Einleitung: Sportliche Großveranstaltungen wie der real,-Berlin-Marathon mit
über 30000 Teilnehmern erfordern eine optimale Planung der medizinischen
Versorgung. Unregelmäßigkeiten bei Veranstaltungen der Vorjahre führten zur
Notwendigkeit eines neuen Versorgungskonzepts. Als Datengrundlage wurden die
medizinischen Behandlungen während des 30. real,-Berlin-Marathon 2003
dokumentiert und analysiert. Ziel des neuen Konzepts sollte die gleichmäßigere
Auslastung der medizinischen Helfer unabhängig von deren Standort sein. Die
begleitende Dokumentation sollte dies überprüfen.
Methoden: 2003 wurden die Daten mittels Patientendokumentationsbogen durch das
medizinische Personal des Deutschen Roten Kreuz (DRK) erhoben. Das neue
Dokumentationskonzept für 2004 beinhaltete die Schulung des Personals sowie
die Nummerierung und Vereinfachung der Bögen, die zentral gesammelt und
digital verarbeitet wurden. Das Personal wurde mittels Fragebogen zum Ablauf
des Einsatzes befragt.
Ergebnisse: 2003 wurden für 31711 Marathonteilnehmer 479 medizinische Helfer
eingesetzt, die 1287 Hilfeleistungen erbrachten. 42% der Hilfeleistungen
fanden an der Strecke statt, wo 56% des Personals stationiert war. Für die
Veranstaltung ergab sich ein Patienten/Helfer-Quotient von 2,7. Dieser Wert
variierte bezogen auf einzelne Hilfsstellen stark, insbesondere im Zielgebiet
(0,3-12). Die Änderungen des Versorgungskonzepts für 2004 führten zu folgendem
Ergebnis: 379 medizinische Helfer erbrachten 1247 Hilfeleistungen für 29022
Sportler. Davon 39% an der Strecke, auf der sich 40% des Personals befand.
Insgesamt ergab sich ein Quotient von 3,2. Die Streuung war im Vergleich zum
Vorjahr deutlich geringer. Die befragten Helfer gaben keine Überforderung an.
Schlussfolgerungen: Die medizinische Versorgung der Marathonveranstaltung
konnte effizienter gestaltet werden. Weniger Helfer (13 anstatt 15 pro 1000
Läufer) konnten durch bessere Aufteilung auf Strecke- und Zielgebiet
individuellere medizinische Versorgung leisten. Die kontinuierliche
Dokumentation und Evaluation medizinischer Hilfeleistungen bei
Großveranstaltungen ist in der Lage Schwachstellen zu erkennen und zu
beseitigen.

Introduction: Athletic events such as the real,-Berlin Marathon with more than
30000 active participants demand an optimised organisation of the medical care
provided to the athletes. Problems at previous events pointed out the need of
an improved medical care concept. As a starting point, the medical treatment
data of the 30. real,-Berlin-Marathon were documented and analysed. The
analysis showed that an ameliorated emergency aid concept should provide a
more evenly spread work load for the medical staff, independent of their
position at the course. The documentation of the event was adapted to document
possible improvements.
Methods: In 2003 medical treatments were documented via medical records by
members of the German Red Cross (DRK). The new care concept for 2004 included
training of the staff and also the numbering and simplification of the medical
records, which were collected centrally and were analysed electronically. The
paramedics were queried via questionnaires about their assessment of the
course of the event.
Results: In 2003 for 31711 marathon participants 479 paramedics and physicians
were on duty who provided aid in 1287 incidents. 42% of the treatments took
place at the course, where 56% of the staff was stationed. For the whole event
an average patient-to-paramedic ratio of 2,7 was determined. This figure
varied widely from one aid post to another, especially near the finish line
(0,3-12). The change in the care concept 2004 lead to the following
improvements: 379 paramedics/physicians supplied assistance in 1247 cases
while 29022 participants took part in the event. 39% of the incidents took
place at the course, where 40% of the aid workers were stationed. The patient-
to-paramedic ratio was 3,2, the overall ratio 3,3. The variation of the ratio
was lower than in the previous year. The questioned staff did not feel
overburdened during the event, according to the answers to the questionnaires.
Conclusion: The marathon medical care could be organised more efficiently in
2004. Fewer paramedics (13 instead of 15 per 1000 athletes) could provide a
more individual assistance thanks to their improved distribution along the
course and the finish area. The results show that sustained documentation and
analysis of medical treatments at mass gathering events is able to detect and
eliminate care concept problems.
