Glukokortikoide (GC) stellen aufgrund ihrer potenten anti-inflammatorischen und immunosuppressiven Eigenschaften eine der am häufigsten eingesetzten Arzneistoffklassen zur Therapie von entzündlichen Erkrankungen dar. Außerdem werden GC aufgrund ihrer pro-apoptotischen Wirkungen im Rahmen der Anti- Tumortherapie eingesetzt. Seit einiger Zeit ist allerdings bekannt, dass GC in Abhängigkeit vom jeweiligen Zelltyp auch zytoprotektive Eigenschaften aufweisen. Die Signalwege, über die GC anti-apoptotisch wirken können sind zudem noch nicht vollständig aufgeklärt worden. Daher war es das Ziel dieses Promotionsvorhabens den Signalweg der anti-apoptotischen Wirkung von Dexamethason (Dex) in humanen Fibroblasten aufzuklären. Im Rahmen dieser Arbeit konnte gezeigt werden, dass der zytoprotektive Effekt von Dex auf einer Regulation des Sphingolipidrheostats beruht. Im Detail bewirkt Dex eine vermehrte Umwandlung der pro-apoptotischen Sphingolipide Ceramid und Sphingosin in den für seine zytoprotektiven Wirkungen bekannten Lipidmediator Sphingosin-1-Phosphat (S1P). Diese verstärkte Transformation von Ceramid in S1P konnte außerdem auf eine Regulation der Sphingosinkinase 1 zurückgeführt werden, die zusätzlich auch durch das GC induziert wird. Intrazellulär gebildetes S1P wird sodann durch den GC-regulierten „ATP-binding cassette“ Transporter ABCC1 aus der Zelle exportiert und bewirkt dort die Aktivierung des S1P3-Rezeptorsubtyps. Außerdem konnte gezeigt werden, dass der S1P3 durch Dex reguliert werden kann. Zur Analyse des weiteren zytoprotektiven Signalweges wurden verschiedene intrazelluläre Mediatoren der Zytoprotektion hinsichtlich ihrer Beteiligung am anti-apoptotischen Effekt von Dex in Fibroblasten untersucht. Es stellte sich heraus, dass der zytoprotektive Signalweg von Dex auf eine Aktivierung der Phosphoinositol-3-Kinase beruht. Als weitere Zielsturktur des intrazellulären Signalweges konnte zudem die ProteinkinaseB, auch Akt-Kinase genannt, identifiziert werden. Hier zeigte sich ebenfalls dass Dex zu einer Regulation der Kinase beitragen konnte. In einem weiteren Schritt wurde der anti-apoptotische Signalweg des GC hinsichtlich einer Beteiligung von Proteinen der sogenannten B-Zell Lymphoma 2 (Bcl-2)-Familie untersucht, von der sowohl pro- als auch anti-apoptotische Vertreter bekannt sind. Dabei stellte sich heraus, dass das anti-apoptotische Protein BclxL (Bcl-2-like 1) in Anwesenheit von Dex induziert wird und zusätzlich essentiell für den zytoprotektiven Effekt des GC in Fibroblasten war. Während apoptotischer Vorgänge kommt es häufig zu einem Absinken des transmembranären, mitochondrialen Potentials. In Anwesenheit von Dex konnte interessanterweise nachgewiesen werden, dass das GC eine Verminderung des Potentials effektiv verhindern konnte. Zusätzlich zeigte sich, dass der anti- apoptotische Effekt von Dex auch auf einer reduzierten Aktivierung der Effektorcaspase 3 beruht.
Due to their potent anti-inflammatoric and immunosuppressive properties glucocorticoids (GC) represent one of the most commonly used drugs in medicine concerning the therapy of inflammatory diseases and for the modulation of immune responses. Moreover they exert pro-apoptotic properties and are consequently used as cotherapeutics during a treatment with cytostatic drugs. But within the last years it has been shown that GC are also able to prevent programmed cell death. However the detailed signalling mechanisms how GC are able to mediate cytoprotection are not well understood. For this reason the aim of this study was to identify the anti-apoptotic signalling mechanism of dexamethasone (Dex) in human fibroblasts. The present study provides evidence that Dex mediates cytoprotection via a tight regulation of the sphingolipid rheostat. In detail stimulation of Dex resulted in a distinct reduction of pro-apoptotic intracellular ceramide and sphingosine levels, whereas intracellular concentrations of cytoprotective sphingosine-1-phosphate (S1P) were increased. Moreover it has been shown that this process was based on the activity of sphingosine kinase1, which is also induced upon stimulation with Dex. This study shows for the first time that the ATP-binding cassette (ABC) transporter ABCC1 accounts for the export of S1P out of the cell. Thus a tight regulation of the ABC-transporter by Dex was discovered. Extracellular S1P has been reported to activate a group of distinct G-protein coupled receptors, named S1P-receptors. Characterization of the involvement of a specific S1P- receptor in the cytoprotective signalling pathway of Dex identified the S1P3-receptor subtype as a crucial mediator of cytoprotection. Concerning the further anti-apoptotic signalling pathway the present study provides evidence for the participation of one of the most prominent intracellular cytoprotective signalling pathways which is referred to as the phosphoinositide-3-kinase (PI3K)/Akt signalling pathway. Thus a tight regulation of the protein kinase Akt by Dex has been revealed. Closer studies implicated the involvement of the B-cell lymphoma 2 (Bcl-2) family. This family consists of both pro- and anti-apoptotic proteins which had been identified as important mediators of the life death decision of a cell. In detail, it has been elucidated that Dex mediates its cytoprotective effect via an upregulation of anti-apoptotic BclxL (Bcl-2 like 1). Most commonly apoptosis is often connected to a reduction of mitochondrial membrane potential, which may be regulated by several intracellular stimuli even GC. This study reveals that Dex prevents the decrease of mitochondrial membrane potential. Additionally it has been found out that this process correlates with a dimished activation of effector caspase 3- a common marker of apoptosis - in human fibroblasts.
