Hintergrund: Der muskuloskelettale Ultraschall (US) hat einen festen Stellenwert in der Rheumatologie und wird in der Diagnostik und Therapieverlaufskontrolle der rheumatoiden Arthritis (RA) genutzt. Trotz vieler Bestrebungen zur Standardisierung und Sicherstellung der Reproduzierbarkeit der Methode, gibt es aktuell noch keinen Konsensus über einen einheitlichen ‚composite score‘ zur sonographischen Verlaufskontrolle der RA auf Patientenebene. Ziel dieser Arbeit war es mittels Re-Evaluation und Optimierung des US7-Scores einen Beitrag zur Etablierung eines sog. ‚composite score‘ zu leisten. Dafür wurden u.a. Unterschiede zwischen dorsalen und palmaren Gelenkregionen sowie der Einfluss der Krankheitsdauer auf die Performance des Scores analysiert. Methoden: Der US7-Score umfasst die Beurteilung von sieben Gelenken und sieben Sehnen der klinisch am stärksten betroffenen Seite. Die Patient:innen wurden zu Studienbeginn, sowie 3 und 6 Monate nach Beginn oder Änderung der DMARD-Therapie im B-Bild (Greyscale, GS) und Power Doppler (PD) mittels US7-Score untersucht. Der Friedman-Test wurde zur Identifizierung häufig betroffener pathologischer Gelenkregionen/ Sehnen und der Chi-Quadrat-Test zum Vergleich der dorsalen und palmaren Regionen verwendet. Das Therapieansprechen und die erhaltenen Informationen aus reduzierten Scores wurden mithilfe des Standardized Response Mean (SRM) und einer linearen Regressionsanalyse bewertet. Subgruppenanalysen wurden separat für die frühe (<2 Jahre Krankheitsdauer) und etablierte RA (≥ 2 Jahre Krankheitsdauer) durchgeführt. Ergebnisse: Die Analyse umfasste 435 Patient:innen (80% weiblich, n=138 mit früher RA, Alter 56,5 (SD 13,1) Jahre, Krankheitsdauer 8,2 (SD 9,1) Jahre). Das dorsale Handgelenk, MCP2 (von palmar) sowie die Extensor digitorum communis (EDC) und Extensor carpi ulnaris (ECU) Sehnen wiesen die höchste Häufigkeit von GS/PD-Synovitis/Tenosynovitis auf (Handgelenk: 45%/43%; MCP2: 35%/28%; EDC: 30%/11%; ECU: 25%/11%). Diese Regionen zeigten innerhalb von 6 Monaten nach Therapiebeginn eine signifikante Verbesserung (alle p≤0,003 im GS/PD). Die dorsale Seite des Handgelenks (p<0,001 im GS/PD) und die palmare Seite der Fingergelenke (p<0,001 im PD) waren häufiger von einer Synovitis/Tenosynovitis betroffen als die jeweils andere Seite. Ein reduzierter US7- Score, bestehend aus dorsalem Handgelenk, palmarem MCP2, EDC- und ECU-Sehnen im GS/PD sowie dorsalem MCP2 im PD zeigte nach 6 Monaten ein moderates Therapieansprechen (SRM 0,443), wobei 76 % der Informationen des ursprünglichen US7-Scores erhalten blieben. Die Krankheitsdauer hatte keinen signifikanten Einfluss auf die Performance des reduzierten (US7) Scores. Fazit: Ein ‚composite score‘ auf Patientenebene zum Therapiemonitoring der RA sollte ausgehend von unserem beschriebenen reduzierten (US7) Score die am häufigsten von Synovitis/Tenosynovitis betroffenen und therapiesensitiven Gelenke/Sehnen i.e. das Handgelenk, MCP2 sowie die EDC- und ECU-Sehnen enthalten.
Background: Musculoskeletal ultrasound (US) plays an important role in rheumatology and is used for the diagnosis and therapy monitoring of rheumatoid arthritis (RA). Despite various efforts to standardize and ensure the reproducibility of the method, there is cur- rently no consensus on a ‚composite score‘ for sonographic therapy monitoring of RA on patient-level. The aim of this work was to contribute to a ‚composite score‘ by re-evaluat- ing and optimizing the US7 score, with a focus on the differences between the dorsal and palmar regions and the influence of the disease duration on the performance of the score. Methods: The US7 score includes assessment of seven joints and seven tendons of the clinically most affected side. Patients were examined by US7 score at baseline and 3 and 6 months after starting or changing DMARD therapy using greyscale (GS) and power Doppler (PD). Friedman test was used to identify commonly affected pathological joint regions/tendons and the chi-square test to compare the dorsal and palmar joint regions. The response to therapy and the information obtained from reduced scores were evalu- ated using the Standardized Response Mean (SRM) and linear regression analysis. Sub- group analyzes were performed separately for the early (<2 years disease duration) and established RA (≥2 years disease duration). Results: The analysis included 435 patients (80% female, with 138 cases of early RA) with a mean age of 56.5 (SD 13.1) years and a mean disease duration of 8.2 (9.1) years. The dorsal wrist, MCP2 palmar, and the extensor digitorum communis (EDC) and exten- sor carpi ulnaris (ECU) tendons had the highest frequency of GS/PD synovitis/tenosynovitis (wrist: 45%/43%; MCP2: 35%/ 28%, EDC: 30%/11%, ECU: 25%/11%). These regions showed significant improvement within 6 months after therapy (all p≤0.003 by GS/PD). The dorsal side of the wrist (p<0.001 by GS/PD) and the palmar side of the finger joints (p<0.001 by PD) were more frequently affected by synovitis/tenosynovitis than the opposite side. A reduced (US7) score, consisting of the dorsal wrist, palmar MCP2, EDC and ECU tendons by GS/PD and dorsal MCP2 by PD, showed a moderate response to therapy (SRM 0.443) after 6 months, while retaining of 76% of the information of the original US7 score. The disease duration had no significant impact on the performance of the reduced (US7) score. Conclusion: A 'composite score' for therapy monitoring of RA on patient-level should be based on our described reduced (US7) score that compromises the joints/tendons most frequently affected by synovitis/tenosynovitis while being sensitive to therapy (i.e. wrist, MCP2, EDC and ECU tendons).
