In der vorliegenden Arbeit wurde die Effektivität der multimodalen Therapie an
der Medizinischen Klinik mit Schwerpunkt Psychosomatik der Charité ̶
Universitätsmedizin Berlin geprüft. Die Teilnahme an einer mindestens
siebentägigen Therapie und ein chronisches Schmerzsyndrom der Patienten waren
die Voraussetzungen für die Einbeziehung der Patientendaten in die
retrospektive Studie. Die zur Auswertung herangezogenen Fragebögen mussten zum
Aufnahme- und Entlassungszeitpunkt ausgefüllt sein. Folgende Fragebögen wurden
ausgewertet: GBB, BSF, PHQ, PSQ, SF-8. Der Fragebogen SOC wurde nur zum
Aufnahmezeitpunkt ausgefüllt. Somit konnten wir Aussagen zum Verlauf der
Therapie bezüglich der Parameter "Beschwerdedruck", "Stimmung","
Gesundheitsbezogene Lebensqualität", "Belastung" und "Depressivität" treffen.
Ein halbes Jahr nach dem stationären Aufenthalt wurden den Patienten
routinemäßig die Fragebögen SF-8 und PSQ mit der Post zugeschickt. Die
Nachhaltigkeit der Therapie bezüglich der gesundheitsbezogenen Lebensqualität
und der subjektiv empfundenen Belastung konnte somit überprüft werden.
Zusätzlich untersuchten wir zwei mögliche Prädiktoren für den Therapieerfolg,
den BMI und den Chronifizierungsgrad nach dem Mainzer Modell (nach
Gerbershagen) (MPSS). Überprüft wurde, inwiefern es Unterschiede zwischen den
BMI-Gruppen (< 25, 25-29,9, > 30) bzw. den Chronifizierungsgrad-Gruppen (Grad
II, Grad III) im Therapieverlauf gab. Zur Beurteilung wurden folgende
Determinanten herangezogen: Beschwerden (GBB), Depressivität (PHQ) und die
gesundheitsbezogene Lebensqualität (SF-8). Durch die multimodale Therapie
konnten depressive Störungen (PHQ) besonders gut gelindert werden. Die
mittlere Effektstärke (d = 0,6) unterstreicht die klinische Relevanz dieses
Ergebnisses. In den anderen Bereichen wurden ebenfalls gute und signifikante
Ergebnisse erzielt. Mit kleinen Effekten konnten einerseits die Belastungen
und die Beschwerden gesenkt und andererseits die Stimmung und die
gesundheitsbezogene Lebensqualität gesteigert werden. Die Patienten
profitierten sogar ein halbes Jahr nach dem stationären Aufenthalt von der
Therapie. Die körperliche Gesundheit (SF-8 PCS) konnte in diesem Zeitraum
nochmals signifikant verbessert werden. Die subjektiv empfundene Belastung
(PSQ) sank durch die Therapie kurzfristig, stieg aber nach sechs Monaten
wieder an. Zwischen den BMI-Gruppen ließen sich im Unterschied zu einer
vergleichbaren Studie keine signifikanten Unterschiede im Therapie-Outcome
feststellen. Zwischen den Chronifizierungsgrad-Gruppen war die Veränderung der
körperlichen Lebensqualität hingegen signifikant. Die Patienten mit einem
Chronifizierungsgrad III profitierten stärker von der Therapie, als Patienten
mit einem Grad II. Bezogen auf die Beschwerden, die Depressivität und die
mentale Gesundheit konnten keine Unterschiede festgestellt werden.
Zusammenfassend lässt sich feststellen, dass die Studienergebnisse die
Effizienz einer auch relativ kurzen stationären multimodalen Therapie
bestätigen. Die chronischen Schmerzpatienten hatten nach der Therapie weniger
Beschwerden, waren weniger depressiv, fühlten sich nicht mehr so stark
belastet und konnten sowohl ihre gesundheitsbezogene Lebensqualität als auch
ihre Stimmung steigern. Die Therapie senkt somit nicht nur Schmerz-Parameter,
sondern wirkt sich auch entsprechend dem biopsychosozialen Modell positiv auf
die psychischen Parameter aus. Insgesamt steigt die Lebensqualität der
Patienten an.

dc.description.abstract
In the present work the effectiveness of the multimodal therapy was examined
at the Medical Department, Division of Psychosomatic Medicine of the Charité
Berlin. The participation in an at least 7-day therapy and a chronic pain
syndrome were the conditions for the inclusion of the patient’s data in the
retrospective study. The following questionnaires had to be completed by the
patients at admission and dismissal: the Giessen Subjective Complaints List,
the Berlin Mood Questionnaire, the Prime MD Brief Patient Health Questionnaire
(PHQ-D), the Perceived Stress Questionnaire (PSQ-20) and the Short Form-8
Health Survey (SF-8). The Sense Of Coherence-Questionnaire (SOC-9) was only
completed on the day of admission. On this basis we could assess the
effectiveness of the multimodal therapy programme with regard to the
parametres ”pressure caused by complaints“, “mood“, “health-related quality of
life“, “stress“ and “degree of depression“. The questionnaires SF-8 and PSQ-20
were sent to the patients again half a year after the day of dismissal.
Therefore the sustainability of the therapy with regard to the health-related
quality of life and strain could be verified. In addition, differences in the
therapy process were examined based on the Body Mass Index (BMI) and the
Mainzer Pain Staging System (MPSS). The following determinants were used as
basis for the assessment: discomfort, degree of depression and the health-
related quality of life. The application of the multimodal pain therapy proved
particularly effective with regard to depressive disorders, but also resulted
in significant improvements in almost all other areas. The physical discomfort
and the individual perception of daily burden and strain could be lowered.
Furthermore, the health-related quality of life could be increased and the
physical health even increased again half a year after the hospitalisation.
The individual perception of daily burden could be temporarily lowered by the
therapy, but increased again six months after the therapy. No differences
regarding the outcome of the therapy could be found between the different BMI-
groups. However, it was found that the therapy could achieve significantly
stronger improvements of the physical condition of patients with Mainzer pain
stage III, than of patients with Mainzer pain stage II. With regard to the
complaints, the degree of depression and the mental health no differences
could be found between these two groups. In conclusion, it can be asserted
that the multimodal therapy is an effective method for the treatment of
patients with chronic pain disorders. After the therapy the patients with
chronic pain had fewer complaints, were less depressed, did not feel as
strongly burdened anymore and could increase their health-related quality of
life as well as their mood. The therapy therefore does not only lower the pain
parameters, but in accordance with the biopsychosocial model also positively
influences the psychic parameters. In conclusion, it can be said that the
quality of life of the patients has increased as a result of the therapy.
