Giardiasis ist eine durch den Endoparasiten Giardia duodenalis (G. duodenalis)
verursachte Infektionskrankheit des gastrointestinalen Trakts, die sowohl
asymptomatisch als auch symptomatisch verläuft und sich unbehandelt in
chronischen oder rezidivierenden Infektionen manifestieren kann. Die Therapie
erfolgt vorwiegend mit dem Nitroimidazol Metronidazol (MTZ). Ein hohe Rate an
Therapieversagern und die weltweite Verbreitung der Erkrankung reflektieren
die Notwendigkeit für alternative Behandlungsmöglichkeiten.
Tetrahydrolipstatin (THL) findet aufgrund seiner inhibitorischen Wirkung auf
gastrointestinale Lipasen weitverbreitete Anwendung in der Therapie der
Adipositas und gilt als sicheres und nebenwirkungsarmes Arzneimittel. Die
Evaluierung der antigiardialen Aktivität von THL war von großem Interesse,
weil THL in einigen Untersuchungen bereits wachstumshemmende Effekte auf
einzellige parasitäre Organismen zeigte und aufgrund seiner Pharmakokinetik
den Vorteil einer in situ Bioverfügbarkeit mit sich bringt. In dieser Arbeit
wurde das antigiardiale Potential von THL gegenüber der sich replizierenden
Form von G. duodenalis, den Trophozoiten, in vitro untersucht. G. duodenalis
weist eine hohe genetische Vielfalt auf und wird in verschiedene Assemblagen
gruppiert, von denen zwei, Assemblage A und B, humanpathogen sind. Daher
wurden sowohl Patientenisolate, als auch Referenzstämme dieser Assemblagen in
die Beurteilung miteinbezogen. Mit Hilfe des Resazurin Assays konnten die
antigiardialen Aktivitäten von THL und MTZ vergleichend bewertet werden. THL
zeigte dabei eine potente und konzentrationsabhängige Inhibition der
Trophozoitenreplikation, die unabhängig vom Genotyp beobachtet wurde und
überwiegend effektiver war als die durch MTZ erzielte. THL führte in vitro zu
einer irreversiblen Schädigung der Trophozoiten, die sich u.a. in einer
erheblichen Beeinträchtigung der Morphologie äußerte. Mit zunehmender THL
Konzentration zeigten Flagellen und dorsale Oberfläche in transmissions- und
rasterelektronenmikroskopischen Analysen eine außergewöhnlich starke
Blasenbildung. Zudem dokumentierte die für Lipide selektive Nilrotfärbung eine
verminderte Lokalisierung der Lipide in der perinukleären Membran der
Trophozoiten durch THL. Um zu überprüfen, ob THL einen Einfluss auf die
Inkorporation von bevorzugt in der Zellmembran und Membran der Flagellen
vorkommenden Phosphatidylcholine hat, wurde ein fluoreszenzmarkiertes Analogon
eingesetzt. THL führte zu einer signifikant verringerten Aufnahme dieses
Phosphatidylcholins. Diese Effekte deuten daraufhin, dass THL über eine
Einflussnahme auf den Lipidstoffwechsel antigiardial wirksam ist. Auf der
Suche nach möglichen Mechanismen für die beobachteten toxischen Effekte von
THL auf die Trophozoiten war die Untersuchung hinsichtlich einer Lipase
Aktivität und deren Inhibition durch THL ein weiterer Ansatz, da es u.a.
Hinweise auf putative Lipasen in den Genomen der derzeit vollständig
sequenzierten Giardia-Isolate gibt. Die Ergebnisse dieser Arbeit bestätigen,
dass Giardia-Trophozoiten die Fähigkeit zur Hydrolyse von Phospholipiden
besitzt. Allerdings war eine Inhibition durch THL in vitro nicht zu
verzeichnen. Zudem wurde unter Verwendung der Kombinationsindex-Methode nach
CHOU and TALALAY (1984) die Interaktion von THL und MTZ charakterisiert. Dabei
zeigte sich, dass diese Kombination in vitro additive Effekte bewirkt.

Giardiasis is a prevalent gastrointestinal infection caused by the protozoan
parasite Giardia duodenalis (G. duodenalis). In humans, giardiasis can be
either asymptomatic or symptomatic, commonly with manifestations in chronic or
relapsing infections if left untreated. The therapy is predominantly based on
the treatment with the nitroimidazole Metronidazole (MTZ). However, a high
rate of treatment failure and the fact that giardiasis is one of the most
common parasitic infections of the human intestinal tract worldwide reflect
the urgent need for alternative treatment options. Due to its inhibitory
effect on gastrointestinal lipases, Tetrahydrolipstin (THL) is currently
widely used as a medication in the treatment of obesity and is considered
safe, with few side effects. The evaluation of the antigiardial activity of
THL was of particularly high interest following some previous reports on its
growth inhibiting effects on several protozoan parasites and its
pharmacokinetic properties associated with favourable in situ bioavailability.
In this study the antigiardial potential of THL against the trophozoites, the
proliferating stage of G. duodenalis, was examined under in vitro conditions.
G. duodenalis shows a high genetic variability and is grouped into different
assemblages, of which two (the assemblages A and B) are pathogenic to humans.
For this reason, isolates of patients as well as reference strains of these
assemblages were considered in this study. The antigiardial activities of THL
and MTZ were compared using the resazurin assay. THL showed a potent and
concentration dependent inhibition of the trophozoites´ replication. This
effect was independent of the genotype and was shown to be more effective
compared with that of MTZ. THL produced an irreversible damage to the
trophozoites under the experimental conditions, evident from dramatic effects
on their morphology. Increasing THL concentrations lead to an extraordinary
high membrane blebbing on flagella and dorsal surfaces in transmission and
scanning electron microscopy analyses. Furthermore, the selective staining of
lipids by nile red revealed a reduced accumulation of lipids in the
perinuclear membrane of the trophozoites in the presence of THL. To verify
whether THL affected the incorporation of phosphatidylcholines, which are
primarily found in the cell membrane and membrane of the flagella, a
fluorescently labelled analogue was used, demonstrating that THL significantly
reduced the uptake of this phosphatidylcholine. These findings suggest that
the antigiardial effects of THL are closely related to an influence on the
lipid metabolism. Since there is strong evidence for the presence of putative
lipases in the genomes of fully sequenced Giardia isolates, the examination of
the activity of lipases and the inhibition of these enzymes by THL appeared
reasonable. The results confirmed that Giardia trophozoites possess the
capability to hydrolyse phospholipids. However, THL apparently failed to
inhibit the activity of these enzymes under the in vitro conditions. With
regard to reported promising applications of combining drugs and the
increasing number of treatment failure in the therapy of giardiasis, the
interaction of the combination of THL and MTZ was evaluated using the
combination index method according to CHOU and TALALAY (1984). Interestingly,
the results provided evidence for additive effects of this drug combination in
vitro.
