Das Ovarialkarzinom ist aufgrund seiner meist späten Diagnosestellung und
einer hohen Rezidivquote einer der gefährlichsten Tumorerkrankungen bei
Frauen. Bis heute gelingt es nicht, zuverlässige Aussagen über den
individuellen Krankheitsverlauf zu treffen und den Patientinnen ein darauf
abgestimmtes Therapieangebot zu machen. Die Methoden der Molekulardiagnostik
versprechen verbesserte Prognoseabschätzung und Hilfe bei
Therapieentscheidungen. Die vorliegende Arbeit soll dazu einen Beitrag
leisten. Mit Hilfe immunhistochemischer Analysen und Genanalysen wurden
Tumorproben hinsichtlich ihrer Genexpression bzw. Mutation zentraler Apoptose-
und Zellzyklusregulatoren untersucht. Die Ergebnisse wurden in einer
retrospektiven Analyse klinischen Daten gegenübergestellt und mit
statistischen Methoden auf signifikante Korrelationen hin untersucht. Es
zeigte sich im Wesentlichen, dass ein Verlust der pro-apoptotischen
Multidomänproteine Bax und Bok, nicht jedoch des homologen Bak, prognostisch
relevant wird, sobald der Zellzyklusregulator p21Cip/WAF-1 aktiviert ist.
Apoptosedefekte werden hier offensichtlich erst ausschlaggebend, wenn zentrale
Zellzykluscheckpunkte aktiviert sind. Die gemeinsame Analyse von p21Cip/WAF-1
mit Bax und Bok ermöglicht daher eine bessere Prognoseabschätzung. Desweiteren
fand sich eine bessere Prognose, wenn eine intakte Koexpression der CDK
Inhibitoren p16INK4a und p21Cip/WAF-1vorlag. Die Analyse des
p53-Mutationsstatus ergab jedoch in der monogenetischen Analyse kein
signifikantes Ergebnis hinsichtlich der Prognose. Diese Resultate der
Untersuchungen von Zellzyklusregulatoren des Rb-Signalwegs und
Apoptoseregulatoren des p53-Signalwegs legen nahe, dass molekulardiagnostische
Methoden nur dann einen Beitrag zur Prognose des Ovarialkarzinoms leisten,
wenn das Zusammenwirken mehrerer molekularer Marker betrachtet wird. Die
zugrundeliegenden Multigenanalysen ermöglichen, bei Vorliegen bestimmter
Genexpressionsprofile, hochsignifikante Schätzungen zur erwarteten
Überlebensdauer zu treffen. Mit Einzelgenanalysen, die den Schwerpunkt
früherer molekulardiagnostischer Arbeiten bildeten, ließen sich dagegen keine
derart signifikanten Prognosen zur Überlebensdauer stellen. Diese Arbeit
unterstreicht damit die Komplexität der Wechselwirkungen in der Tumorbiologie.
Zugleich werden die Potentiale der Molekulardiagnostik für die Prognose des
Ovarialkarzinoms deutlich.

dc.description.abstract
Ovarian carcinoma is of bad prognosis mostly because of late detection time
and a high number of relapse cases. It is still impossible to make reliable
statements about the individual course of disease and to develop case specific
individual treatments. From methods of molecular diagnosis optimized prognosis
and decision support for alternative therapies are expected. In this thesis,
based on immune histochemical and genetic analysis, tumour specimens were
analysed with respect to gene expression and mutation of important apoptosis
and cell cycle regulators. In a retrospective analysis the results were
compared with clinical data. Significant correlations were identified with
statistical methods. The results show that a loss of the pro-apoptotic multi
domain proteins Bax and Bok is of prognostic relevance if the cell cycle
regulator p21 is active. Apoptosis defects are obviously only relevant if
important cell cycle check points are active. Therefore the combined analysis
of p21 with Bax and Bok enables a more precise prognosis. It is also shown
that the combined analysis of p21 and Bak does not yield statistically
significant results. However, a better prognosis was also found when the CDK
inhibitors p16 and p21 were both intact. Finally, a monogenetic analysis
showed that the p53 mutational status does not influence the prognosis. The
results indicate for the cell cycle regulators of the Rb signal path and the
apoptosis regulators of the p53 signal path that methods of molecular
diagnostics can support the prognosis of ovarian carcinoma only if the
combined effect of several molecular markers is considered. The underlying
multigenetic analysis enables highly significant estimates about the expected
survival time. Monogenetic analysis where the focus was on in earlier works on
molecular diagnostic, do not enable such highly significant survival time
prognosis. This thesis underlines the complexity of tumour biology. The
potentials of molecular diagnosis for the prognosis of ovarian carcinoma are
shown.
