Melatonin wird in der Epiphyse gebildet. Synthese und Freisetzung wird durch
Beleuchtung der Retina gesteuert. Die Information wird über den
retinohypothalamischen Trakt zunächst zum Hypothalamus geleitet, von dort über
die absteigende Sympathikusbahn und prae- und postganglionäre sympathische
Fasern zur Epiphyse. Dieses neuronale System wird durch Dunkelheit stimuliert
und durch Licht gehemmt. Das Maximum der Melatoninsekretion liegt nachts
zwischen zwei und vier Uhr, das Minimum waehrend des Tages zwischen 12.00 und
14.00 Uhr. In der Leber wird Melatonin schnell hauptsaechlich zu
6-Hydroxymelatoninsulfat metabolisiert und mit dem Urin ausgeschieden. Die im
nächtlichen Sammelurin gemessene Konzentration von 6-Hydroxymelatoninsulfat
korreliert gut mit der Konzentration des Serummelatonins. Ziel unserer Arbeit
war es herauszufinden, ob bei Diabetikern mit vegetativer Neuropathie die
Melatoninproduktion gestoert ist. Wir untersuchten 20 Patienten mit Diabetes
mellitus im Alter von 51 bis 83 Jahren. 15 dieser Patienten (Gruppe 3) hatten
eine sensomotorische und vegetative Neuropathie. Die übrigen 5 untersuchten
Patienten (Gruppe 2) hatten keine klinischen Symptome einer vegetativen oder
sensomotorischen Neuropathie. Weiterhin untersuchten wir eine Kontrollgruppe
(Gruppe 1) von 25 Probanden im Alter von 52 bis 86 Jahren ohne Diabetes
mellitus und ohne Neuropathie. Wir bestimmten 6-Hydroxymelatoninsulfat im
Urin. Die Probengewinnung zur Erfassung der Maximalsekretion erfolgte aus
einem 6-Stunden-Sammelurin von 0 bis 6 Uhr, eine weitere Probengewinnung zur
Erfassung des Minimums der Melatoninsekretion erfolgte aus einem 6-Stunden-
Sammelurin von 12 bis 18 Uhr. Aus den gemessenen 6-Hydroxy-Melatonin-Sulfat-
Konzentrationen wurde die ausgeschiedene Menge bezogen auf das ausgeschiedene
Kreatinin (mg/mol Krea) berechnet. Zwischen den Gruppen ergeben sich keine
signifikanten Unterschiede bezueglich der Ausscheidung am Tag. Nachts war die
Ausscheidung in der Normalgruppe signifikant hoeher als in der Gruppe mit
vegetativer Neuropathie. Die Ergebnisse unserer Arbeit zeigen, dass Pat. mit
Diabetes mellitus und vegetativer Neuropathie einen verminderten naechtlichen
Anstieg ihrer Melatoninproduktion im Vergleich zur Normalgruppe haben. Als
Ursache hierfür nehmen wir eine Stoerung der neuronalen Stimulation der
Pinealis im Rahmen der vegetativen Neuropathie an. Die Patienten mit Diabetes
mellitus ohne gravierende Polyneuropathie lagen mit ihrer naechtlichen
Melatonin-Sulfat -Ausscheidung tendenziell zwischen der Normalgruppe und der
Neuropathiegruppe, jedoch kann auf Grund der geringen Probandenzahl in dieser
Gruppe keine sichere Aussage gemacht werden. Es zeigte sich eine relativ hohe
Streubreite der Melatonin-Sulfat -Ausscheidung in der Normalgruppe, so dass
ein cut off-Wert nicht sicher festgelegt werden konnte. In Anbetracht der noch
geringen Patientenzahl waeren daher weitere Untersuchungen notwendig.

dc.description.abstract
Melatonin is synthesized in the pineal gland. Synthesis and release are
regulated by the illumination of the retina. The information travels via the
retinohypothalamic tract to the hypothalamus, from there via the descending
central sympathetic tract and the pre- and postganglionic sympathetic fibers
to the pineal gland. This neuronal system is stimulated by darkness and
inhibited by light. The maximum of melatonin secretion occurs between 2.00 and
4.00 a.m., the minimum is during the day time between 12.00 and 2.00 p.m. In
the liver melatonin is rapidly metabolized by hydroxylation and conjugation
and is excreted by the kidney as 6-sulfatoxymelatonin (the main metabolite of
melatonin) .The urinary excretion of 6-sulfatoxymelatonin closely parallels
serum melatonin concentrations. We examined whether the involved pathways are
disturbed by diabetic vegetative neuropathy. We examined 20 patients with
diabetes mellitus aged 51 to 83 years. 15 of these patients (group 3) had a
symmetric sensorimotor and vegetative neuropathy. The other 5 patients had no
or only minor symtoms of neuropathy (group 2). In addition to these two groups
we examined a group of 25 normals aged 52-86 without diabetes mellitus or
neuropathy (group 1). We examined 6-sulfatoxymelatonin in the urine. Urine was
collected between 0.00 and 6.00 a.m. to examine the maximal secretion; the
second sample of urine was taken between 12.00 and 6.00 p.m. in order to
examine the time of minimal melatonin secretion. The amount of
6-sulfatoxymelatonin, secreted during the two collecting periods, was
expressed in relation to the excreted creatinine (mg/mol crea). There were no
significant differences between the groups with respect to melatonin secretion
during day time; however melatonin secretion at night was significantly higher
in the group of normals than in the group of patients with polyneuropathy. The
results show that diabetic patients with sensorimotor and vegetative
neuropathy only have a minor nocturnal increase of melatonin secretion; this
increase is significantly smaller than in the group of normals. We assume that
the vegetative neuropathy leads to an impaired neuronal stimulation of the
pineal gland. Patients with diabetes mellitus without clinically evident
neuropathy show nocturnal melatonin values which were between those of the
normal group and the group of patients with neuropathy .Due to the small
number in this group a definite statement is not possible . In the group of
normals melatonin secretion varied considerably. Therefore at least until now
a definite cut off value cannot be determinated. Further studies are
necessary.
