Kenntnisse der pharmakogenetischen Hintergründe der Enzymexpression sind nicht nur von Bedeutung für die Optimierung der individuellen Therapie durch Verbesserung der Effektivität und Verringerung der Gefahr schädlicher Nebenwirkungen, insbesondere bei Wirkstoffen mit geringer therapeutischer Breite. Sie sind auch durch ihre Beteiligung an endogenen Stoffwechselvorgängen von Bedeutung. Ein Beispiel ist das Isoenzym Cytochrom P450 3A5 (CYP3A5), das bei Kaukasiern polymorph in Abhängigkeit vom CYP3A5*1/*3-Polymorphismus exprimiert wird. Hierbei führt das bei Kaukasiern häufigere Major-Allel CYP3A5*3 zu einer Splicevariante, die mit einer nur geringen oder fehlenden CYP3A5-Expression einhergeht. Dagegen ist das Minor- Allel CYP3A5*1 mit einer hohen CYP3A5-Expression verbunden. Im Gegensatz zu CYP3A4 wird CYP3A5 ebenfalls in der Niere exprimiert und stellt hier das vorherrschende CYP3A-Enzym dar. Somit besteht durch die polymorphe Expression von CYP3A5 nicht nur ein potentieller Einfluss auf die Metabolisierung von Wirkstoffen, die über CYP3A-Enzyme verstoffwechselt werden, sondern auch ein möglicher Einfluss auf den Steroidmetabolismus, sowie die Natriumresorption und damit auf die Blutdruckregulation. Ziel der vorliegenden Arbeit war daher die Untersuchung des CYP3A5-Isoenzyms hinsichtlich seiner Bedeutung für die Pharmakotherapie mit Cyclosporin nach Nierentransplantation, sowie seines Einfluss bei der Blutdruckregulation. Dabei ist es nicht gelungen einen Einfluss des CYP3A5*1-Allels auf Dosierung und Talspiegelkonzentration von Cyclosporin zu bestätigen. Auch konnte kein Einfluss auf die Funktion oder das Überleben des Transplantats festgestellt werden. Jedoch zeigte sich ein signifikant protektiver Effekt hinsichtlich des Patientenüberlebens, der auch nach Transplantatverlust erhalten blieb. Die Überlebensanalyse einer Population aus Dialysepatienten ohne Cyclosporintherapie zeigte dagegen keinen Einfluss des CYP3A5*1-Allels auf das Überleben. Damit konnte gezeigt werden, dass der CYP3A5*1/*3-Polymorphimsus, auch wenn er keine Bedeutung für die direkte Cyclosporintherapie besitzt, von Bedeutung für die Prognose nach Transplantation ist. Hierbei sind Effekte über den Metabolitstoffwechsel von Cyclosporin denkbar. Zur Klärung der Bedeutung der CYP3A5-Expression für die Cyclosporintherapie und vor allem hinsichtlich seines Einfluss auf den Metabolitstoffwechsel und das Patientenüberleben erscheinen daher weitere Untersuchungen nötig. Die Untersuchung des Einfluss von CYP3A5 und des CYP3A5*1/*3-Polymorphimsus auf den Blutdruck in der Allgemeinbevölkerung zeigte einen signifikant niedrigeren systolischen Blutdruck bei Trägern des CYP3A5*1-Allels. Obwohl der Effekt in einer geschlechtsabhängigen Analyse nur noch bei weiblichen Personen signifikant war, bestätigen die Ergebnisse einen Zusammenhang zwischen der CYP3A5-Expression und dem Blutdruck und unterstützen die Hypothese eines protektiven Mechanismus durch CYP3A5. Auch wenn die Bedeutung für die klinische Praxis gering ist, stellt CYP3A5 somit einen Faktor dar, der die Blutdruckregulation beeinflusst und dabei hilft, das komplexe Bild der Regelmechanismen besser zu verstehen. Vor diesem Hintergrund erscheinen weitere Untersuchungen zur Bedeutung der CYP3A5-Expression für die Blutdruckregulation und die Klärung der zugrunde liegenden Mechanismen angebracht. Dabei können neben klinische Studien auch experimentelle Untersuchungen im Tiermodell, zum Beispiel bei SHR und WKY, sowie gezielte quantitative Analysen auf mRNA- und Proteinebene, dazu beitragen die Bedeutung der CYP3A-Isoenzme für die Blutdruckregulation besser zu verstehen.
Knowledge of the genetic background of polymorphic expression of enzymes involved in drug metabolism is important for the improvement of individual therapy by enhancement of efficacy and decrease of risk of side effects. This is in particular relevant for agents with a small therapeutic index. An example is the isoenzyme Cytochrome P450 3A5 (CYP3A5), which is polymorphically expressed by Caucasians depending on the CYP3A5*1/*3 polymorphism. The major allele CYP3A5*3 cause a splice variant which disturbs correct splicing of transcripts and leads to low protein expression and enzyme activity. In contrast, the minor allele CYP3A5*1 is associated with high expression of CYP3A5. Unlike the CYP3A4 enzyme, CYP3A5 enzyme is also expressed in the kidney and represents the dominant renal CYP3A enzyme. Hence, the polymorphic expression of CYP3A5 suggests not only a potential impact on the metabolism of drugs, which are metabolized by CYP3A enzymes, but also a possible influence on renal function and blood pressure regulation by influencing steroid metabolism, and sodium reabsorption. Therefore, the object of the present dissertation was the study of the CYP3A5 isoenzyme and its relevance for the pharmacotherapy with cyclosporine after kidney transplantation and its influence on blood pressure regulation. Although an effect of the CYP3A5*1 allele on dosage and trough level of cyclosporine as well as function and survival of the graft could not be detected, a significant protective effect regarding the overall survival of patients was observed. In contrast, the survival analysis of a population consisting of non-transplanted dialysis patient without cyclosporine therapy did not show any effect of the CYP3A5*1 allele on the patient survival. In the current work, it was possible to demonstrate, that the CYP3A5*1/*3 polymorphism confers a survival benefit in kidney transplant patients that are chronically treated with a cyclosporine-based regimen. At this, effects due to an additional effect on cyclosporine metabolism and the concentration pattern of cyclosporine metabolites are possible. Further research is necessary to clarify the relevance of CYP3A5 expression for cyclosporine therapy and its influence on the cyclosporine metabolism as well as the survival of patients. The study of the influence of CYP3A5 and the CYP3A5*1/*3 polymorphism on blood pressure in the general population demonstrated a significant lower systolic blood pressure for individuals with the CYP3A5*1 allele. Although a gender specific analysis showed that the effect remains only significant for females, the results confirm a connection between CYP3A5 expression and blood pressure and support the hypothesis of a protective mechanism by CYP3A5. Even if the relevance for clinical practice is little, CYP3A5 represents a factor which influences blood pressure regulation and is of assistant for a better understanding of the complex blood pressure regulation. In this regard, further studies of the relevance of CYP3A5 expression for blood pressure regulation and clarifying of the mechanism are required.
