Die Therapie akuter Verbrennungen, insbesondere für Patienten mit Wunden des
Grades II°b, ist bisher unbefriedigend gelöst. Die Dissertation evaluiert ein
innovatives Therapiekonzept, basierend auf einer primären Hautzellisolation
aus fötalem oder adultem Gewebe, einer autologen Sprühtransplantation sowie
einer temporären Versorgung applizierter Zellen über aktive Wundauflagen aus
perfundierten Hohlfaserkapillaren. Ziel ist es, über eine schonende Isolation
den Ertrag basaler Keratinozyten-Vorläuferzellen zu erhöhen und diese
gleichmäßig mit einem intraoperativen Sprühverfahren im defekten Wundgrund zu
verteilen. Zur Überbrückung der initial suboptimalen Mikrozirkulation im
Wundbett werden die Hohlfaserkapillaren mit therapeutischen Lösungen
perfundiert. Optional können den Lösungen Wachstumsfaktoren, wie z. B.
rhGDF-5, beigemengt werden. Die Hautzellisolation erfolgt über einen Dispase-
Trypsin-Ansatz mit anschließender Abzentrifugation überschüssiger Enzyme und
Zellwaschung. Die Sprühtransplantation wird über eine prozessorgesteuerte
Pumpe und einen speziell entwickelten Sprühkopf realisiert. Die Wundauflage
besteht aus einem Silikongerüst mit eingebetteten MicroPES® TF-10
Hohlfaserkapillaren, welche über ein Schlauchsystem perfundiert werden können.
Die Zellisolation und Sprühapplikation wurden in einer ersten Publikation in
vitro mit humanen fötalen und adulten Keratinozyten sowie dermalen
Fibroblasten getestet. In zwei klinischen Machbarkeitsstudien wurden diese
Verfahren an geeigneten Verbrennungspatienten in einem intraoperativen,
ambulanten Setting erprobt. In zwei weiteren Arbeiten wurde das Konzept einer
aktiven Wundauflage vorgestellt und in vitro sowie in vivo evaluiert. Für die
In-vivo-Studie kam ein Schweine-Wundmodell mit diversen Perfusionslösungen
(mit und ohne rhGDF-5) zum Einsatz. Die Datenerhebung fand über mikroskopische
Dokumentation, die Aufnahme morphometrischer und biochemischer Parameter,
Zellzählungen, Immunhistochemie, Histologie und klinische Fallbeschreibungen
statt. Es konnte experimentell gezeigt werden, dass eine erfolgreiche
Isolation und Sprühapplikation basaler epidermaler Progenitoren und dermaler
Fibroblasten ohne Vitalitätseinbußen möglich ist. Eine klinische
Sprühtransplantation lieferte an neun Verbrennungspatienten funktional und
kosmetisch ansprechende Ergebnisse innerhalb des ersten Jahres nach der
Behandlung. Im Schweine-Wundmodell konnte bereits durch Spülung der aktiven
Wundauflage mit 1 %-iger Trehaloselösung eine geförderte Heilung beobachtet
werden. Mit Zusatz des Faktors rhGDF-5 verstärkte sich der positive Trend in
der Wundheilung. Perfusionsansätze mit weiteren Gruppen zeigten eine
signifikant verschlechterte Wundheilung (p < 0,5). Das vorgestellte
Gesamtkonzept kann für Verbrennungspatienten des Grades II°b zu einem
beschleunigten Wundverschluss und einer ästhetisch verbesserten Therapie
führen.

dc.description.abstract
The treatment of acute burns, especially for patients with 2b degree wounds,
is not resolved satisfactorily. The thesis evaluates an innovative therapeutic
concept based on a primary skin cell isolation from fetal or adult tissue, an
autologous spray-transplantation and a temporary wound supply via an active
wound dressing based on hollow fiber capillaries. The aim is to increase the
yield of regenerative basal keratinocyte precursor cells through a gentle
isolation technique and to distribute them evenly in the damaged wound bed via
intraoperative spray dispersion. To bridge the lack of microcirculation in the
wound bed, the hollow fiber capillaries are perfused with therapeutic
solutions that optionally contain regenerative growth factors such as rhGDF-5.
The skin cell isolation is realised with a dispase-trypsin approach followed
by centrifugation of excess enzymes and a wash step. The spray-transplantation
is implemented via a processor-controlled pump and a specially developed spray
nozzle. The active wound dressing consists of a silicone scaffold with
embedded MicroPES® TF-10 hollow fiber capillaries and a supporting tubing
system for perfusion. The testing of the cell isolation and spray application
was carried out in a published in vitro study using human fetal and adult
keratinocytes as well as dermal fibroblasts. In two published clinical
feasibility studies both methods were tested on suitable burn patients in an
intraoperative ambulant setting. In two other published studies the concept of
an active wound dressing was introduced and evaluated in vitro as well as in
vivo. For the in vivo study, a porcine wound model was applied while using
different perfusion solutions (with and without rhGDF-5). Data collection was
conducted by means of microscopic documentation, recording morphometric and
biochemical parameters, cell counts, immunohistochemistry, histology and
clinical case reports. It was shown experimentally that a successful isolation
and spray-transplantation of basal epidermal progenitors and dermal
fibroblasts is possible without a loss of viability. A clinical spray-
transplantation of nine burn patients led to functionally and cosmetically
improved results within the first year after treatment. Flushing of 1 %
trehalose solution via the active wound dressing demonstrated promoted wound
healing in the porcine model already. With the addition of the factor rhGDF-5
the positive trend in wound healing was more pronounced. The otherwise tested
perfusion setups showed a significantly impaired wound healing (p < 0.5). The
proposed concept may lead to accelerated wound closure and an aesthetically
improved therapy for 2b degree burn patients.
