Prediction and prevention of disease progression in early axial spondyloarthritis

Abstract

Disease progression in axial spondyloarthritis (SpA) refers to the development of structural damage in the sacroiliac joints and in the spine. In the German Spondyloarthritis Inception Cohort (GESPIC), which includes patients with early axial SpA (ankylosing spondylitis – AS – with symptom duration up to 10 years and non-radiographic axial SpA with symptom duration up to 5 years), we investigated rates and predictors of structural damage progression in the sacroiliac joints and in the spine. In the sacroiliac joints, the rate of definite sacroiliitis development (i.e. progression from non-radiographic axial SpA to AS) was 11.6% after two years. Elevated level of C-reactive protein (CRP) was found to be the only significant predictor of progression from non-radiographic axial SpA to AS fulfilling the modified New York criteria. In the spine, the rate of radiographic progression over two years was 0.95 ± 2.78 mSASSS (modified Stoke Ankylosing Spondylitis Spine Score) units in AS and 0.46 ± 1.63 mSASSS units in non-radiographic axial SpA. The following factors were independently predictive for significant radiographic spinal progression (mSASSS worsening in 2 units and more or development of new syndesmophytes) over two years: the presence of structural damage (syndesmophytes) at baseline, elevated acute phase reactants (CRP or erythrocyte sedimentation rate – ESR) and smoking. Furthermore, smoking demonstrated a dose-dependent impact on the development of structural damage in the spine in axial SpA with the highest progression rate (mSASSS worsening 2.2 ± 4.6 over 2 years) in patients who smoked more than 10 cigarettes a day. Further, several biomarkers with predictive/protective value for progression of structural damage in the spine were identified, among others dickkopf-1 (with protective value) and vascular endothelial growth factor – VEGF (with predictive value). Elevated serum level of VEGF (>600 pg/ml) was found to be highly specific as a predictor of radiographic spinal progression in patients with axial SpA, especially in patients who were at high risk for such a progression due to the presence of syndesmophytes. Therefore, VEGF is a biomarker which is able to improve the predictive model for structural damage progression in the spine based on syndesmophytes, acute phase reactants and smoking status assessment. We were able to demonstrate that high non-steroidal anti-inflammatory drugs (NSAIDs) intake (more than 50% of the maximal recommended dose over time) is able to retard progression of structural damage in the spine in axial SpA. AS patients with high NSAIDs intake had a significantly lower likelihood of radiographic progression even after adjustment for baseline structural damage, elevated CRP and smoking status. Importantly, the positive effect of the NSAIDs intake on progression of structural damage in the spine was nearly exclusively evident in patients who were at high risk for radiographic progression due to the presence of syndesmophytes and elevated CRP. Thus, investigation of predictors of structural damage development opened ways for prevention of such a progression and, therefore, for improvement of the long-term outcome. These ways are: early initiation of effective anti-inflammatory therapy, smoking cessation, and continuous administration of NSAIDs (unless contraindicated) in symptomatic patients, especially in the presence of syndesmophytes and elevated CRP.

Unter Krankheitsprogression bei axialer Spondyloarthritis (SpA) ist in der Regel die Entstehung von strukturellen Schäden in den Sakroiliakalgelenken und in der Wirbelsäule gemeint. Wir untersuchten das Ausmaß und die Prädiktoren der Progression von solchen strukturellen Schäden bei Patienten mit früher axialer Spondyloarthritis (ankylosierender Spondylitis – AS – mit Symptomdauer bis zu 10 Jahren und nicht-röntgenologischer axialer SpA mit Symptomdauer bis zu 5 Jahren) in der GESPIC (German Spondyloarthritis Inception Cohort) Kohorte. In den Sakroiliakalgelenken betrug die Rate der Entstehung der definitiven röntgenologischen Sakroiliitis (was eine Progression von der nicht-röntgenologischen axialen SpA zur AS bedeutet) 11,6% nach 2 Jahren. Erhöhtes C-reaktives Protein (CRP) war der einzige signifikante Prädiktor dieser Progression. In der Wirbelsäule beobachteten wir eine mittlere röntgenologische Progressionsrate von 0,95 ± 2,78 mSASSS (modified Stoke Ankylosing Spondylitis Spine Score) Einheiten bei der AS und 0,46 ± 1,63 mSASSS Einheiten bei der nicht-röntgenologischen axialen SpA. Als unabhängige mit einer röntgenologischen Progression in der Wirbelsäule (Verschlechterung des mittleren mSASSS-Scores um ≥2 Einheiten bzw. Syndesmophytenneubildung über zwei Jahre) assoziierte prädiktive Faktoren wurden das Vorliegen von Syndesmophyten zu Baseline, erhöhte Marker der systemischen Inflammation (CRP bzw. Blutsenkungsgeschwindigkeit) und Rauchen identifiziert. Außerdem zeigte das Rauchen einen dosisabhängigen Einfluss auf die Progression von strukturellen Schäden in der Wirbelsäule bei axialer SpA, wobei die höchste Progressionsrate (mittlere mSASSS-Verschlechterung von 2,2 ± 4,6 nach 2 Jahren) bei Patienten, die im Durchschnitt mehr als 10 Zigaretten pro Tag geraucht hatten, beobachtet wurde. Des Weiteren konnten wir eine Reihe von Biomarken mit prädiktiver bzw. protektiver Bedeutung für die Progression von strukturellen Schäden in der Wirbelsäule identifizieren, unter anderem Dickkopf-1 – mit protektivem Wert, und der endotheliale Gefäßwachstumsfaktor (vascular endothelial growth factor – VEGF) – mit prädiktivem Wert. Ein erhöhter VEGF-Serumspiegel (>600 pg/ml) zeigte sich als spezifischer Prädiktor der röntgenologischen Progression in der Wirbelsäule bei axialer SpA, vor allem bei Patienten mit bereits vorhandenen Syndesmophyten. Somit kann der VEGF das prädiktive Modell für röntgenologische Progression, das auf Syndesmophyten, Entzündungsparametern und Raucher-Status basiert, verbessern. Darüber hinaus konnten wir zeigen, dass eine hohe Einnahme von nichtsteroidalen Antirheumatika (mehr als 50% der maximalen zugelassenen Dosis über die Zeit) die Progression von strukturellen Veränderungen in der Wirbelsäule bei axialer SpA verlangsamen kann. AS-Patienten mit hoher NSAR- Einnahme hatten eine signifikant niedrigere Wahrscheinlichkeit der röntgenologischen Progression in der Wirbelsäule auch nach Adjustierung für das Vorhandensein von Syndesmophyten zu Baseline, erhöhtem CRP und Raucher- Status. Wichtig ist dabei, dass die positive Auswirkung von NSAR auf die röntgenologische Progression fast ausschließlich bei Patienten mit Risikofaktoren für diese Progression (Syndesmophyten und erhöhtes CRP) beobachtet wurde. Die Untersuchungen von Prädiktoren für die Entstehung von strukturellen Schäden bei axialer SpA bereiten den Weg und zeigen Maßnahme zur Prävention solcher Progression und damit auch zur Verbesserung von langzeitigem Therapieergebnis auf. Zu den Maßnahmen zählen: früher Beginn einer antientzündlichen Therapie, Verzicht auf Rauchen, und eine kontinuierliche Gabe von NSAR (bei fehlenden Kontraindikationen) bei symptomatischen Patienten, insbesondere bei vorhandenen Syndesmophyten und erhöhtem CRP.
