Einige hämorrhagische Fieberviren, wie beispielsweise das Lassa Virus, CCHFV
oder das Ebola und Marburg Virus sind hochkontagiös und gehen mit einer hohen
Letalitätsrate einher, weshalb sie von besonderer Bedeutung für das klinische
Management und den Bevölkerungsschutz sind. In Deutschland haben diese,
vornehmlich in den Tropen und Subtropen verbreiteten, Infektionen vor allem
aufgrund des zunehmenden Tourismus an Bedeutung gewonnen, durch den es in den
letzten Jahren vereinzelt zum Import nach Deutschland kam. Durch ihre relativ
unspezifische Anfangssymptomatik ist eine Abgrenzung zu anderen
Infektionskrankheiten, wie beispielsweise Malaria oder einer Infektion mit
einem nicht isolierbedürftigen hämorrhagischen Fiebervirus anhand des
klinischen Bildes häufig schwierig. Um daher im Verdachtsfall angemessen
reagieren zu können, werden große Ansprüche an die laborgestützte Diagnostik
gestellt. So muss zum einen innerhalb kürzester Zeit eine große Anzahl
wichtiger Differentialdiagnosen abgeklärt werden. Zum anderen muss eine
zuverlässige virologische Diagnostik unter Hochsicherheitsbedingungen
erfolgen, auf deren Basis eine begründete Verdachtsdiagose gestellt bzw. eine
Infektion ausgeschlossen werden kann. Dem entsprechend wurde im Rahmen dieser
Arbeit ein Screeningverfahren für hämorrhagische Fieberviren entwickelt,
welches die konventionelle Polymerase-Ketten-Reaktion mit der
Pyrosequenzierung kombiniert, um die Nukleinsäuren der einzelnen Erreger
schnell, sensitiv und spezifisch zu detektieren und diese in der
anschließenden Sequenzierung zu typisieren. Da es sich um ein
nukleinsäurebasiertes Nachweisverfahren handelt, eignet es sich vor allem in
der frühen Phase einer Infektion als Diagnostikmethode. Es ist universell auf
eine Vielzahl differentialdiagnostisch wichtiger Erreger sowohl viraler als
auch bakterieller und parasitärer Genese anwendbar, die durch die
Vereinheitlichung der Temperaturprofile sowie der Kombination einzelner PCR-
bzw. PSQ-Assays parallel getestet werden können. Über die Etablierung interner
Kontrollen sowie die Konstruktion spezieller Kontrollplasmide wurde bekannten
Problemen in der PCR-Diagnostik begegnet und die Zuverlässigkeit des
entwickelten Verfahrens weiter erhöht. Insgesamt bietet somit das entwickelte
Screeningverfahren den Ansatz einer sensitiven, spezifischen und auch
zuverlässigen labordiagnostischen Abklärung verschiedener hämorrhagischer
Fieberviren sowie wichtiger Differentialdiagnosen mit nur einem Verfahren,
wodurch vor allem Zeit, Kosten, Reagenzien und auch Probenmaterial gespart
werden können.

Certain hemorrhagic fever viruses, as for example the Lassa virus, Crimean-
Congo-HF-virus and Ebola and Marburg virus, are highly contagious and are
associated with a very high rate of mortality; thus these viral infections
have a great impact on both the clinical management of patients, as well as
the protection of public health. Originating largely from tropical or
subtropical regions, these viral fevers have grown in importance in Germany
due to the increase in tourism abroad, and have in several cases been imported
into the country. Because the infections share relatively unspecific initial
symptoms, it is quite difficult to differentiate them from other infectious
diseases according to their clinical symptoms, such as malaria or infections
caused by non-transmissible between humans hemorrhagic fever viruses. Hence,
to be able to react appropriately in case of a suspicious case, great demands
are put into the laboratory diagnosis: On one hand, a large number of
differential diagnoses have to be ruled out in a short space of time; on the
other, a reliable virological diagnosis has to be performed under conditions
of high biosafety level, on the basis of which a reasonable final diagnosis
can be established. Accordingly, this study has come up with a method of
screening for hemorrhagic fever viruses, which combines the conventional
technique of polymerase chain reaction with pyrosequencing, with the aim of
being able to quickly, sensitively and specifically detect the nucleic acids
of individual viral agents, and to subsequently typify them during the
following DNA-sequencing. Since this procedure of verification is based on
viral nucleic acids, it is particularly useful as a diagnostic tool in the
early stages of infection. The procedure is universally applicable for a
multitude of diagnostically significant infectious agents both of viral and
bacterial, as well as parasitic origin, which can be tested at the same time
through the standardisation of temperature profiles, as well as the
combination of different PCR- and PSQ assays. Through the establishment of
internal controls and the construction of special control plasmids, known
problems of PCR-based diagnosis were counteracted and the reliability of the
developed technique further increased. Overall, the newly developed screening-
method offers a sensitive, specific and reliable approach to the clarification
of various hemorrhagic fever viruses in the laboratory, as well as
differential diagnoses in only one procedure, thus conserving time, reagents,
funds and testing materials.
