CYP2C9 katalysiert die Biotransforamtion der oralen Antikoagulantien
S-Warfarin, S- und R-Acenocoumarol. Wie bereits in in vitro Studien
festgestellt wurde, wird auch der Vitamin K-Antagonist Phenprocoumon über
CYP2C9 metabolisiert. Jedoch gibt es bisher noch keine sicheren in vivo
Studien, die den Einfluß von CYP2C9-Polymorphismen bestätigen. 28 gesunde
Probanden, mit den hetero- und homozygoten CYP2C9 Allelvarianten *1 (Wildtyp),
*2 (Arg144Cys) und *3 (Ile359Leu) erhielten eine orale Einmaldosis von 12 mg
Phenprocoumon in Kombination mit Vitamin K (Konakion ®), um größere
Blutungskomplikationen zu vermeiden. Gemessen wurden die Plasma- und 12
Stundensammelurin-Konzentrationen der beiden Enantiomere sowie der 4`-, 6`-
und 7`- OH-Metabolite mittels HPLC (High performance Liquid Chromatographie)
und Massenspektrometer. Für R- Phenprocoumon wurde kein signifikanter Effekt
der *2- und *3-Allelvarianten festgestellt. Für S-Phenprocoumon jedoch zeigte
sich bei zunehmender Anzahl an *2- und *3-Allelen eine reduzierte Clearance.
Die S/R- Ratio der Plasmaclearance von Phenprocoumon war bei
*1/*1-Allelträgern mit einem Median von 0.95 höher, als bei *3/*3-Allelträgern
(0.65). Die Plasma- und Urinkonzentrationen von 4`-, 6`- und 7`- OH-
Phenprocoumon waren in homozygoten*2- und *3-Allelträgern signifikant
erniedrigt. Verglichen mit dem Wildtyp wiesen CYP2C9*3/*3-Allelträger eine
mittlere AUC für (R,S) 7`-OH-Phenprocoumon von ca. 25% auf. Die AUC für (R,S)
6`-OH-Phenprocoumon für für homozygote *3- Allelträger betrug nur ca. 50% der
AUC des Wildtyps. Bezogen auf die Hydroxilierung von Phenprocoumon zeigten
CYP2C9*2- und *3-Allelträger eine erniedrigte metabolische Kapazität, als der
Wildtyp. Jedoch zeigte sich in dieser Studie kein signifikanter Effekt der
CYP2C9-Polymorphismen auf die Pharmakokinetik von Phenprocoumon.

dc.description.abstract
CYP2C9 catalyses the biotransformation of the oral anticoagulants S-warfarin
and S- and R-acenocoumarol. According in vitro data, phenprocoumon, a vitamin
K-antagonist, is also metabolized by CYP2C9. The impact of the CYP2C9
polymorphism on phenprocoumon pharmacokinetics has yet not been studied. 28
healthy heterozygous and homozygous carriers of CYP2C9*1 (wild-type), *2
(Arg144Cys) and *3 (Ile359Leu) alleles received a single oral dose of 12 mg of
racemic phenprocoumon in combination with vitamin K (Konakion ®) to avoid
major bleeding complications. Plasma and 12-h urine concentrations of both
enantiomers and their monohydroxylated metabolites were measured by high-
performance liquid chromatography with mass spectometry detection. No
significant effect of the CYP2C9 variants *2 and *3 on R-phenprocoumon
pharmacokinetic parameters were detected whereas S-phenprocoumon clearance
tended to decrease with increasing number of *2 and *3 alleles. The ratio of
S- to R-phenprocoumon plasma clearance were higher with a median of 0.95 in
carriers of *1/*1 versus 0.65 in *3/*3. Plasma and urine concentrations of
4`-, 6`- and 7`-hydroxyphenprocoumon were significantly lower in homozygous
carriers of the CYP2C9*2 and *3 variants compared to the wild- type. Carriers
of CYP2C9*3/*3 had a median AUC of (R,S) 7`-hydroxyphenprocoumon of only
approximately 25% compared to CYP2C9*1/*1. The AUC of (R,S) 6`- OH-
phenprocoumon was only approximately 50% in CYP2C9*3/*3 compared to the
homozygous wild-type. In conclusion *2 and *3 alleles had a lower capacity
regarding phenprocoumon hydroxylation than those with *1/*1. However,
regarding phenprocoumon hydroxylation CYP2C9 genotypes had only marginal
effects on S- and R-phenprocoumon total clearance.
