Infektiöse Komplikationen in der Neutropeniephase sind ein Hauptproblem im Rahmen myeloablativer Hochdosischemotherapien (HDCT) mit nachfolgender autologer peripherer Stammzelltransplantation (PBSCT). In dieser retrospektiven Studie wurden in vier Kliniken 490 Patienten mit insgesamt 600 konsekutiven Zyklen einer HDCT mit anschließender PBSCT bis zum Tag 30 nach erfolgter Transplantation hinsichtlich Fieberepisoden und klinisch oder mikrobiologisch dokumentierter Infektionen ausgewertet. In 80,5% der HDCT- Zyklen trat in der Phase der Neutropenie (≤ 1000 neutrophile Granulozyten/µl) Fieber auf. Eine Infektion konnte in 29,0% aller Hochdosischemotherapiezyklen klinisch und/oder mikrobiologisch nachgewiesen werden. Die Rate dokumentierter Infektionen war bei Patienten mit Leukämien signifikant höher als bei Patienten mit Lymphomen bzw. Multiplem Myelom und bei diesen wiederum signifikant höher als bei Patienten mit soliden Tumoren (p < 0,0005). Eine Multivarianzanalyse (abhängige Variable: "Auftreten einer dokumentierten Infektion") mit den unabhängigen Variablen Grunderkrankung, Alter, Geschlecht, Zeitpunkt des Einsatzes der HDCT innerhalb des Krankheitsverlaufs (Erstlinien- oder Rezidivtherapie), Ganzkörperbestrahlung, retransfundierte CD34+-Zellzahl/kgKG, Positiv- und Negativpurging, applizierte G-CSF-Dosis nach PBSCT und antimikrobielle Prophylaxe wurde durchgeführt. Dabei ergaben sich erstmals Art der Grunderkrankung (Leukämie vs. Lymphom/Multiples Myelom vs. solider Tumor) (p = 0,0014), zunehmendes Alter (p = 0,0197) und retransfundierte CD34+-Zellzahl (< 5 x 106/kgKG vs. ≥ 5 x 106/kgKG) (p = 0,0455) als Faktoren, die mit einem signifikant erhöhten Risiko der Entwicklung dokumentierter Infektionen verknüpft waren. Patienten mit diesen Risikofaktoren könnten eventuell von einem modifizierten Infektionsmanagement profitieren. Für alle anderen Patienten wäre eine Reduktion bzw. Aufgabe der antimikrobiellen Infektionsprophylaxe, eine frühzeitige Entlassung in die ambulante Weiterbehandlung und ein nichtstationäres Management von neutropenischen Fieberepisoden denkbar.
Infections due to profound neutropenia are a major problem in patients treated with high-dose chemotherapy (HDCT) followed by autologous peripheral stem cell transplantation (PBSCT). In this retrospective study 490 patients in four hospitals with 600 consecutive cycles of HDCT with PBSCT have been analysed until day 30 after transplantation regarding fever episodes and clinically or microbiologically documented infections. Fever occured in 80.5% of all cycles of HDCT during neutropenia (≤ 1000 neutrophils/µl) and an infection was documented clinically and/or microbiologically in 29.0%. In patients with leukemia a documented infection was proven significantly more often than in patients with lymphomas/multiple myeloma and in these patients significantly more often than in patients with solid tumours (p < 0.0005). A multivariate analysis (dependent variable: "occurance of a documented infection") has been performed with the independent variables diagnosis, age, time of HDCT (first- line or relapse therapy), total body irradiation, stem cell support, positive and negative purging, dosis of G-CSF after PBSCT and antimicrobiological prophylaxis. As a result diagnosis (leukemia vs. lymphoma/multiple myeloma vs. solid tumour) (p = 0.0014), increasing age (p = 0.0197) and retransfused count of CD34+ stem cells (< 5 x 106/kg body weight vs. ≥ 5 x 106/kg body weight) (p = 0.0455) were detected as significant risk factors for the development of documented infections. Possibly patients with these risk factors could profit from a modified management of infections. For all other patients reduction or giving up of antimicrobiological prophylaxis, earlier discharge from hospital or even outpatient treatment of febrile neutropenia is imaginable.
