Thermotolerante Campylobacter spp. gehören in Deutschland zu den häufigsten
Erregern bakterieller Ente-ritiden beim Menschen. Der Erreger wird
hauptsächlich durch kontaminierte Lebensmittel, zumeist Geflü-gelprodukte
übertragen. Die kulturelle Isolierung aus Lebensmitteln bereitet -
insbesondere wegen der ho-hen Wachstumsansprüche und Empfindlichkeit des
Keimes - häufig Probleme, was sich auch in der Vielzahl der entwickelten
Verfahren ausdrückt. Eine erste Normierung für den kulturellen Nachweis
erfolgte auf in-ternationaler Ebene mit der Veröffentlichung der ISO
10272:1995 (E), die zwei optionale Anreicherungsverfahren beschreibt. Im
ersten Teil der vorliegenden Studie wurde der Einfluss verschiedener
kultureller Anreicherungsverfah-ren in Anlehnung an die ISO 10272:1995 (E) auf
die Isolierungsrate von Campylobacter spp. bei natürlich kontaminierten
frischen und tiefgefrorenen Geflügelfleischprodukten untersucht. Ferner wurde
die Notwen-digkeit einer Blut-Supplementation der Preston-Bouillon sowie der
Effekt unterschiedlicher Inkubationszei-ten überprüft. Mit den beiden
Anreicherungsverfahren nach ISO 10272:1995 (E), d.h. der Preston-Bouillon und
der Park und Sanders-Bouillon, konnten aus insgesamt 80 (52 %) der 153 Proben
thermotolerante Campylobacter spp. isoliert werden. Die höchsten Nachweisraten
ergaben sich für Geflügelinnereien (65 %), gefolgt von Geflügelfleisch (48 %)
und Geflügelfleischzubereitungen (20 %). Bei tiefgefrorenen Produkten wurden
insgesamt höhere Prävalenzen ermittelt (58,4 %) als bei frisch angebotener
Ware (43,8 %). Unter Verwen-dung von Preston-Bouillon konnten aus 77 Proben
(50 %) und mit dem Selektivmedium nach Park und Sanders aus 76 Proben (50 %)
Campylobacter spp. isoliert werden. Hinsichtlich der Methodik lassen sich
folgende Schlussfolgerungen ziehen: 1. Mit beiden Anreicherungsverfahren
wurden in Geflügelfleischproben vergleichbare Prävalenzen ermit-telt. Daher
ist für die Untersuchung von frischen und tiefgefrorenen
Geflügelfleischprodukten das An-reicherungsverfahren nach Preston wegen des
geringeren Material-, Zeit- und Arbeitsaufwandes der Park und Sanders-
Anreicherung vorzuziehen. 2. Die Nachweisrate wurde durch die unterschiedliche
Inkubationsdauer (22 ± 2 h oder 46 ± 2 h) nicht beeinflusst. 3. Bei Verwendung
von frisch hergestellter Preston-Bouillon (maximal 3 Tage alt) ist eine
Blutsupplemen-tation nicht unbedingt erforderlich. Darüber hinaus zeigen die
quantitativen Untersuchungen, dass 22,5 % der Campylobacter-positiven Proben
mit beachtlichen Keimzahlen zwischen log 3,3 und log 4,1 KbE/Oberfläche
kontaminiert sind. Dabei han-delt es sich fast ausschließlich um
Geflügelinnereien. Da sowohl in der amtlichen Routinediagnostik als auch in
der Lebensmittelwirtschaft der Wunsch nach schnellen Screeningmethoden stetig
zunimmt, wurden im zweiten Teil der vorliegenden Studie zwei PCR-Verfahren
hinsichtlich ihrer Einsatzmöglichkeiten in der lebensmittelhygienischen
Routinediagnostik über-prüft. Für diesen Zweck wurde eine
Vergleichsuntersuchung von 80 natürlich kontaminierten Geflügel-fleischproben
durchgeführt, wobei das seminested PCR-System nach WEGMÜLLER et al. (1993) und
die pg50/pg3-PCR nach OYOFO et al. (1992) mit dem kulturellen
Referenzverfahren nach ISO 10272:1995 (E) (Preston-Protokoll) hinsichtlich der
Sensitivität und der Spezifität getestet wurden. Mit dem kulturellen
Referenzverfahren nach ISO 10272:1995 (E) und dem seminested PCR-System nach
WEGMÜLLER et al. (1993) konnten jeweils in 44 von 80 Proben thermotolerante
Campylobacter spp. bzw. spezifische Gensequenzen nachgewiesen werden. Dagegen
wurden mit dem pg50/pg3-PCR-System nach OYOFO et al. (1992) 43 positive, ein
falsch-negatives und 5 falsch-positive Ergebnisse erzielt, so dass sich eine
relative Sensitivität von 97,7 % und eine relative Spezifität von 86,1 %
ergaben. Bei der Beurteilung dieser Ergebnisse ist allerdings zu
berücksichtigen, dass die 5 falsch-positiven Befunde nicht durch eine
Hybridisierung bestätigt worden waren. Aus den Ergebnissen dieser
Verfahrensvergleichsuntersuchung lassen sich folgende Schlussfolgerungen
ziehen: 1. Die seminested PCR nach WEGMÜLLER et al. (1993) ist aufgrund der
hohen Sensitivität und Spezifität als schnelles Screeningverfahren für den
Nachweis von thermotoleranten Campylobacter spp. in Geflü-gelfleisch geeignet.
Sie bietet allerdings nicht die Möglichkeit zur Automatisierung und gestattet
auch keine Speziesdifferenzierung zwischen C. jejuni und C. coli. 2. Beim
Einsatz der pg50/pg3-PCR nach OYOFO et al. (1992) ist es zwingend
erforderlich, durch eine ab-schließende Hybridisierung die Identität der
nachgewiesenen PCR-Produkte zu bestätigen, um die Sen-sitivität und vor allem
die Spezifität des Systems zu erhöhen. 3. Im lebensmittelhygienischen
Routinelabor, in dem zwar die PCR, nicht aber die Southernblot-Hybridisierung
apparativ und personell etabliert ist, stellt die seminested PCR eine
Alternative zur vor-läufigen amtlichen Methode nach § 64 LFGB (BVL, 2006) dar.
4. Durch das parallele Mitführen von externen Amplifikationskontrollen können
falsch-negative Ergebnis-se aufgrund inhibitorischer DNA-Extrakte zuverlässig
vermieden werden. 5. Mit dem kommerziellen DNA-Extraktionskit (DNeasy™ Tissue
Kit, Fa. Qiagen) lässt sich Campylo-bacter-DNA im Routinelabor schnell und
effektiv aus Geflügelfleisch und -innereien extrahieren.

Thermotolerant Campylobacter spp. belong to the most frequent causative
organisms of bacterial enteritis in man. The pathogen is transmitted mainly by
contaminated food, especially poultry products. Because of the high growth
requirements and the sensitivity of the microorganism, the cultural isolation
from food is often difficult, a fact which leads to the development of a large
number of methods. A first international standardi-zation of the cultural
detection method took place with the publication of ISO 10272:1995 (E) which
de-scribes two optional enrichment methods. In the first part of the study the
influence of different cultural enrichment methods according to ISO 10272:1995
(E) on the isolation rate of Campylobacter spp. in naturally contaminated
fresh and frozen poultry products was investigated. Furthermore the necessity
of blood supplementation of the Preston broth and the effect of different
incubation times were tested. With the enrichment procedures according to ISO
10272:1995 (E) (Preston broth and Park and Sanders broth) thermotolerant
Campylobacter spp. could be isolated from 80 (52 %) of 153 samples. The
highest detection rate was determined for poultry offal (65 %), followed by
poultry meat (48 %) and poultry meat preparations (20 %). Frozen products
showed higher prevalence rates (58,4 %) than fresh products (43,8 %). Using
Preston broth 77 samples (50 %) were found Campylobacter spp.-positive,
whereas 76 samples were Campylobacter spp.-positive when Park and Sanders
broth was used. The following conclusions can be drawn from the comparison of
the cultural detection methods: 1. The two enrichment procedures performed
equally well in the detection of Campylobacter spp. in poultry samples. So the
Preston protocol should be preferred to the Park and Sanders method for the
examination of fresh and frozen poultry meat because it is less labour
intensive, and less material and time consum-ing. 2. The detection rate was
not influenced by incubation time (22 ± 2 h or 46 ± 2 h). 3. A blood
supplementation is not generally required when freshly produced Preston broth
is used. Furthermore the quantitative examination revealed that 22,5 % of the
Campylobacter positive samples, especially poultry offal, were contaminated
with high population levels from log 3,3 to log 4,1 cfu/surface. In the second
part of the study two PCR-assays were compared for there suitability in
routine food-examination. For this, 80 naturally contaminated poultry samples
were analysed for Campylobacter spp. using the seminested PCR assay of
WEGMÜLLER et al. (1993), the pg50/pg3-PCR of OYOFO et al. (1992) and the
cultural reference method according to ISO 10272:1995 (E) (Preston protocol),
with regard to the sensitivity and specificity of the PCR assays. 44 of 80
samples were positive for Campylobacter spp. and specific gene sequence using
the cultural refer-ence method according to ISO 10272:1995 (E) and the
seminested PCR-System of WEGMÜLLER et al. (1993), respectively. The use of the
pg50/pg3-PCR of OYOFO et al. (1992) yielded 43 positive, 1 false-negative and
5 false-positive results. The relative sensitivity and specificity rate was
calculated to be 97,7 % and 86,1 %, respectively, although considering that
the 5 false-positive results were not confirmed by hy-bridization. The
following conclusions can be drawn from the results comparing the PCR-
detection methods 1. The rapid and sensitive seminested PCR assay of WEGMÜLLER
et al. (1993) provides a suitable tool for specific detection of
thermotolerant Campylobacter spp. in poultry. But there is no possibility for
auto-mation and for differentiation of the species C. jejuni und C. coli. 2.
The pg50/pg3-PCR assay (OYOFO et al., 1992) isn’t a useful screening method
because there is a strong necessity to verify the obtained amplicons by
southernblot-hybridization in order to increase sensitivity and specificity.
3. For laboratories, which have not established hybridization technique yet,
the seminested PCR assay ful-fils the necessity to verify the amplicons (DIN,
2006b) and provides therefore an alternative to the pub-lished provisional
method according to § 64 LFGB (BVL, 2006). 4. To avoid false-negative results,
extern amplification controls are reliable tools. 5. The commercially
available DNA-Extraction kit used in this study (DNeasy™ Tissue Kit, Qiagen)
facili-tates rapid and effective extraction of bacterial DNA out of the tested
food matrix (poultry).
