Die Elektrochemotherapie ist eine neue Form der Lokaltherapie solider Tumoren.
Nach vorheriger intratumoraler Zytostatikainjektion erfolgt im Intervall von
wenigen Minuten die lokale Applikation definierter Spannungsstöße. Durch
Anpassung der Impulsparameter kommt es zu einer reversiblen Störung der
Membranstruktur mit Erhöhung der Permeabilität für hydrophile Ionen, Proteine
und DNS-Fragmente, die sich entsprechend dem Konzentrationsgradienten
verteilen. Bleomycin ist in der Elektrochemotherapie als Zytostatikum
etabliert. Der hydrophile Molekülcharakter ermöglicht eine auf das
Elektroporationsareal beschränkte, intrazelluläre Anreicherung. Folge ist eine
lokale Erhöhung der Toxizität bei reduzierter Dosis. Das Grünteekatechin
Epigallokatechingallat ist auf Grund seiner tumorpräventiven und
zytostatischen Eigenschaften Gegenstand zahlreicher Untersuchungen. Das
günstige Nebenwirkungsprofil macht die Substanz für den Einsatz in der
Elektrochemotherapie interessant. In Vorversuchen wurde die Reduktion des
Wachstums von KB-Tumorzellen durch Elektrochemotherapie mit
Epigallokatechingallat im Vergleich zu Bleomycin, Epikatechin und einer
Kontrollgruppe untersucht. Für Epigallokatechingallat sowie Bleomycin wurde
eine signifikante Wachstumssuppression nachgewiesen. Im in-vivo-Modell mit 48
T-zelldefizienten Mäusen (Typ NMRI) wurden in zwei Versuchsreihen
Plattenepithelkarzinome (human, Zunge) erzeugt und nach Erreichen einer Größe
von 0,6 bzw. 1,0 cm3 in sechs Versuchsgruppen aufgeteilt. Den Versuchstieren
wurde PBS (Gruppe A und B), Bleomycin (Gruppe C und E) oder
Epigallokatechingallat (Gruppe D und F) intratumoral injiziert. Die Tumore der
Gruppen B, C und D wurden 10 min nach Injektion zusätzlich einer
Elektroporation unterzogen. Im vierwöchigen Nachbeobachtungszeitraum wurde der
Wachstumsverlauf der Tumoren dokumentiert. An Tag 28 erfolgte die Tötung der
Versuchstiere und Explantation der Tumore mit histologischer Aufarbeitung.
Unabhängig von der Elektroporation zeigten die Gruppen mit PBS- und
Epigallokatechingallatbehandlung (Gruppen A, B, D, F) eine Tumorprogression
(181 – 232,5 % der Ursprungsgröße). Die Injektion von Bleomycin ohne
anschließende Elektroporation (Gruppe E) führte zu einer Stagnation der
Tumorgröße (106,5 % der Ursprungsgröße). Zu einer signifikanten Verkleinerung
mit zum Teil kompletter Remission (4,5 % der Ursprungsgröße) kam es unter der
Elektrochemotherapie mit Bleomycin (Gruppe C). Der Antikörper MIB1 belegte in
Gruppe E eine Restaktivität und in Gruppe C keinerlei Teilungsaktivität der
Tumorzellen. Die in-vivo-Ergebnisse bestätigen für Bleomycin eine
Wirkungsverstärkung durch Elektroporation. Für Epigallokatechingallat konnte
weder in der Elektrochemotherapie noch für die Reinsubstanz eine signifikante
antitumorale Wirkung nachgewiesen werden. Die Elektroporation hat in
Kombination mit Bleomycin zu einer deutlich Wirkungsverstärkung geführt, was
für eine geeignete Auswahl der Parameter der Elektroporation spricht.
Unterschiede finden sich in der Pharmakodynamik. Der Wirkmechanismus der
Katechine ist nicht vollständig geklärt. Es gibt Hinweise auf intra- und
extrazelluläre Signaltransduktionswege. Würde die antitumorale Wirkung durch
extrazelluläre Rezeptorbindung vermittelt (z.B. tNOX, LR1), führte eine
Elektroporation zu keiner Wirkungsverstärkung. Als weitere Ursache für eine
fehlende in-vivo-Wirksamkeit des Katechins sind noch ungeklärte interstitielle
sowie zytosolische Interaktionen in Betracht zu ziehen. Die
Elektrochemotherapie mit Bleomycin ist eine neue tumorspezifische und
nebenwirkungsarme Möglichkeit der lokalen Tumortherapie. Inwieweit Katechine
als zukünftiges antitumorales Substrat geeignet sind, konnte in dieser Arbeit
nicht abschließend geklärt werden. Die durchgeführten Vorversuche lassen einen
positiven Effekt vermuten

Introduction: Traditional chemotherapy is often limited by a poor
intracellular drug uptake combined with relevant side effects.
Electrochemotherapy allows a high intracellular uptake at low local dosage. On
our search for a new cytotoxic electroporation agent we examined the
efficiancy of the green tea ingredient epigallocatechingallat (EGCG). Methods:
48 nude mice with implanted oral scquamous cell cancer. Treatment in 6 groups
(8 animals) with EGCG, Bleomycin, Buffer solution or in combination with
electrochemotherapy. After four weeks documantation of tumor growth and
pathological evaluation. Results: As known from clinical results the
electroporation with bleomycin led to a significant reduction of tumor volume.
However the electroporation with EGCG showed no effect on the tumor growth.
The pathological report (HE, MIB-1) confirmed our findings. Discussion:
Contrary to our in vitro findings the in vivo chemotherapy with
epigallocatechingallat did not reduce the tumor growth. After discussing
different parameters we identified three possible reasons: 1) due to high
redox potential quick inactivation of EGCG, 2) possible extracellular mediated
cytotoxic effect would not profit from electroporation, 3) quick declining
interstitial EGCG concentration.
