse during the frontline chemotherapy
showed lower NF-κB subunits gene expression as compared to relapses after
chemotherapeutic treatment. Conclusion: An increase of efficacy, which was
achieved without an increase of toxic effects, and favorable interactions with
selected chemotherapeutics provide the basis for an incorporation of BTZ and
HDACi into current BCP-ALL treatment regimens. However, scheduling of drug
application warrants attention, since only concomitant application of BTZ and
HDACi induced synergistic effects and interactions with certain
chemotherapeutics depended on the sequence of drug exposure.
