Endothelin-1 (ET-1) ist ein potenter Vasokonstriktor. Die intravenöse Gabe von ET - 1 führt zu einer schnellen und kurzfristigen Vasodilatation, gefolgt von einem langanhaltenen Blutdruckanstieg. Die vermutete Rolle des ET-1 als Blutdruck kontrollierendes Peptid konnte jedoch nicht klar bestätigt werden, human ET–1 transgene Mäuse stellten sich als nicht hypertensiv heraus. Diverse Studien zeigen Stickstoffmonoxid als physiologischen Gegenspieler von Endothelin-1, sie sind Komponenten eines lokalen negativ-rückgekoppelten Regelkreises. Es wurde vermutet, dass die Effekte von ET-1 durch einen gegenregulatorischen Anstieg von NO, produziert von der induzierbaren Stickstoffmonoxid-Synthase (iNOS), aufgehoben werden. Aus diesem Grund wurden transgene Mäuse gezüchtet, die das humane Endothelin-1 Gen und einen iNOS knockout haben. Es sollte untersucht werden, wie der iNOS -/- die Herzfunktion, den Blutdruck und den kardialen Phänotyp der human Endothelin-1 transgenen Mäuse beeinflusst. Die Messung des systolischen Blutdrucks zeigte einen höheren Druck bei den iNOS -/- ET +/+ im Vergleich mit den ET-1 transgenen Tieren, gleichzeitig wiesen sie ein niedrigeres BNP auf. Die Herzkathetermessung hat gezeigt, dass sowohl die Gruppe der iNOS -/- ET +/+ als auch der ET +/+ eine diastolische Dysfunktion zeigten. Jedoch war die Diastolische Steifigkeit (EDPVR) und der Linksventrikuläre endddiastolische Druck (LVEDP) signifikant niedriger bei den iNOS -/- ET +/+ als bei den ET +/+ Tieren. Die Herzen der iNOS -/- ET +/+ Mäuse zeigen zwar auch eine vermehrte interstitielle Fibrose, aber keine vermehrte perivaskuläre Fibrose wie die Endothelin transgenen Mäuse. Schlussfolgerung: Der systolische Blutdruck der iNOS -/- ET-1 transgenen Tiere ist signifikant höher als der der ET-1 transgenen Mäuse, aber nicht höher als der der Wildtyptiere. Sie zeigen keinen Bluthochdruck. Experimente mit eNOS knockout ET-1 transgenen Mäusen, die hypertensiv sind, haben gezeigt, dass vermutlich eNOS in der Blutdruckregulation eine größere Rolle spielt. Die exzessive Freisetzung von NO unter pathophysiologischen Bedingungen führt zu einer reduzierten Herzfunktion, durch einen Abbau von freiem NO zu reaktivem Sauerstoff, der die zellulären Strukturen schädigt und Herzinsuffizienz und Apoptose vorantreibt. Ein zusätzlicher iNOS-Knockout in dem ET+/+ transgenen Mausmodell mildert signifikant die Herzschädigung, dies ist schlüssig mit anderen Studienergebnissen. Daraus lässt sich schließen, dass die ET-1 induzierte Schädigung zu mindest partiell durch iNOS vermittelt wird.
Endothelin-1 (ET-1) is a potent vasoconstrictor. Intravenous administration of ET-1 causes a rapid and transient vasodilation followed by a sustained increase in blood pressure. The supposed role of ET-1 as a blood pressure controlling peptide could not be verified; human ET-1 transgenic mice were not hypertensive. Counterregulatory interactions between ET and nitric oxide are well established. It was hypothesized that effects of ET-1 may be antagonized by an increase of nitric oxide, which is produced by the inducible nitric oxide synthase (iNOS). Therefore, cross-bred animals of ET-1 transgenic mice and iNOS knockout mice were generated. The influence of the lack of iNOS on the heart function, blood pressure und cardiac phenotype was evaluated in these animals. In iNOS -/- ET +/+ mice systolic blood pressure was elevated and BNP plasma levels were lower when compared to ET +/+ mice. Assessment of cardiac function via micro conductance catheter showed in both groups a diastolic dysfunction. But the diastolic stiffness (EDPVR) and the leftventricular enddiastolic pressure (LVEDP) were lower in iNOS -/- ET +/+ mice versus ET +/+ mice. Both groups exhibited cardiac interstitial fibrosis, but the iNOS -/- ET +/+ mice had no perivascular fibrosis. Conclusion: The systolic blood pressure was elevated in iNOS -/- ET +/+ mice versus ET +/+ mice, but not versus wildtype mice. They are not hypertensive. A study with hypertensive eNOS-/- ET +/+ show that eNOS plays a major role in regulation of blood pressure. The excessive release of NO under pathophysiological conditions contributes to a depressed cardiac function by the degradation of free NO to reactive oxygen species harming cellular structures and promoting cardiac failure and apoptosis. Our study demonstrates that ET transgenic mice develop left ventricular stiffening with subsequent diastolic dysfunction. Additional knock out of iNOS significantly attenuates cardiac injury which is conclusive with other studies. We thus conclude that ET-1-induced cardiac injury is at least partially mediated by iNOS.
