Patienten mit chronischen Erkrankungen haben einen hohen Bedarf an
psychotherapeutischer Unterstützung. Die Effektivität von Bibliotherapie - das
Lesen von Texten zu therapeutischen Zwecken - wird in Metaanalysen
übereinstimmend im mittleren Bereich (d = 0,565 - 0,84) angegeben. Für viele
Therapeuten ist Bibliotherapie eine wichtige Komponente ihrer Therapie - auch
ohne bisherigen empirischen Wirkungsnachweis. Hauptfragestellung der Studie
war, ob die speziell für die untersuchte Stichprobe erstellten
bibliotherapeutischen Materialien zu einer Behandlungsintensivierung und
-ökonomisierung innerhalb einer Verhaltenstherapie beitragen können? Methode:
Eine randomisierte kontrollierte Studie wurde an 377 Patienten in der
Abteilung Verhaltenstherapie und Psychosomatik / Rehabilitationsklinik Seehof
durchgeführt. Bezüglich wichtiger Störvariablen (Alter, Bildung,
Psychopathologie und Intelligenz der Patienten sowie Geschlecht, Ausbildung
und Erfahrung der Therapeuten) war die Randomisierung erfolgreich. Die
Bibliotherapiegruppe erhielt zusätzlich zur Routinetherapie mindestens eine
von sechs Broschüren nach klinischen Erwägungen ihres Therapeuten und es
verblieb dem Patienten genügend Zeit (MTage= 31, SD = 9.7), sich damit
auseinanderzusetzen. Die Kontrollgruppe erhielt keine von diesen sechs
Broschüren. Ergebnisse: Bibliotherapie ist eine Methode, die additiv zu einer
Psychotherapie und psychosomatischen Rehabilitation das Krankheits- und
Bewältigungswissen erhöhen kann (r = .40). Vor allem Männer (r = .20) und
diejenigen mit einer abgeschlossenen 10-12 jährigen Schulbildung (r = .17)
profitierten davon. Jedoch führte das Lesen und Wissen um funktionale
Bewältigungsstrategien nicht zu einer Veränderung im selbsterlebten Umgang mit
der eigenen Krankheit (p < .05) und die Broschüren hatten wenn, dann einen
negativen Einfluss auf die Therapiebeziehung und den Therapieprozess sowohl
aus der Sicht der Patienten als auch der Therapeuten. Auch hatte die
Bibliotherapie einen negativen Einfluss auf einige Behandlungsergebnismaße in
der Veränderungsbeurteilung und in der Beurteilung des Reha-Erfolges aus Sicht
der Therapeuten. Die Ergebnisse deuten darauf hin, dass die Patienten, die
bibliotherapeutische Informationen erhalten hatten, sowohl die Veränderungen
in verschiedenen Bereichen als auch den Reha-Aufenthalt und das Reha-Ergebnis
überwiegend schlechter beurteilten als die Patienten, die keine Broschüren
erhalten hatten. Weiterhin sprechen die Daten dafür, dass die negativen
Reaktionen durch Bibliotherapie in den Behandlungsergebnismaßen und der
Beurteilung der Therapiesitzung abhängig sind von der Schulbildung. Die
negativen Effekte sind stärker je niedriger die Schulbildung der Leser ist.
Unter Berücksichtigung der Beurteilung der Therapiesitzung als Kovariate - aus
Patienten wie auch aus Therapeutensicht - hatte die Bibliotherapie überwiegend
keinen, die Kovariate dagegen einen signifikanten Einfluss auf das
Behandlungsergebnis. Zusammenfassung: Trotz der Vorliebe vieler Therapeuten
ihren Patienten Lesematerialien zu empfehlen, kann nach den Ergebnissen dieser
Studie, eine Informierung innerhalb einer Verhaltenstherapie durch
schriftliche Lesematerialien nicht uneingeschränkt empfohlen werden. Sie
sollte im Einzelfall therapeutisch indiziert, mit großer therapeutischer
Sorgfalt und unter Abwägung der Gründe, die gegen einen Einsatz sprechen,
angewendet werden. Bei Ausbleiben der angestrebten Wirkungen oder einer
Veränderung in der Therapeut-Patient-Beziehung muss eine Analyse des
therapeutischen Misserfolges empfohlen werden. Die Studie zeigt, dass gute
therapeutische Intentionen alleine nicht sicherstellen, dass es auch zu guten
therapeutischen Wirkungen kommt und Patientenaufklärung nicht zwangsläufig zu
einer Verbesserung der Therapie (-beziehung) und des Behandlungsergebnisses
führt als auch das Patientenempowerment verbessert.

Background Patients with chronic diseases have a high necessity of
psychotherapeutic assistance. The effectivity of bibliotherapy – reading texts
for psychotherapeutic reasons – is depicted in metaanalysis in a middle range
(d = 0.565 – 0.84). Bibliotherapy is an important component in the therapy of
many therapists – even without empiric evidence. Aim This study aims to
examine whether or not the bibliotherapeutic material created especially for
the representative sample increases the efficiency and intensifies the
treatment in behavioral therapy. Method A randomized, controlled study was
conducted with 377 patients of the department of behavioral therapy and
psychosomatic at the Rehabilitation Centre Seehof. The randomization was
successful in reference to age, education, psychopathology and intelligence of
the patient and sex, training and experience of the therapists. The
bibliotherapy group obtained in addition to the routine therapy at least one
of six brochures which the therapist had chosen. The patients had sufficient
time (MDays = 31, SD = 9.7) to read the brochures. Results Bibliotherapy is a
method which can, in addition to a psychotherapy and a psychosomatic
rehabilitation, increase the knowledge about the disease and the best way to
cope with it (r = .40). Especially men (r = .20) and those with a completed
10-12 year education (r = .17) benefited. However, the reading and knowledge
of functional coping strategies did not lead to a change in personally dealing
with their own illness (p < .05). The brochures had, if at all, a negative
influence on the therapeutic relationship and the therapy process, both from
the perspective of the patient and the therapist. The bibliotherapy also had,
if, a negative influence on some therapy outcome measures assessed by the
therapists. The results suggest that patients who received bibliotherapeutic
information considered the changes in various areas as well as the stay in the
rehabilitation centre and their outcome more negatively than the patients who
had received no brochures. The data further suggest that the adverse reactions
to the therapeutic outcome and the assessment of the therapy session owing to
the bibliotherapy are dependent on the school education. The negative effects
increase with lower educational levels of the readers. Taking into account the
assessment of the therapy session as a covariate – from the perspective of the
patients and the therapists – bibliotherapy did not have a significant effect
on the treatment outcome; the covariate, however, did. Conclusion Despite the
preference of many therapists to recommend reading materials to their
patients, the results of this study suggest that the addition of written
reading material within a behavioral therapy should not be recommended
unrestrictedly. It should be therapeutically indicated in each individual case
and be applied with great care under consideration of the reasons that speak
against its application. In absence of the desired effects or a change in the
therapist-patient relationship, an analysis of the therapeutic failure must be
recommended. The study shows that to ensure good therapeutic effects, good
therapeutic intentions are not sufficient on their own, and that informing
patients does not necessarily improve the therapy (and therapeutic
relationship), the outcome of the therapy, or the patient empowerment.
