Zielsetzung: Das Gallenblasenkarzinom zählt mit einer Inzidenz von 0,8 - 1,2 %
aller cholecystektomierten Patienten zu den seltenen malignen Tumoren.
Aufgrund der spät auftretenden Symptomatik liegt häufig bei Diagnosestellung
ein fortgeschrittenes Tumorleiden vor. Ziel der vorliegenden retrospektiven
Analyse des Krankengutes der Chirurgischen Abteilung des Krankenhauses Moabit
aus den Jahren Juni 1990 Mai 2000 ist es, anhand des großen Patientengutes
von über 6.000 Cholecystektomien zu beschreiben, wie Patienten mit der
Diagnose Gallenblasenkarzinom als Zufallsbefund behandelt wurden und wie ihre
Überlebenszeit war. Patienten und Methoden: Von Juni 1990 Mai 2000 wurden im
Krankenhaus Moabit Berlin 6653 Patienten mit vollständig dokumentiertem
Verlauf mit symptomatischer Cholecystolithiasis operativ behandelt, 97 %
laparoskopisch. Bei insgesamt 42 Patienten (0,6 %) fand sich ein
Gallenblasenkarzinom. Der poststationäre Verlauf dieser Patienten wurde anhand
erfragter Daten der weiterbehandelnden Ärzte evaluiert. Resultate: Von den 42
Patienten mit Gallenblasenkarzinom waren 37 weiblich und 5 männlich. Das
Durchschnittsalter betrug 69 Jahre. In 16 Fällen (39 %) bestand präoperativ
ein Verdacht auf mögliche Malignität. Während im Tumorstadium Tis und T1 die
laparoskopische Cholecystektomie als kurativer Eingriff angesehen wurde, fand
im Tumorstadium T2 und T3 eine radikale Nachresektion mit den Lebersegmenten
4b und 5 sowie eine Lymphadenektomie entlang des Ligamentum hepatoduodenale
statt. Die durchschnittliche Überlebenszeit aller 42 Patienten betrug in der
vorliegenden Arbeit 447 Tage. Schlussfolgerung: Während im Tumorstadium Tis
und T1 die laparoskopische Cholecystektomie als kurativer Eingriff angesehen
werden kann, sollte im Tumorstadium T2 und T3 eine radikale Nachresektion mit
den Lebersegmenten 4b und 5 sowie eine Lymphadenektomie entlang des Ligamentum
hepatoduodenale stattfinden. Um eine sichere R0-Rektion zu gewährleisten,
sollte auch der Zystikusstumpf nachreseziert werden. Ab dem Tumorstadium T3
ist auch bei ausgedehnter Nachresektion nicht mit einer Verbesserung der
5-Jahresüberlebenszeit zu rechnen.

Introduction: Gallbladdercarcinoma is rare and has an incidence of 0.8-1.2% in
cholecystecomized patients. Symptoms occur late in the course of the disease;
therefore advanced cancerous invasion is often encountered at the time of
diagnosis. Goal of this retrospective analysis is to describe treatment and
survival time after incidental diagnosis of gallbladder carcinoma in patients
who underwent cholecystecomy at the Department of Surgery at Krankenhaus
Moabit between 1990 and 2000. Patients and Methods: Between June 1990 and Mai
2000 6653 Patients underwent cholecystectomy for symptomatic
cholecystolithiasis. 97 % were treated laparascopically, 3 % conventionally.
All patient charts were fully documented. 42 patients (0.6%) were diagnosed
with gallbladder carcinoma. Physicians, who were involved in postoperative
care, were directly questioned and the data analyzed. Results: Of 42 patients
with gallbladdercarcinoma 37 were female, 5 male. Average age was 69 years. In
16 cases (39%) there was a preoperative suspicion for malignancy. Laparascopic
cholecystectomy was considered curative for tumorgrades Tis andT1. Patients
with tumorgrades T2 and T3 underwent radical repeat resection of liversegments
4b and 5, as well as lymphadenectomy along the hepatoduodenal ligament.
Average survival time of all 42 patients was 447 days. Conclusion: While
laparasopic cholecystecomy can be considered curative for tumogrades Tis and
T1, we recommend radical repeat resection of liversegments 4b and 5, as well
as lymphadenctomy along the hepatoduodenal ligament for patients with
tumorgrades T2 and T3. The cystic stump should also be re-resected to
guarantee a clean R0-section. For tumogrades T3 and higher, 5 year survival
rate could not be prolonged even with extensive re-resection.
