Diese Arbeit soll zeigen, dass die Magnetresonanzspektroskopie (MRS) als non-
invasive Methode genutzt werden kann, um unter in vivo Bedingungen bei
alkoholkranken Patienten online cerebrale Stoffwechselveränderungen nach
Midazolamapplikation messen zu können, die weitere Aufschlüsse über die
substantielle Wirkweise der Benzodiazepine bei der Behandlung des
Alkoholentzugssyndroms (AES) geben können. Je 10 Patienten einer Ziel- und
einer Kontrollgruppe wurden in einem Siemens Magnetom
Magnetresonanztomographen mit einer Feldstärke von 1,5 Tesla einer
volumenselektiven Magnetresonanzspektroskopie anhand der STEAM-Sequenz im
Bereich des linken Corpus striatum unterzogen. Die Akquisitionszeit pro
Spektrum betrug 6 Minuten und 31 Sekunden. In der Zielgruppe konnte nach
Midazolamapplikation ein signifikanter Anstieg für Glutamin (Gln) nachgewiesen
werden, sowie ein signifikanter Abfall für Laktat. Nicht signifikante Anstiege
ließen sich für die exzitatorischen Transmitter Glutamat und die Summe aus
GABA, Glutamin und Glutamat (Glx) konstatieren sowie für Inositol, Cholin und
Laktat. In der Kontrollgruppe wurden signifikante Anstiege für Cholin und
Glutamat (Glu) beobachtet. Nicht signifikante Anstiege ließen sich für GABA,
Gln, Glx, Inositol und Laktat verzeichnen. Die Signifikanz einzelner
Metabolitenveränderungen (Glutamin, Laktat) in der Gruppe alkoholkranker
Patienten, verbunden mit der Nachvollziehbarkeit dieser Veränderungen im
Hinblick auf die klinische Wirkungsweise der Benzodiazepine in der Behandlung
des AES, legt nahe, dass die MRS unter Berücksichtigung ihrer Vorteile und
Grenzen ein wertvolles Instrument auf dem Weg ist, cerebrale
Stoffwechselprozesse unter Medikamenten- oder anderweitiger
Substratapplikation zu erforschen und zu verstehen.

This examination demonstrates that magnetic resonance spectroscopy as a non-
invasive method can be used to detect cerebral metabolic changes after
midazolam administration to alcohol addicted patients under in vivo
conditions. This online-monitoring tool should be used to get further
information about the pharmacodynamics of the benzodiazepines concerning the
treatment of the alcohol withdrawal syndrome. During this study protocol 10
alcohol addicted patients ( target group ) and 10 non-alcoholic patients (
control group ) were examined in a Siemens Magnetom MRT with a magnetic field
intensity of 1,5 Tesla, using the STEAM-Sequence as a volume selective
magnetic resonance spectroscopy-protocol. The region of interest was the left
corpus striatum. The acquisition time-period was 6 minutes and 31 seconds to
obtain a single spectrum. The examination of the target group showed a
significant increase for glutamine (Gln) and a significant decrease for
Lactate. Non significant increases could be shown for glutamate (Glu), the sum
of GABA, glutamine and glutamate (Glx), inositol, cholin and lactate. In the
control group we could see a significant increase for glutamate (Glu) and
cholin. Non significant increases could be detected for GABA, Gln, Glx,
inositol and lactate. The significance of single cerebral metabolic changes (
glutamine, lactate ) in the group of alcohol addicted patients associated with
the plausibility of these changes concerning to the mode of functioning of the
benzodiazepines in the treatment of alcohol withdrawal syndrome points out,
that magnetic resonance spectroscopy is a valuable method to explore and to
understand cerebral metabolic pathways after administration of drugs and other
substances.
