Diese Dissertation beschäftigt sich mit der Untersuchung der Genotyp-Phänotyp-
Korrelation bei Patienten mit diagnostiziertem Morbus Wilson. Morbus Wilson
ist eine autosomal rezessiv vererbte Krankheit, welche durch Mutationen im
ATP7B-Gen auf dem Chromosom 13 bedingt ist. Bisher wurden über 250
verschiedene Mutationen identifiziert, welche durch Veränderungen des ATP7B-
Gens die Funktion der kupfertransportierenden ATPas7B, für die das Gen
kodiert, beeinträchtigen können. Durch diese Störung des Kupfermetabolismus
kann es zu toxischer Kupferakkumulation und damit verbundenen
Funktionsstörungen der Leber, des ZNS, der Augen, der Nieren und anderer
Organe kommen. In dieser Arbeit wurden insgesamt 718 Personen als Index-
Patienten oder als Verwandte dieser Index-Patienten genetisch untersucht,
wobei 145 verschiedene Mutationen im ATP7B-Gen identifiziert wurden. Unter
diesen befanden sich 61 bereits bekannte und 84 bisher noch nicht beschriebene
Mutationen. Zunächst wurden für 11 verschiedene Länder Mutationsspektren
erstellt. Dies ist von großer Bedeutung für die gezielte Suche nach
Mutationen. Dadurch wird ein Familienscreening erleichtert und die schnellere
Einführung prophylaktischer und therapeutischer Maßnahmen ermöglicht. Aufgrund
der großen Heterogenität des Genotyps im ATP7B-Gen gestaltete sich die
Untersuchung möglicher Genotyp-Phänotyp-Korrelationen schwierig. Tendenziell
konnte hierbei beobachtet werden, dass homozygote Träger einer missense-
Mutation gegenüber denen mit einer nonsense- oder frameshift-Mutation weniger
stark verringerte Coeruloplasmin-Serum-Konzentrationen und Kupfer-Serum-
Konzentrationen aufweisen. Homozygote Träger einer missense-Mutation leiden
tendenziell öfter unter einer neurologischen Primärsymptomatik. Genotyp-
Phänotyp-Korrelationen waren aufgrund der ausreichend großen Patientenzahl nur
für die H1069Q-Mutation, die 3402delC- und für die L708P-Mutation zu erkennen.
Die H1069Q-Mutation stellt sich hier mit verhältnismäßig milden Veränderungen
und einem späten Krankheitsbeginn dar, während die 3402delC-Mutation sowie die
L708P-Mutation zu schwereren Veränderungen und einem früheren Krankheitsbeginn
führen. Bei der Untersuchung eines Zusammenhanges zwischen der Präsenz eines
Kayser-Fleischer-Ring und der Primärsymptomatik zeigte sich dieser bei einem
Großteil der symptomatischen Patienten, wobei er unter den Patienten mit einer
neurologischen Primärsymptomatik bei allen diagnostiziert wurde. Aufgrund der
Überlegung, dass andere genetische Veränderungen Einfluss auf Schweregrad,
Primärsymptomatik oder Alter der Erstmanifestation haben könnten, erfolgte bei
vielen homozygoten Patienten die HFE-Genotypisierung und die Bestimmung des
APOE-Genotyps. Bei Veränderungen im HFE-Gen ließ sich kein Zusammenhang mit
dem Phänotyp des Morbus Wilson erkennen. Unter den Patienten mit dem Genotyp
APOE 3/3 sind verhältnismäßig viele asymptomatische Patienten und ein späteres
Alter der Erstsymptomatik zu beobachten.

dc.description.abstract
This dissertation contains an analysis of a genotype-phenotype-correlation in
Wilson´s Disease. Wilson´s Disease is an autosomal recessive disorder which is
caused by mutations located in the ATP7B gene on chromosome 13. Until now more
than 250 different mutations could be found. They change the ATP7B gene and in
this way the function of the copper transporting ATPase7B for which the gene
codes. The copper metabolism´s disorder can lead to toxic copper accumulations
which cause malfunctions of the liver, the CNS, the eyes, the kidneys and
other organs. In the course of this dissertation 718 persons have been
genetically examined as Index-patients or the relatives of such. In this way
145 different mutations have been identified. While 61 were already known, the
other 84 have been identified for the first time. First of all, spectrums of
mutations for 11 different countries have been developed. This has great
importance to the selective search of mutations. It makes family screening
easier and makes it possible to start earlier with prophylactic and
therapeutic treatment. The big heterogeneity of the genotype in the ATP7B gene
complicates the examination of a genotype-phenotype-correlation to a great
extent. In this work 33 different mutations have been phenotypically
considered. It could be seen that homozygote missense-mutations tend to result
in not that strong decreased ceruloplasmin- and copper-serum-concentrations as
in nonsense-, frameshift- or splice-mutations. Homozygote carriers of
missense-mutations do more often have neurological primary symptoms. Genotype-
phenotype-correlations could be seen for the H1069Q-, the 3402delC- and the
L708P-mutation. The H1069Q-mutation presents phenotypic comparatively mild
changes and a late onset of the illness, while the 3402delC-mutation and the
L708P-mutation lead to a heavier form and an earlier onset of the disease.
During the inspection of the coherence between the primary symptoms and the
presence of a Kayser-Fleischer-Ring it arose that a Kayser-Fleischer-Ring
could be detected in the bigger part of the symptomatic patients. Concerning
patients with neurological primary symptoms it could be found in almost all
cases. Because of the deliberation that other genetic variances could have
influence on severity code, primary symptoms and age of onset, a HFE- and an
APOE-genotyping has been done in many persons. There could not be seen a
coherence between changes in the HFE-gene and the phenotype. There are many
asymptomatic patients in the group of patients who carry the APOE-genotype
3/3; in addition to that the age of onset is later than in the group of
patients with another APOE-genotype.
