Die Thrombozyten-Aktivierung spielt eine entscheidende Rolle in der
Pathogenese der koronaren Herzerkrankung und der Pathophysiologie des akuten
Koronarsyndroms. Dementsprechend bildet die antithrombozytäre Therapie einen
Grundpfeiler der Behandlung einer koronaren Herzerkrankung Bezüglich der
Erhaltungsdosis unterscheiden aktuelle Leitlinien nicht zwischen Patienten mit
akutem Myokardinfarkt und elektiven Patienten. In der vorliegenden
Untersuchung konnte gezeigt werden, dass Myokardinfarkt, C-reaktives Protein
und Fibrinogen in engem Zusammenhang stehen zur erhöhten residuellen
Thrombozytenreaktiviät 48 h nach koronarer Stentimplantation. Dies würde - in
Anbetracht der Tatsache, dass eine gesteigerte residuelle
Thrombozytenreaktivität mit einem erhöhten kardiovaskulären Risiko nach PTCA
assoziiert ist - eine theoretische Grundlage für Intensivierung der
antithrombozytären Erhaltungstherapie darstellen. Darüber hinaus wurde ein
neuer Point-of-care-Test zur Messung der Thrombozytenfunktion
(Impedanzaggregometrie) verglichen mit dem bisherigen Goldstandard der
Lichttransmissionsaggregometrie nach Born. Schlussfolgerung: Patienten, die
einer koronaren Stentimplantation bei akutem Myokardinfarkt unterzogen werden,
weisen eine gesteigerte residuelle Thrombozytenreaktivität auf, welche
mindestens 48 h nach koronarer Stentimplantation anhält. Diese Erkenntnis
bietet eine Begründung für eine weitergehende intensivierte antithrombozytäre
Therapie nach Myokardinfarkt. Weiter konnte nicht hinreichend geklärt werden,
ob die Impedanzaggregometrie als neuer Point-of-care-Test zur Überwachung der
Thrombozytenfunktion tatsächlich geeignet ist, obwohl es im Vergleich zum
bisherigen Gold-Standard beachtliche Übereinstimmungen gab.

dc.description.abstract
Platelet activation plays a crucial role in the pathogenesis of of coronary
heart disease and the pathophysiology of acute coronary syndrome. Accordingly,
one is the antiplatelet therapy the mainstay of treatment of coronary heart
disease. With respect to the maintenance dose current guidelines do not
distinguish between patients with acute myocardial infarction and elective
patients. In the present study could be shown that myocardial infarction,
C-reactive protein and fibrinogen are closely related to the enhanced residual
platelet reactivity 48 h after coronary stent implantation. This would
represent - in consideration of the fact that an enhanced residual platelet
reactivity is associated with increased cardiovascular risk after PTCA - a
theoretical basis for intensification of antiplatelet maintenance therapy. In
addition, a new point-of-care test used to measure platelet function
(impedance aggregometry) was compared with the previous gold standard of light
transmissions aggregometry after Born. Conclusion: Patients undergoing
coronary stenting for acute myocardial infarction exhibit an enhancement of
residual platelet reactivity, which sustained for at least 48 h after coronary
stent implantation. This finding provides a rationale for further intensified
antiplatelet therapy after myocardial infarction. Next could not be
sufficiently clarified whether the impedance aggregometry is really suitable
as a new point-of-care test for the monitoring of platelet function, although
in comparison to the current gold standard was remarkable coincidences.
