The treatment of Rockwood type V acromioclavicular (AC) joint dislocations remains controversial, as the superiority of surgical interventions over non-operative treatments continues to be debated within orthopedics. The primary aim of this study was to evaluate the clinical and radiological outcomes of non-operative treatments for acute Rockwood type V AC joint dislocations and compare them with the outcomes of operative treatments using arthroscopically-assisted stabilizations. In this retrospective case-control study, 48 patients with an acute Rockwood type V AC joint dislocation, who were initially non-operatively treated and completed a 12-month follow-up (FU), were included. This cohort was compared with 48 patients who underwent an arthroscopically-assisted coracoclavicular (CC) and acromioclavicular (AC) stabilization using a suture button technique. Both cohorts were matched in terms of age, gender, and affected side of the shoulder. Shoulder functionality was assessed using patient-reported measurements such as Subjective Shoulder Value (SSV), Nottingham Clavicle Score (NCS), Constant Score (CS), and pain visual analogue scale (VAS). The time to return to sport as well as patient satisfaction with the cosmetic appearance of their injured shoulders was also included. Radiological examination comprised CC distance, CC difference ratio, and the degree of horizontal instability. At the final FU (62 ± 43 months), no significant differences (p>0.05) were observed between both cohorts in terms of shoulder functionality as measured by SSV and NCS. However, the mean CS score of the non-operative group was higher than the operative group, with a mean score of 92.3 ±10 points and 87.7 ± 12 points, respectively (p=0.02). Patients in the non-operative group reported lower pain levels on palpation of the AC joint compared with the operative group, with a mean VAS score of 0.19 ± 0.5 vs 1.2 ± 2.2, respectively (p=0.004). At the latest FU, the CC difference ratio in the non-operative group was significantly reduced compared to post-injury levels, decreasing from 2.6 ± 0.8 to 2.0 ± 0.5, (p< 0.001). The CC difference ratio in patients treated operatively increased from 0.67 ± 0.3 at post-operation to 1.3 ± 0.3 at the latest FU, (p< 0.001), with 56% of the patients of this cohort experiencing a loss of reduction resulting in a Rockwood type III state at the latest FU. In contrast, 54% of patients treated non-operatively had spontaneous reduction of injury severity from Rockwood type V to Rockwood type III. In this study, we observed that non-operative treatment for acute Rockwood type V AC joint dislocations yielded similar clinical outcomes to surgical intervention with suture button fixation. Patients treated non-operatively had a spontaneous reduction of the AC joint with an improved CC distance at the latest FU, reducing their injury severity to a type III state in 54% of the cases. Conversely, over half of the patients in the operative group had a loss of reduction.
Die Behandlung von Akromioklavikulargelenk-(AC)-Luxationen des Rockwood-Typs V ist ein diskutiertes Thema in der Orthopädie, da bisher keine klare Überlegenheit von operativen gegenüber konservativen Methoden nachgewiesen werden konnte. Ziel dieser Studie war es, die klinischen und radiologischen Ergebnisse von konservativen und operativen Behandlungen bei akuten Rockwood-Typ-V-Luxationen zu vergleichen. In dieser retrospektiven Fall-Kontroll-Studie wurden 48 Patienten untersucht, die initial konservativ behandelt wurden und eine Nachbeobachtungszeit von mindestens 12 Monaten abgeschlossen hatten. Diese Gruppe wurde mit einer Kohorte von 48 Patienten verglichen, die operativ durch eine arthroskopisch-assistierte Stabilisierung des korakoklavikulären (CC) und akromioklavikulären (AC) Bandapparats versorgt wurden. Beide Gruppen wurden hinsichtlich Alter, Geschlecht und betroffener Schulterseite abgeglichen. Zur Bewertung der Schulterfunktionalität wurden patientenberichtete Messinstrumente wie der Subjective Shoulder Value (SSV), der Nottingham Clavicle Score (NCS), der Constant Score (CS) und eine Schmerzbewertung mittels visueller Analogskala (VAS) eingesetzt. Zusätzlich wurde die Zeit bis zur Rückkehr zum Sport und die Zufriedenheit mit dem kosmetischen Ergebnis der Schulter erfasst. Radiologische Parameter umfassten die CC-Distanz, das CC-Differenzverhältnis und den Grad der horizontalen Instabilität. Nach einer durchschnittlichen Nachbeobachtungszeit von 62 ± 43 Monaten zeigten beide Gruppen ähnliche Ergebnisse hinsichtlich SSV und NCS (p > 0,05). Die konservative Gruppe erzielte jedoch einen höheren mittleren CS-Wert (92,3 ± 10 Punkte) als die operative Gruppe (87,7 ± 12 Punkte, p = 0,02). Außerdem berichteten Patienten in der konservativen Gruppe über weniger Schmerzen bei Palpation des AC-Gelenks (VAS: 0,19 ± 0,5 vs. 1,2 ± 2,2, p = 0,004). Radiologisch verbesserte sich das CC-Differenzverhältnis in der konservativen Gruppe signifikant (von 2,6 ± 0,8 auf 2,0 ± 0,5, p < 0,001), während es in der operativen Gruppe von 0,67 ± 0,3 auf 1,3 ± 0,3 anstieg (p < 0,001). 54 % der konservativ behandelten Patienten zeigte eine spontane Verbesserung der Verletzungsschwere von Rockwood-Typ V auf Typ III. Im Gegensatz dazu wurde ein Reduktionsverlust bei 56 % der operativ behandelten Patienten festgestellt, was bei der letzten Nachuntersuchung zu einem Rockwood-Typ-III-Zustand führte. Zusammenfassend zeigt die Studie, dass konservative Behandlungen bei akuten AC-Gelenkluxationen des Rockwood-Typs V vergleichbare, teilweise sogar bessere klinische und radiologische Ergebnisse erzielen können als operative Verfahren. Während konservative Ansätze häufig zu einer spontanen Verbesserung der Gelenkstellung führten, traten in der operativen Gruppe häufiger Reduktionsverluste auf. Diese Ergebnisse legen nahe, dass eine konservative Behandlung bei dieser Verletzungsart eine effektive und schonende Alternative zur operativen Therapie darstellen kann.
