Coronary artery disease (CAD) is one of the top leading causes of death worldwide. Accurate diagnosis of obstructive CAD is critical for effective management and early prevention of adverse cardiovascular events. Invasive coronary angiography (ICA) is the gold standard for the diagnosis of symptomatic patients with stable chest pain and suspected CAD, while coronary CT angiography (CCTA) is recommended by all recent guidelines as a non-invasive gatekeeper to ICA. We investigated whether combining coronary artery calcium (CAC) with the results of CCTA would improve the diagnostic performance in patients with stable chest pain and suspected CAD, especially in the case of non-diagnostic CCTA using ICA as a reference standard. We analysed 2315 patients from 29 original studies from the Collaborative Meta-Analysis of Cardiac CT (COME-CCT) dataset, which includes patients from prospective diagnostic studies that compared CCTA to ICA. All patients had available ICA results, CCTA results and CAC score. We developed two diagnostic models: one using CCTA alone and another combining CCTA with CAC score. The models’ diagnostic performance was compared based on the area under the receiver operating characteristic (ROC) curves, Brier score, scaled Brier score, discrimination slope, calibration slope, and diagnostic odds ratio (DOR). Subgroup analyses were stratified by gender, age groups (less than 45 years, 45-65 years, and over 65 years), and type of chest pain. Internal validation was performed using 250 bootstraps, the internal-external validation was based on the DOR, while the clinical consequences were validated by using 10-fold cross-validations with 100 repeats of the decision curve analysis (DCA). To replicate the clinical practice scenario, a clinical model was developed using CAC cut-off values to dichotomise the CAC score. The combination of CAC score and CCTA results significantly improved the diagnosis of obstructive coronary artery disease compared with CCTA alone. The area under the curve (AUC) for the CAC score plus CCTA model was 0.87 [95% CI: 0.86 to 0.89] compared to 0.80 [95% CI: 0.78 to 0.82] for CCTA alone (p < 0.001). Furthermore, the combination improved Brier score, scaled Brier score, discrimination slope, calibration slope, and the DOR. The clinical model increased the diagnostic accuracy for obstructive CAD to 87% [95% CI: 86 to 88%] vs 80% [95% CI: 78 to 81%] for CCTA alone (p < 0.001). The DCA showed that the CAC-plus-CCTA model had a greater net clinical benefit than CCTA alone at almost all threshold probabilities. The subgroup analyses showed consistent improvements in the diagnosis of obstructive CAD in favour of the CAC-plus-CCTA model compared with CCTA alone in almost all clinically relevant groups. Our results suggest that incorporating CAC score into routine diagnostic workflows for patients with stable chest pain and suspected CAD improves the diagnostic accuracy, especially in the case of non-diagnostic CCTA.
Die koronare Herzkrankheit (KHK) ist nach wie vor eine der häufigsten Todesursachen weltweit. Wir untersuchten, ob die Kombination von Koronarkalk-Score mit den Ergebnissen der CCTA die diagnostische Leistung bei Patienten mit stabilen Brustschmerzen und Verdacht auf KHK verbessern würde, insbesondere im Fall einer nicht-diagnostischen CCTA mit IKA als Referenzstandard. Wir analysierten 2315 Patienten aus 29 Studien der Collaborative Meta-Analysis of Cardiac CT (COME-CCT), die Patienten aus prospektiven diagnostischen Studien enthält, die CCTA mit ICA verglichen. Die Kombination von Koronarkalk-Score und CCTA-Ergebnissen verbesserte die Diagnose der obstruktiven koronaren Herzkrankheit im Vergleich zur CCTA allein erheblich. Die Subgruppenanalysen zeigten konsistente Verbesserungen bei der Diagnose einer obstruktiven KHK zugunsten des Koronarkalk-Score-CCTA-Modells im Vergleich zur alleinigen CCTA in fast allen klinisch relevanten Gruppen. Unsere Ergebnisse legen nahe, dass die Einbeziehung des Koronarkalk-Score in die Routinediagnostik bei Patienten mit stabilen Schmerzen und Verdacht auf eine KHK die diagnostische Genauigkeit verbessert, insbesondere im Falle einer nicht-diagnostischen CCTA.
