Clopidogrel, ein irreversibler P2Y12 Rezeptorantagonist der Thrombozyten,
hemmt die ADP-induzierte Plättchenaktivierung und damit letztendlich die
Plättchenaggregation. Nach einer Thrombozytenaktivierung sekrieren die
Plättchen Tissue factor (TF) und CD40L. P-Selektin (CD62P) lässt sich auf der
Oberfläche aktivierter Plättchen nachweisen. Bei Patienten mit einem Akuten
Koronarsyndrom (ACS) sind als Zeichen der erhöhten Plättchenfunktion erhöhte
TF-Konzentrationen im Blut festgestellt worden1,2. Zur Bestimmung der
Plättchenreaktivität stehen verschiedene Messmethoden zur Verfügung. Große
interindividuelle Unterschiede im Ansprechen auf Clopidogrel wurden in der
Literatur beschrieben, wobei die Inzidenz von sogenannten Clopidogrel „Low-
respondern“ zwischen 5% und 25 % in Abhängigkeit vom Messverfahren
schwankt3,4. Zur Bestimmung der Plättchenaggregation unter einer Therapie mit
Clopidogrel bei Patienten mit einer Koronaren Herzkrankheit (KHK) wurden die
Impedanzaggregometrie sowie die Bestimmung des Status der VASP-
Phosphorylierung und die Messung des Oberflächenmarkers P-Selektin in dieser
Arbeit benutzt. Neben Thrombozytenaggregationshemmern werden zur Therapie der
KHK auch HMG-CoA Reduktasehemmer eingesetzt. Statine besitzen neben ihren den
Fettstoffwechsel beeinflussenden Eigenschaften auch antithrombozytäre,
antikoagulatorische und profibrinolytische Wirkungen5-8. In dieser Arbeit
wurden 1) drei verschiedene Methoden zur Erfassung der Plättchenaggregation
miteinander verglichen sowie der Einfluss einer Clopidogreldosis von 150
mg/Tag bei sogenannten Clopidogrel „Low-respondern“ auf die Reduzierung der
Plättchenreaktivität untersucht; 2) der Einfluss von Clopidogrel auf die Höhe
der zirkulierenden TF - Konzentration im Blut bei Patienten mit stabiler KHK,
ST- Hebungs-Infarkt und bei gesunden Kontrollen untersucht. Besteht eine
Beziehung zwischen der TF-Konzentration und der Thrombozytenreaktivität,
gemessen an der sCD40L-Konzentration im Plasma? 3) analysiert, inwiefern die
unter Clopidogreltherapie gehemmte Plättchen-degranulation und thrombozytäre
Zytokinausschüttung durch eine Statintherapie mit 40 mg Atorvastatin im
Vergleich zu einer Kombinationstherapie aus 10 mg Atorvastatin und 10 mg
Ezetimibe weiter reduziert wird. Signifikante Korrelationen bestehen zwischen
dem Status der VASP-Phosphorylierung und der Impedanzaggregometrie. Eine
Clopidogreldosis von 150 mg/Tag führte nur bei sechs von zehn Patienten zu
einer Reduktion der Plättchenaggregation. Ferner reduzierte die Therapie mit
Clopidogrel die TF- Konzentration im Blut von Patienten mit stabiler KHK. Die
Hemmung der Plättchendegranulation führte über eine verminderte Ausschüttung
von TF aus den α-Granula zu einer Reduzierung der Plättchen vermittelten TF-
Expression in Endothelzellen und Monozyten. Bei Patienten mit einem akuten
Ereignis war diese Reduktion der TF-Konzentration unter Therapie mit
Clopidogrel nicht zu beobachten. Bei vergleichbarer Reduzierung der LDL-
Cholesterin Spiegel führte eine Monotherapie mit 40 mg Atorvastatin zu einer
signifikanten Abnahme der Plättchenaggregation.

dc.description.abstract
Clopidogrel, an irreversible P2Y12 receptor antagonist, inhibits the ADP-
induced platelet activation and finally the platelet aggregation. Activated
platelets express Tissue factor (TF) and CD40L. Also P-selectin (CD62P) is
exposed upon activation on platelet surface. Increased concentration of TF in
blood were found in patients with an acute coronary syndrome1,2. Different
tests are availabel for quantifying platelet function. In literature a large
inter-individual variability in reply to clopidogrel is described. The
incidence of so-called low-responder fluctuates from 5% to 25% depending on
the diagnostic tests3,4. For measuring platelet aggregation beneath
clopidogrel therapy we used the impedance aggregometry, the assessment of the
state of VASP-phosphorylation and the flow cytometrical detection of
P-selectin (CD62-P). In addition HMG-CoA reductase inhibitors (statins) were
used for therapy in patients with coronary artery disease. Statins have also a
lot of pleiotropic effects5-8. In this study 1) three different methods of
measuring platelet aggregation were compared to each other. Furthermore, we
examined whether doubling the clopidogrel dose to 150 mg/d in so-called
clopidogrel low-responder would lead to a reduction in platelet aggregation.
2) We investigated the effect of clopidogrel on circulating TF in the blood of
patients with stable CAD, ST-elevation myocardial infarction and healthy
controls. Is there a relationship between plasma TF and the platelet
degranulation marker sCD40L? 3) We analyzed if combined treatment with
ezetimibe plus low-dose atorvastatin had different effects on platelet
function and plasma chemokine levels than highe-dose therapy with atorvastatin
alone. There were significant correlations between the state of VASP-
phosphorylation and the impedance aggregometry. Doubling the clopidogrel dose
to 150 mg/d was effective to reduce the agonist-induced platelet aggregation
in only 6 of 10 patients. Moreover, clopidogrel reduced the TF concentration
in the blood of patients with stable CAD. Lowering platelet degranulation may
lead to a decrease in platelet release of TF and thereby to a reduction in
platelet-mediated TF expression in endothelium cells and monocyte. No
reduction in TF concentration in blood was found in patient with an acute
coronary syndrome. A therapy with 40 mg/d atorvastatin but not with 10 mg/d
ezetimibe plus 10 mg/d atorvastatin reduced significantly the platelet
aggregation despite a similar decrease of LDL-cholesterol.
