Einleitung. Die antenatale Glukokortikoidbehandlung mit Betamethason (BET) bei drohender Frühgeburt wird weltweit zur Lungenreifeinduktion durchgeführt und reduziert die neonatale Morbidität und Mortalität. Jedoch kann sie insbesondere bei hohen, repetitiven Dosen zu einem verminderten fetalen Wachstum, sowie zu einer negativen Beeinflussung des Gesundheitszustands der Neugeborenen bis ins Erwachsenenalter führen. Um die Mechanismen genauer zu verstehen, bedarf es Genexpressionsstudien an der humanen Plazenta, der als Schnittstelle zwischen Mutter und Kind eine besondere Rolle zukommt. Eine geläufige Methode ist die quantitative real-time reverse Transkriptase Polymerase-Kettenreaktion (RT-qPCR). Deren Verlässlichkeit ist jedoch stark abhängig von der Verwendung valider interner Kontrollgene (ICG) zur Normalisierung von Zielgenen. Zielsetzung. Ziel dieser Studie war es, für Glukokortikoidstudien in der humanen Plazenta eine geeignete Gruppe aus ICG zur Validierung von RT-qPCR-Experimenten zu finden. Anhand des Glukokortikoidrezeptors (NR3C1) als Zielgen sollte gezeigt werden, dass die mRNA-Expression alleine durch eine unterschiedliche und inadäquate Auswahl der ICG signifikant variieren kann. Methodik. Sieben ICG (B2M, HMBS, HPRT1, PPIA, RPL19, SDHA und YWHAZ) wurden durch RT-qPCR-Versuche in der humanen Plazenta (n=96) auf ihre Expressionsstabilität bezüglich der Faktoren antenatale BET- Gabe, Schwangerschaftsalter bei Geburt, fetales Geschlecht und plazentare Lokalisation der Proben untersucht. Dafür wurden zwei Verfahren angewandt: (1) In einer „klassischen“ Analyse wurde jedes einzelne ICG statistisch ausgewertet und auf signifikante Gruppenunterschiede getestet. (2) Mithilfe der Software-basierten Algorithmen geNorm, NormFinder und BestKeeper wurde ein Ranking der ICG entsprechend ihrer Expressionsstabilität erstellt. Anschließend wurden die Rohdaten des Zielgens NR3C1 mit verschiedenen ICG beziehungsweise ICG-Kombinationen normalisiert und die Ergebnisse miteinander verglichen. Ergebnisse. Während in den peripheren Proben keine signifikanten Gruppenunterschiede nachweisbar waren, zeigten sich nach BET-Behandlung in den zentralen Proben bei fünf der sieben getesteten ICG signifikante Effekte, besonders in der Gruppe der weiblichen Neugeborenen. Zudem fand sich eine Herabregulation der HPRT1- und PPIA-Expression bei fortschreitendem Schwangerschaftsalter. Demnach wären nach der „klassischen“ Analyse nur SDHA und YWHAZ zur Normalisierung geeignet. Die Software-basierte Analyse befand dagegen die Kombination aus HMBS, PPIA, RPL19 und SDHA als am besten geeignet. Je nachdem, welches ICG beziehungsweise welche ICG-Kombination zur Normalisierung des NR3C1-Gens eingesetzt wurde, lieferte die statistische Auswertung unterschiedliche Ergebnisse. Schlussfolgerung. Die Verwendung verschiedener Normalisierungsverfahren kann die Ergebnisse von Genexpressionsstudien signifikant beeinflussen. Da die Auswahl der ICG einen wesentlichen Einfluss auf die Ergebnisse hat, sind die Evaluation und der Einsatz geeigneter ICG für eine akkurate und valide Normalisierung notwendig. Für Glukokortikoidstudien in der humanen Plazenta wird die Kombination aus HMBS, PPIA, RPL19 und SDHA zur Normalisierung empfohlen.
Introduction. The antenatal glucocorticoid treatment betamethasone (BET) is applied worldwide to induce lung maturation in cases of imminent premature delivery, reducing neonatal morbidity and mortality. However, the treatment can reduce fetal growth as well as the neonate’s state of health until adulthood, especially when given in high, repetitive doses. The placenta plays a special role during treatment as the interface be-tween mother and child. For a better understanding of the underlying mechanisms, gene expression studies of the human placenta are needed. A common tool is quantitative real- time reverse transcriptase polymerase chain reaction (RT-qPCR), though its reliability is strongly dependent on the use of valid internal control genes (ICGs) for the normalisation of target genes. Objective. The aim of this study was to find a suitable set of ICGs for RT-qPCR assays in glucocorticoid studies of the human placenta. The glucocorticoid receptor (NR3C1) was used as a target gene to demonstrate that mRNA expression can vary significantly solely due to the selection of different and inadequate ICGs. Methods. Seven ICGs (B2M, HMBS, HPRT1, PPIA, RPL19, SDHA and YWHAZ) were investigated in RT- qPCR experiments of the human placenta for their expression stability in relation to antenatal BET treatment, gestational age at birth, fetal sex and placental localisation of the samples. Two different procedures were applied: (1) In a “classical” analysis each single ICG was statistically investigated and tested for significant group differences. (2) Using the software-based algorithms geNorm, NormFinder and BestKeeper a ranking of the ICGs was generated corresponding to their expres-sion stability. Subsequently, the raw data of the target gene NR3C1 was normalised with different ICGs and ICG combinations respectively, and its results were compared. Results. There were no significant group differences detected in the peripheral samples. However, BET treatment lead to significant effects in the central samples in five of the seven ICGs, especially in the female neonates. According to the “classical” analysis a downregulation of HPRT1 and PPIA expression was found with progressive gestational age indicating that only SDHA and YWHAZ would be suitable for normalisation. However, the software-based analysis evaluated HMBS, PPIA, RPL19 and SDHA as the most appropriate combination of ICGs for normalisation. Depending on the selection of ICGs for the normalisation of NR3C1, different results were revealed by the statistical analyses. Conclusion. The use of different methods for normalisation can significantly influence the results of gene expression studies. Since the selection of ICGs has an essential impact on the results, the evaluation and the use of suitable ICGs are necessary for an accurate and valid normalisation. The combination of HMBS, PPIA, RPL19 and SDHA is recommended for target gene normalisation in glucocorticoid studies of the human placenta.
