Purpose: The purpose of this study was to compare the value of multi-lesion
vs. single-lesion assessment for the prediction of overall survival (OS) in
patients with hepatocellular carcinoma (HCC) before transarterial
chemoembolization (TACE) using traditional diameter based and new 3D
quantitative tumor burden analysis. Methods and Materials: A total of 300
patients with HCC treated using TACE at the Johns Hopkins university hospital
were identified from a prospectively collected database. Patients were
excluded due to portal vein invasion (n=142), missing or insufficient MRI
(n=28) or infiltrative HCC (n=8). 122 patients were included into the final
data analysis. Arterial-phase, contrast-enhanced MRI acquired before TACE was
used to measure overall and enhancing diameters in all HCC lesions measured by
two independent board certified radiologists. A 3D segmentation analysis was
performed to assess total liver volumes and to quantify the enhancing tumor
volume (ETV [cm3]) for each lesion. Enhancing tumor burden (ETB [%]) was
defined as the ratio between ETV [cm3] and liver volume. Patients were
stratified into high and low tumor burden groups following the staggered
Barcelona Clinic Liver Cancer and Milan criteria. For this purpose all one-
dimensional thresholds were extrapolated using the formula: Volume = 4/3πr3.
OS prediction was estimated using Kaplan-Meier analysis as well as uni- and
multivariate cox proportional hazard ratios (HR). Concordances of each
assessment method measuring all and each lesions were calculated. The method
with the highest correlation was further evaluated in order to identify the
ideal number of lesions needed for an accurate prediction of OS. Results: All
radiological assessment methods achieved good separation of the Kaplan-Meier
survival curves (p<0.05). Multivariate analysis showed a HR of 5.2 (95%
confidence interval (CI), 3.1-8.8, p<0.001) for ETV [cm3] and HR 6.6 (95% CI,
3.7-11.5, p<0.001) for ETB [%] vs. HR 2.6 (95% CI, 1.2-5.6, p=0.012) for
overall diameter and HR 3.0 (95% CI, 1.5-6.3, p=0.003) for enhancing diameter.
Concordances were highest for ETB [%], with no added predictive power for
multi-lesion assessment. Conclusion: 3D-quantitative assessment is a stronger
predictor of survival as compared to diameter-based measurements in patients
with HCC treated with TACE. Assessing multiple lesions provides no substantial
improvement in predicting OS than evaluating the dominant lesion alone.

Einleitung: Ziel der Studie ist die Evaluation MRT-basierter 3D-
Analyseverfahren bei Patienten mit hepatozellulärem Karzinom (HCC) zur
präzisen Bestimmung der Tumorlast. Der Fokus liegt dabei auf der Beantwortung
der Fragen, ob dreidimensionale Analyseverfahren einen Vorteil gegenüber
etablierten ein- und zweidimensionalen Verfahren bieten und wenn ja wie viele
Tumorläsionen bei multifokalem HCC evaluiert werden müssen, damit eine valide
Aussage zum Überleben der Patienten getroffen werden kann. Methodik: Insgesamt
wurden 300 HCC-Patienten aus einer prospektiv geführten Datenbank des Johns
Hopkins University Hospital ermittelt. 122 Patienten wurden in die finale
Datenanalyse einbezogen. Ausschlusskriterien waren: 1. Portalveneninvasion
(n=142), 2. Fehlende oder unzureichende MRT Bildgebung (n=28) und 3. Invasiv
wachsendes HCC (n=8). Bei allen Patienten wurde die Erstdiagnose HCC gestellt
und im Verlauf eine Transarterielle Chemoembolisation (TACE) durchgeführt. Auf
Basis der MRT-Bildgebung bei Diagnose-stellung wurde eine 3D-gestützte
quantitative Analyse durchgeführt. Dabei wurden jeweils Enhancing Tumour
Volume (ETV [cm3]) sowie Enhancing Tumour Burden (ETB [%], das Verhältnis
zwischen ETV [cm3] und Lebervolumen) berechnet. Außerdem wurden Tumor-
durchmesser sowie Durchmesser der stärksten Kontrastmittelaufnahme von zwei
Radiologen unabhängig voneinander gemessen. Patienten wurden anschließend auf
der Basis der Barcelona Clinic Liver Cancer-Klassifikation und der Milan
Kriterien in zwei Gruppen eingeteilt. Die ursprünglich eindimensionalen
Schwellenwerte wurden hierfür mit Hilfe der Formel: Volumen = 4/3πr3
extrapoliert. Die statistische Datenanalyse beinhaltete die Erstellung von
Kaplan-Meier Überlebenskurven und die Berechnung der uni- und multivariaten
Cox Proportional Hazard Ratios (HR). Außerdem wurden Konkordanzanalysen
durchgeführt. Ergebnisse: Alle radiologischen Evaluationsmethoden erreichten
eine gute Separation der Kaplan-Meier Kurven (p<0,05). Die multivariate
Analyse ergab ein Hazard Ratio (HR) von 5,2 (95% Konfidenzintervall (KI)
3,1-8,8; p<0,001) für ETV [cm3] und ein HR von 6,6 (95% KI 3,7-11,5; p<0,001)
für ETB [%]. Dem gegenüber stehen ein HR von 2,6 (95 % KI 1,2-5,6; p=0,012)
für Tumordurchmesser und ein HR von 3,0 (95 % KI 1,5-6,3; p=0,003) für Durch-
messer der stärksten Kontrastmittelaufnahme. Das Konkordanzniveau war für ETB
[%] am höchsten. Des Weiteren konnte für eine multifokale Berechnung kein
signifikanter Nutzen im Hinblick auf eine Überlebensprognose festgestellt
werden. Schlussfolgerung: Quantitative 3D-Evaluationsmethoden stellen im
Vergleich zu etablierten ein- und zweidimensionalen Methoden einen stärkeren
und präziseren Prädiktor für das Überleben von Patienten mit HCC dar. Die
Evaluation mehrerer Tumorläsionen birgt gegenüber der Evaluation der
dominanten Tumorläsion keinen wesentlichen Vorteil.
