Das Glaukom ist ein Missverhältnis zwischen Augeninnendruck (IOD) und
Sehnervenresistenz, das mit Papillenexkavation und charakteristischen
Gesichtsfeldausfällen (glaukomatöse Optikusneuropathie) einhergeht. Es steht
in industrialisierten Ländern nach der altersabhängigen Makuladegeneration und
Diabetes mellitus an dritter Stelle aller Erblindungsursachen. Die Erhöhung
des IOD ist einer der wichtigsten Risikofaktoren der Sehnervendefekte und
stellt die einzige, therapeutisch beeinflussbare Größe dar. Verantwortlich für
die IOD-Erhöhung sind Abnormitäten des Kammerwasserabflusses über den
Kammerwinkel. 85% des Kammerwassers fließen über das Trabekelmaschenwerk (TM)
ab. Die häufigste Form des Glaukoms ist das Primäre Offenwinkelglaukom (POWG),
welches durch Abflussstörungen über das TM gekennzeichnet ist. TM ist ein
kontraktiles, aktiv an der IOD-Regulation beteiligtes Gewebe. Seine Relaxation
führt zur Herabsetzung des Kammerwasserabflusswiderstandes und zur IOD-
Senkung. Zwischen ihm und dem Ziliarmuskel (ZM) besteht ein funktioneller
Antagonismus; Relaxation und Kontraktion beider Gewebe wirken sich jeweils
gegensinnig auf den IOD aus. Folglich soll ein ideales Pharmakon zur IOD-
Senkung das TM relaxieren und ZM, zur Vermeidung von Nebenwirkungen, wenig
oder nicht beeinflussen. Ein Pharmakon müsste also in Kontraktionsmechanismen
eingreifen, die nur in TM, nicht aber in ZM wirksam sind. Dies scheint für
Kontraktionsregulationsmechanismen zuzutreffen, die auf Ca2+-Sensitivierung
beruhen und aus glatter Muskulatur bekannt sind. Die Hypothese der
vorliegenden Arbeit ist daher gewesen, dass diese Regulationsmechanismen durch
Ca2+-Sensitivierung auch in TM Kontraktionen vermitteln. Western-Blot-Analysen
und Kontraktionsexperimente haben folgende Resultate ergeben: ·Das
Kontraktionsprotein Myosin ist in humanem und bovinem TM nachweisbar. ·Das
Aktivierungsenzym von Myosin, die Myosin-Light-Chain-Kinase (MLCK), ist in
humanem und bovinem TM enthalten. ·Phosphoserine der MLCK sind in humanem und
bovinem TM nachweisbar. ·Myosin und MLCK liegen in humanem und bovinem TM
assoziiert vor. ·Der spezifische MLCK-Inhibitor ML-7 inhibiert Endothelin-1
(ET1) und Cabachol-induzierte Kontraktionen des TM. Daraus lassen sich
folgende Schlüsse ziehen: 1\. Die Ergebnisse belegen zusammen mit vorherigen,
dass TM alle wesentlichen Elemente eines vollständig ausgeprägten
glattmuskulären Kontraktionssystems besitzt und somit die Grundvoraussetzung
für die Existenz auf Ca2+-Sensitivierung beruhender Kontraktions-mechanismen
erfüllt. 2\. Die Protein-Nachweise in bovinem TM bestätigen Ergebnisse von
zahlreichen Kontraktionsexperimenten, die auf der Annahme von kalzium-
sensitivierbaren glattmuskulären Systemen im Trabekelwerk beruhen. 3\. Die
Hypothese der Kontraktionsregulation durch Ca2+-Sensitivierung in TM wird
durch alle Ergebnisse unterstützt. Eine pharmakologische Beeinflussung der
Regulation durch Ca2+-Sensitivierung ist demnach für eine therapeutische IOD-
Senkung bei POWG als Alternative der gegenwärtigen neben-wirkungsreichen
Glaukomtherapie in Erwägung zu ziehen. Prinzipiell kommen der selektive ROCK-
Inhibitor Y-27632 oder ähnlich wirkende Substanzen als geeignete Pharmaka in
Frage. Es bedarf weiterer intensiver Forschung, um im Hinblick auf eine
verbesserte Therapie des Glaukoms ein detaillierteres Verständnis der
Kontraktionsregulation im TM, insbesondere durch Ca2+-Sensitivierung, zu
gewinnen.

dc.description.abstract
Glaucoma is characterised by inbalance of intraocular pressure (IOP) and optic
nerve resistance. Findings comprise excavation of the optic nerve disk and
characteristic visual field defects (glaucomatous neuropathy of the optic
nerve). In industrialised countries glaucoma is the third commonest cause of
blindness after age related macula degeneration and diabetes mellitus.
Excessive IOP is one of the most important risk factors for optic nerve damage
and represents the only avenue for therapeutic interaction. Abnormalities of
the aqueous humor outflow via the chamber angle are responsible for the
increase of IOP. 85% of aqueous humor outflow passes the trabecular meshwork
(TM). Primary open angle glaucoma (POAG) is the most frequent form of glaucoma
and is characterised by obstructions of outflow facilities via the TM. TM is a
contractile tissue which is actively participating in the regulation of IOP.
Relaxation of TM induces lowering of outflow resistance and decrease of IOP.
Among TM and the ciliary muscle (CM) a functional antagonism exists;
relaxation and contraction of both tissues cause inverse action of the IOP.
Consequently the optimal drug for lowering IOP should induce relaxation of TM
but should not influence CM. Accordingly this drug should influence mechanisms
of contraction that are effective in TM but not or only little in CM. This
seems to be applicable for mechanisms of contraction which are based upon
Ca2+-sensitisation and which are known from research on smooth muscles. The
hypothesis of this dissertation was that mechanisms of regulation based upon
Ca2+-sensitisation mediate contractions also in TM. Immunoblot-analyses and
contraction experiments showed the following results: ·The contraction protein
Myosin is detectable in human and bovine TM. ·Human and bovine TM contains
Myosin-Light-Chain-Kinase (MLCK), the enzyme for activation of Myosin.
·Phosphoserine-domains of MLCK are detectable in human and bovine TM. ·Myosin
and MLCK molecules are associated in human and bovine TM. ·The specific MLCK-
inhibitor ML-7 inhibits contractions of TM induced by endothelin-1 (ET-1) and
cabachol. These results lead to the following conclusions: 1\. Together with
previous the above results show that TM features all essential elements of a
definitive smooth muscle contraction system. Consequently TM fulfills the
basic requirement for contraction regulation based upon Ca2+-sensitisation.
2\. The protein detection in bovine TM confirms results of numerous
experiments based upon the assumption of smooth muscle systems regulated by
Ca2+-sensitisation. 3\. All results support the hypothesis of contraction
regulation by Ca2+-sensitisation in TM. According to this a pharmacological
intervention in the regulatory system based upon Ca2+-sensitisation as a
therapeutic decrease of IOP in POAG could be considered as an alternative to
the current management in glaucoma which is associated with numerous side
effects. In principle substances like the selective ROCK-inhibitor Y-27632 or
similarly acting compounds may be considered as appropriate drugs. For better
treatment of glaucoma further research is required in order to achieve a more
detailed understanding of contraction regulation based upon
Ca2+-sensitisation.
