Zytomegalievirus und Humanes Herpesvirus 6 nach allogener Stammzelltransplantation

Abstract

Patienten nach allogener Stammzelltransplantation sind durch Herpesvirusinfektionen stark gefährdet. In dieser Arbeit wurden Ergebnisse quantitativer PCR Untersuchungen von 1241 Plasmaproben für CMV, HHV-6A und -6B mit den entsprechenden klinischen Daten 82 stammzelltransplantierter Patienten korreliert. Dabei wurden unter anderem der Zeitpunkt des Auftretens von Infektionen, Risikofaktoren, das Konditionierungsregime, das Vorhandensein von GvHD und die Kinetik des Engraftments untersucht. Weiterhin erfolgte für CMV ein Vergleich der Nachweismethoden. CMV DNA konnte bei 50 % der Patienten nachgewiesen werden. Als Risikofaktoren für eine CMV Infektion wurde eine serologische CMV Risikokonstellation (p=0,035), sowie eine akute GvHD II- IV (p=0,0018) identifiziert. HHV-6A und –6B konnten bei 29 % und 43 % der Patienten nachgewiesen werden. Als Risikofaktoren für eine HHV-6B Infektion konnten die Transplantatquelle (p=0,035), eine GvHD (p=0,031) und ein positiver CMV Serostatus (p=0,041) nachgewiesen werden. Keine der untersuchten Variablen konnte mit einem HHV-6A Nachweis assoziiert werden. Ein klarer Zusammenhang zwischen dem Auftreten der untersuchten Viren konnte nicht nachgewiesen werden. HHV-6A und –6B Virämien dauerten, im Vergleich zu CMV, kürzer an und waren intermittierend nachweisbar. Beim Vergleich der myeloablativen und der dosisreduzierten Konditionierung bestanden keine Unterschiede bezüglich der Häufigkeit des Nachweises von CMV DNA oder Antigen, Nachweiszeitpunkt oder maximaler DNA Last. Es zeigte sich ein deutlich verzögertes thrombozytäres Engraftment bei Patienten mit einer frühen HHV-6B Infektion (p=0,006). Ebenso konnte das Auftreten einer akuten GvHD mit einer HHV-6B Infektion (p=0,025) in Zusammenhang gebracht werden. Die Unterscheidung zwischen den beiden Varianten HHV-6A und –6B nach SZT ist somit von klinischer Bedeutung. Der Vergleich des Antigennachweises mit der quantitativen real-time PCR im TaqMan Format für den Nachweis von CMV ergab einen deutlich häufigeren CMV DNA Nachweis als Antigennachweis (50 % zu 26 % der Patienten). Durch die quantitative PCR konnte CMV DNA im Median 2 Wochen vor einer Antigenämie nachgewiesen werden (p=0,000). Die quantitativen PCR hat eine Sensitivität von 100% in Bezug auf das Auftreten einer Organerkrankung. Spezifität und positiver Vorhersagewert sind jedoch niedriger als die des Antigennachweises. Durch Einführung eines Grenzwertes, ab welchem eine PCR Probe als positiv gilt, erhöhen sich Spezifität und positiver Vorhersagewert der quantitativen PCR und gleichen sich den Ergebnissen des Antigennachweises an.

Infections with herpesviruses cause significant morbidity and mortality after stem cell transplantation. In this dissertation the results of quantitative PCR for detection of CMV, HHV-6A and –6B in 1241 plasmasamples were correlated to clinical data of the corresponding 82 patients, undergoing stem cell transplantation. We studied the incidence and onset of infection, risk factors, the conditioning regime, the occurrence of GVHD and the kinetics of engraftments. Furthermore we compared different methods for detection of CMV. CMV DNA was detected in 50% of patients. A serological status of risk constellation (p=0,035) and an acute GVHD II-IV (p=0,0018) were identified as risk factors for CMV infection. In our patient group HHV-6A and -6B were detected in 29% and 43% of patients, respectively. As risk factors for infection with HHV-6B we identified stem cell source (p=0,035), GVHD (p=0,035) and a positive serological status of CMV (p=0,041). None of the included variables could be associated with detection of HHV-6A. No definite association between the occurrence of the examined viruses was proved. Compared to CMV the duration of HHV-6A and -6B viremia was shorter and the detection intermittent. There were no differences according the incidence of CMV DNAemia and CMV antigenemia, the onset or peak level after reduced- intensitiy compared to myeloablative conditioning. The early detection of HHV- 6B DNA was associated with a delay of platelet engraftment (P=0,006), as well as the occurrence of acute GVHD (p=0,025). Therefore the distinction of the two variants HHV-6A und –6B after SCT is clinically significant. Comparing pp65 Antigen Detection and quantitative Real-Time PCR for CMV, we found significantly more patients to be CMV DNA positive than antigen positive (50 % to 26 %). CMV DNA assessed by quantitative PCR was detected two weaks earlier than antigenemia (p=0,000). Quantitative PCR has a sensitivity of 100% according to occurrence of CMV disease. However, the PCR has a much lower specificity and positive predicitve value for CMV disease than antigen detection. Assuming a threshold beyond which a PCR sample is considered positive, the specifity and positive predicitve value of quantitative PCR approach to those of the antigen detection.
