In dieser Doktorarbeit wurde der prolongierte neuroprotektive Effekt volatiler
Anästhetika untersucht. Die Möglichkeit, die volatilen Anästhetika Isofluran
und Halothan als Präkonditionierungsstimuli einzusetzen, um eine
Ischämietoleranz auf einen subsequenten ischämischen Insult zu induzieren,
wurde in vivo an männlichen ausgewachsenen Wistarratten geprüft. Die Tiere
wurden drei Stunden einer 1,2 %igen Halothan- oder 1,4 %igen Isoflurannarkose
im atmosphärischen Luftgemisch ausgesetzt. Zusätzlich wurde aufgrund von
Blutgasveränderungen während der Halothannarkose eine weitere Gruppe
eingeführt, die zusätzlich zu Halothan und Luftgemisch 10 % Sauerstoff
erhielt. Nach 24 Stunden wurde eine Okklusion der Arteria cerebri media bei
den Ratten durchgeführt und vier Tage danach das Infarktvolumen evaluiert.
Eine Isofluranpräkonditionierung oder die Präkonditionierung mit Halothan und
Sauerstoff führten zu einer signifikanten Verringerung des Infarktvolumens im
Vergleich zur Kontrollgruppe. Die alleinige Behandlung mit Halothan bewirkte
keine signifikante Reduzierung des Infarktvolumens. Der neuroprotektive Effekt
von Isofluran war direkt nach Präkonditionierung nachweisbar und hielt bis 24
Stunden nach Präkonditionierung an. Mittels Western-Blot-Analysen von
homogenisierten Anteilen des Neokortex von Ratten wurde eine signifikante
Expression der induzierbaren NO Synthtase (iNOS) 18 Stunden bis 24 Stunden
nach Präkonditionierung nachgewiesen. Hemmung der iNOS-Aktivität mit
Aminoguanidin in vivo führte zu einem Verlust des neuroprotektiven Effektes
von Isofluran. Somit scheint iNOS ein wichtiger Vermittler der durch
Präkonditionierung induzierten Ischämietoleranz zu sein. Aufgrund der in
dieser Arbeit dargestellten Ergebnisse sind weitere Untersuchungen sinnvoll,
die die Mechanismen der Präkonditionierung aufklären und eventuell zu klinisch
durchführbaren Methoden der Neuroprotektion als Therapie oder Prophylaxe von
ischämischen Schlaganfällen führen.

dc.description.abstract
In this project, the prolonged neuroprotective effect of volatile anaesthetics
was investigated in male Wistar-rats. The animals were pretreated with
halothane or isoflurane in normal air (1,2% and 1,4%, respectively). Because
of changes in the blood-gases in the halothane group, a third group was
treated with halothane in addition with 10% oxygen. 24 hours after the pre-
conditioning phase, an occlusion of the arteria media was performed. Four days
later, the volume of the infarction was evaluated. Preconditioning with
isoflurane and with the mixture of halothane and oxygen significantly reduced
the infarct volumes compared with the control group. Treatment with halothane
was not associated with a significant reduction of infarction. The
neuroprotective effect of isoflurane was detectable directly after the pre-
conditioning and was significant until 24 hours after pre-conditioning.
Homogenated neocortical areas of rats were investigated for the expression of
inducible NO synthetase (iNOS) by western-blot analysis. Significantly
increased expression was detectable 18 to 24 hours after pre-conditioning. The
neuroprotective effect of pre-conditioning with volatile anaesthetics was
diminished by inhibition of iNOS via aminoguanidine in vivo. This shows that
iNOS is an important mediator of the neuroprotection induced by pre-
conditioning. Based on these findings, more studies on the mechanisms of pre-
conditioning are needed. This could lead to clinical strategies to use pre-
conditioning as treatment or prophylaxis for ischemic strokes.
