VEGF wird einer Vielzahl von Zellen produziert (z.B. endotheliale und
epitheliale Zellen) und spielt eine wichtige Rolle bei vielen Prozessen. Er
aktiviert zwei tyrosinkinaserezeptoren: VEGF-Rezeptor-1 (VEGFR-1, FLT1) und
VEGF-Rezeptor-2 (VEGFR-2, FLK/KDR). Die meisten bekannten VEGF-Effekte (z.B.
Proliferation und Migration von endothelialen Zellen) sind durch VEGFR-2
vermittelt. Über die Funktion von FLT1 ist bislang weniger bekannt. FLT1
moduliert die VEGF-Wirkung am FLK-Rezeptor indem es den freien, aktiven
Wachstumsfaktor bindet. Es existieren zwei Splicevarianten des FLT1-Rezeptors,
den membranständigen Rezeptor, sowie eine lösliche Isoform (sFLT-1). Bei
enthalten eine höher-affine VEGF-Bindungsstelle als FLK. FLT1 wird nicht nur
in endothelialen Zellen, sondern auch auf Leukozyten und auf verschiedenen
epithelialen Zellen exprimiert. Es scheint Einfluß auf die Funktion von
Makrophagen zu nehmen und spielt ein Rolle bei der Pathogenese entzündlicher
und maligner Erkrankungen sowie der Atherosklerose. Das Ziel meiner
Doktorarbeit war es, ein besseres Verständnis in die Funktion und Regulation
des FLT1-Rezeptors zu erhalten. Hierzu untersuchte ich den Einfluß von FLT1
und sFLT1 auf die Pathogenese der Präeklampsie, einer Erkrankung der
Schwangeren. Aktuelle Studien beschrieben eine übermäßige Ausschüttung von
sFLT1 bei präeklamptischen Frauen, die durch den hemmenden Einfluß auf die
Aktivität von VEGF und dem Plazentaren Wachstumsfaktor (PlGF) zur Pathogenese
der Präeklampsie führen könnte. Ich konnte zeigen, dass die übermäßige
Ausschüttung von sFLT1 aktivierte Neutrophile Granulozyten von
präeklamptischen Schwangeren davon abhält, auf ein VEGF-Signal vermehrt zu
migrieren, da FLT1 und sFLT1 in den Neutrophilen Granulozyten präeklamptischer
Frauen niedriger exprimiert werden. Im Gegensatz hierzu waren die Serum-
Konzentrationen von sFLT1 bei präeklamptischen Patientinnen erhöht, wodurch
eine VEGF-gesteuerte FLT1-Heraufregulation in Neutrophilen Granulozyten
verhindert wurde. Die VEGF-abhängige Expression des FLT1-Rezeptors war
Promotorgesteuert. Die Aktivität des FLT1-Promotors war durch sFLT1
vermindert. Weitere in-vitro-Experimente zeigten, dass die Migraton der
Neutrophilen Granulozyten durch VEGF reguliert wurde und überschießende
Serumkonzentrationen von sFLT1 dies verhinderten. Die VEGF-abhängige Migration
von Neutrophilen Granulozyten ist bei Patientinnen mit einer Präeklampsie
folglich in Folge der überschießenden sFLT1-Ausschüttung vermindert. Diese
Ergebnisse führten dazu, dass ich mehr über die transkriptionelle Regulation
von FLT1 und den Einfluß genetischer Varianten erfahren wollte. Ich fand einen
SNP mit einem Basenpaarausausch von C nach T, der bei ca. 6% der untersuchten
Individuen vorlag. Der SNP lag in einer vermeintlichen P53-Bindungsstelle. Nur
der Promotor mit dem T SNP (FLT1-T) reagierte auf P53-Stimulation in
Reportergenanalysen oder wenn die endogene Genexpression in Zelllinien mit
unterschiedlichem SNP-Status untersucht wurde. Die recht seltene
FLT1-T-Variante verbindet dieses Gen mit dem großen Netzwerk, das durch den
Transkriptionsregulator P53, einen Tumorsuppressor, der eine Rolle in vielen
Stressreaktionen spielt, koordiniert wird. Zusammenfassend konnte ich zeigen,
dass die FLT1-Expression in hohem Maße von der Promotoraktivität abhängt und
hierdurch eine Vielzahl von Prozessen des Gefäßsystems beeinflußt werden.

dc.description.abstract
VEGF and its isoforms are produced by a variety of cells, e.g. endothelial and
epithelial cells, in different tissues and play an important role for many
processes within the vascular system by activating two tyrosinkinase
receptors: VEGF-receptor-1 (VEGFR-1, FLT1) and VEGF-receptor-2 (VEGFR-2,
FLK/KDR). Up to now, most of the effects (e.g. proliferation and migration of
endothelial cells) by VEGF are referred to the activity of VEGFR-2, whereas
the function of FLT1 is less clear. It is reported that FLT1 is able to
modulate signalling of VEGF through FLK by sequestering VEGF, because its mRNA
is alternatively spliced to encode both a full-length receptor tyrosine kinase
and a soluble isoform (sFlt-1) that contain a higher affinity VEGF–binding
domain than FLK. FLT1, which is not only expressed on endothelial cells but
also on leukocytes and different epithelial cells, seems to promote the
function of macrophages, inflammatory diseases, cancer metastasis, and
atherosclerosis via its kinase activity. The aim of my dissertation was to
shed more light into the function and regulation of FLT1. Therefore, I studied
the influence of FLT1 and sFLT1 on the pathogenesis of preeclampsia a
pregnancy-associated disease, because recent studies have suggested that
excess secretion of sFLT1 may contribute to the pathogenesis of preeclampsia
by antagonizing VEGF and placental growth factor (PlGF). Here, I was able to
demonstrate that excess of sFLT-1 seems to prevent activated neutrophils from
women with preeclampsia from additional migration by VEGF, because
preeclamptic women expressed lower FLT-1 and sFLT-1 in neutrophils. In
contrast, serum levels of sFLT-1 in patients with preeclampsia were increased
and, therefore, inhibited upregulation of FLT-1 in neutrophils by neutralizing
VEGF. VEGF-dependent FLT-1 expression was regulated by changing FLT-1-promoter
activity. Promoter activity was decreased by sFLT-1. Further in vitro
experiments demonstrated that migration of neutrophils is regulated by VEGF
via FLT-1 and excess of sFLT-1. Thus, VEGF-dependent migration of neutrophils
is decreased during preeclampsia as a consequence of excess circulating sFlt1.
These data stimulated me to understand more the transcriptional regulation of
FLT1 and its genetic variation. I identified a C-to-T SNP upstream of the
transcriptional start site in approximately 6% of the people examined. The SNP
is located within a putative p53 response element. Only the promoter with the
T SNP (FLT1-T) was responsive to p53 when examined with reporter assays or by
endogenous gene expression analysis in cell lines with different SNP status.
The infrequent FLT1-T variant results in the inclusion of this gene into the
network coordinated by the master transcription regulator p53, a tumor
suppressor gene that is highly responsive to a variety of stress conditions.
In summary, I was able to show that FLT1 expression is highly regulated by its
promoter activity and, thereby, gains influence on many processes within the
vascular system.
