Prädiktion von Präeklampsie-assoziierten Komplikationen mit Hilfe des sFlt-1 /PlGF-Quotienten

Abstract

Einleitung: Die Präeklampsie (PE) ist eine der Hauptursachen für maternale und fetale Morbidität und Mortalität und geht mit zahlreichen Komplikationen einher. Pathogenetisch besteht ein Ungleichgewicht zwischen anti- und pro- angiogenen Faktoren. Das Ziel unserer Studie ist es, zu zeigen, ob durch zusätzliche Bestimmung des Quotienten aus dem anti-angiogenen Faktor soluble Fms like tyrosine kinase-1 (sFlt-1) und dem pro-angiogenen Faktor Placental Growth Factor (PlGF) die Prädiktion von Präeklampsie-assoziierten Komplikationen innerhalb der nächsten vier Wochen bei Schwangeren mit Verdacht auf Präeklampsie möglich ist. Methodik: Im Rahmen einer multizentrischen, prospektiven, doppelt-verblindeten Studie untersuchten wir 209 Schwangere. Mittels des automatisierten Elecsys® ELISA erfolgte die Bestimmung von sFlt-1 und PlGF im Serum bei Erstpräsentation. Der sFlt-1/PlGF-Quotient wurde bezüglich seiner prädiktiven Aussagekraft untersucht. Dafür wurden retrospektiv die Probandinnen anhand der aufgetretenen Präeklampsie- assoziierten Komplikationen in sechs Gruppen aufgeteilt und die Vorhersagegenauigkeit des sFlt-1/PlGF-Quotienten bestimmt. Ergebnisse: 72 der 209 Probandinnen entwickelten mindestens eine Präeklampsie-assoziierte Komplikation. Unter den 161 Probandinnen, die sich vor der 34. Schwangerschaftswoche (SSW) vorstellten, traten bei 34 Komplikationen auf. Der sFlt-1/PlGF-Quotient zeigte signifikant höhere Medianwerte in Schwangerschaften, bei denen Komplikationen auftraten, verglichen mit jenen ohne Komplikationen (Gruppe 6: 87,4 versus 5,6, p < 0,001). Bei Untersuchungen vor der 34. SSW waren diese Unterschiede noch prägnanter (Gruppe 6 < 34 SSW: 148,4 versus 5,6, p < 0,001). Für die Prädiktion einer iatrogenen Entbindung aufgrund einer Präeklampsie oder eines HELLP-Syndroms (Gruppe 1) ergab sich eine hohe Vorhersagegenauigkeit (Trennwert 114,4: Sensitivität = 79 %, Spezifität = 89 %, NPV = 98 %, PPV = 35 %, - LR = 0,24, + LR = 7,19). Ebenso konnte das Auftreten mindestens einer Präeklampsie-assoziierten Komplikation bei Untersuchung vor der 34. SSW (Gruppe 6 < 34\. SSW) präzise vorhergesagt werden (Trennwert 49,4: Sensitivität = 87 %, Spezifität = 85 %, NPV = 96 %, PPV = 60 %, - LR = 0,15, + LR = 6,00). Schlussfolgerung: Unsere Ergebnisse verdeutlichen, dass sich der sFlt-1/PlGF-Quotient für die Prädiktion von Präeklampsie-bedingten Komplikationen eignet. Ein Trennwert von 49,4 erreichte die beste Vorhersage für das Auftreten mindestens einer Komplikation mit einem NPV von 96 % und einem PPV von 60 % bei Untersuchung vor der 34. SSW. Die additive Bestimmung des sFlt-1/PlGF-Quotienten ist ein wichtiges Instrument zur differentialdiagnostischen Untersuchung von Schwangeren mit einem hohen Risiko für Präeklampsie. Durch den Test kann das klinische Management hinsichtlich einer gezielten therapeutischen Entscheidungsfindung erleichtert werden und Präeklampsie-assoziierte Komplikationen können frühzeitig erkannt werden.

Introduction: Preeclampsia (PE) is a major cause of maternal and fetal morbidity and mortality and is associated with numerous complications. The imbalance of angiogenic factors plays a key role in the pathogenesis. The aim of our study is to demonstrate, whether the additional measurement of the ratio of the antiangiogenic factor soluble Fms like tyrosine kinase-1 (sFlt-1) to the proangiogenic factor Placental Growth Factor (PlGF) is able to predict the occurrence of preeclampsia associated complications within four weeks in women with the suspicion of preeclampsia. Methods: In this prospective, multicentered, double-blinded study we examined 209 pregnant women. Serum levels of sFlt-1 and PlGF were measured at presentation using fully automated Elecsys® ELISA. The participants were retrospectively divided into six groups based on the occurrence of preeclampsia associated complications and the predictive accuracy of the sFlt-1/PlGF ratio was determined. Results: 72 of 209 participants developed at least one preeclampsia associated complication. Among 161 participants who were included before 34 weeks of gestation, 34 had adverse outcomes. The median sFlt-1/PlGF ratio was significantly higher in participants who experienced any adverse outcome compared to those who did not (group 6: 87.4 versus 5.6, p < 0.001). The results were more distinctive at examination before 34 weeks of gestation (group 6 < 34 weeks: 148.4 versus 5.6, p < 0.001). The prediction of having an iatrogenic delivery because of preeclampsia or HELLP syndrome (group 1) had a high predictive accuracy (cut- off 114.4: sensitivity = 79 %, specificity = 89 %, NPV = 98 %, PPV = 35 %, - LR = 0.24, + LR = 7.19). The occurrence of any adverse outcome at inclusion before 34 weeks gestation (group 6 < 34 weeks) could also be accurately predicted (cut-off 49.4: sensitivity = 87 %, specificity = 85 %, NPV = 96 %, PPV = 60 %, - LR = 0.15, + LR = 6.00). Conclusion: Our results demonstrate that the sFlt-1/PlGF ratio has the potential to predict preeclampsia associated complications. At a ratio cut-off of 49.4, the best prediction for any adverse outcome is achieved with a NPV of 96 % and a PPV of 60 % at presentation before 34 weeks of gestation. The additional determination of the sFlt-1/PlGF ratio is an important tool in the differential diagnostic work-up in women presenting with high risk for the disease. The test has the potential to facilitate clinical management with regard to a targeted therapeutic decision-making and could help to identify adverse outcomes at an early stage.
