Hochdosis-Methotrexat (HD-MTX) gilt als wirksamste Primärtherapieoption primärer Lymphome des zentralen Nervensystems (PCNSL). Oftmals treten in diesem Kontext potenziell prognoselimitierende schwere Akuttoxizitäten auf. Es wurden jedoch noch keine Prognosefaktoren etabliert, mit denen eine individuell risikoadaptierte HD-MTX-Therapie planbar ist. Diese Arbeit verfolgte zwei Ziele: 1. Erstmalige Identifizierung unabhängiger Prognosefaktoren für Akuttoxizitäten der HD-MTX-Therapie bei PCNSL, 2. Entwicklung von Prognosefaktor basierten Prognosepunktwertsystemen zur prätherapeutischen Beurteilung des individuellen Patientenrisikos für ausgewählte Akuttoxizitäten. Datengrundlage dieser Arbeit war eine Phase 3-Studie mit 526 auswertbaren immunkompetenten PCNSL-Patienten unter Therapie mit HD-MTX oder HD-MTX+Ifosfamid (G-PCNSL-SG-1-Studie). Zur Identifizierung von Prognosefaktoren wurden patientenassoziierte, vor Therapiebeginn verfügbare Variablen (Baseline-Variablen) sowie Akuttoxizitäten der HD-MTX- Therapie (Einteilung nach WHO) mittels univariater und multivariater Analysen auf statistische Signifikanz hin überprüft. Die Entwicklung von Prognosepunktwertsystemen erfolgte durch Berechnung eines individuellen Toxizitätsprognosepunktwertes für jeden Studienpatienten und eine anschließende Zusammenfassung der Studienpatienten in drei Risikogruppen für jede Toxizität. Folgende Baseline-Variablen konnten als Prognosefaktoren für die jeweiligen Toxizitäten identifiziert werden (p < 0,1): Alter ≥ 60 vs. < 60 Jahre für Hämoglobin, Leukozyten, Granulozyten, Thrombozyten, Hämatotoxizität (Hämoglobin, Leukozyten, Granulozyten und Thrombozyten kombiniert), Mukositis und Infektion; Karnofsky-Index < 70 vs. ≥ 70 % für Thrombozyten und Vigilanz; Geschlecht weiblich vs. männlich für Hämoglobin, Granulozyten und Hämatotoxizität; Body Mass Index (BMI) > 25 vs. ≤ 25 kg/m2 für Hämoglobin; BMI ≤ 25 vs. > 25 kg/m2 für Vigilanz; Laktatdehydrogenase im Serum erhöht ja vs. nein für Lunge und Infektion; Lungenfunktionsstörung vorhanden ja vs. nein für Thrombozyten, Hämatotoxizität und Vigilanz; kardiovaskuläre Erkrankung vorhanden ja vs. nein für Herzfunktion; kardiovaskuläre Erkrankung vorhanden nein vs. ja für Granulozyten und Transaminasen; Therapieregime HD- MTX+Ifosfamid vs. HD-MTX für Hämoglobin, Leukozyten, Granulozyten, Thrombozyten, Hämatotoxizität, Lunge, Infektion und Herzfunktion. Die Prognosepunktwert¬systeme ergaben für die Hämatotoxizität ein Auftrittsrisiko von 20,0 %, 34,2 % und 44,4 % für die Niedrig-, Intermediär- und Hochrisikogruppe und für die Toxizität Infektion respektive 17,5 %, 27,1 % und 45,3 %. Erstmalig wurden für das Auftreten schwerer Akuttoxizitäten der HD- MTX-Therapie bei PCNSL valide Prognosefaktoren identifiziert und darauf aufbauend für zwei Akuttoxizitäten Prognose¬punktwert¬systeme entwickelt. Diese Ergebnisse könnten zukünftig für die Entwicklung individualisierter HD- MTX basierter Therapieansätze bei PCNSL-Patienten hilfreich sein.
High-dose methotrexate (HD-MTX) is commonly accepted as the most effective option for first-line therapy in primary central nervous system lymphoma (PCNSL). In this context, patients often suffer from acute toxicities potentially limiting their prognosis. However, prognostic factors that may be helpful for developing individual risk-adapted HD-MTX-based treatment strategies have not been established yet. This study focused on two aspects: 1. Identification of independent prognostic factors for acute HD-MTX-related toxicities in PCNSL for the first time ever, 2. Development of prognostic scoring systems to assess the individual risk of the patient for selected acute toxicities before treatment implementation. This study was based on a phase 3 trial with 526 evaluable immunocompetent PCNSL patients undergoing therapy with HD-MTX or HD-MTX+ifosfamide (G-PCNSL-SG-1 study). Patient- related, pre-therapeutic variables (baseline variables) and acute HD-MTX toxicities were included in univariate and multivariate analyses to identify prognostic factors. The prognostic scoring systems were developed by calculating individual prognostic scores for each patient, followed by deployment of three patient risk groups for each toxicity. The following baseline variables were identified as prognostic factors for the respective toxicities (p < 0.1): age ≥ 60 vs. < 60 years for hemoglobin, leukocytes, granulocytes, platelets, hematologic toxicity (leukocytes, granulocytes, hemoglobin and platelets combined), mucositis and infection; Karnofsky performance status < 70 vs. ≥ 70 % for platelets and state of consciousness; gender female vs. male for hemoglobin, granulocytes and hematologic toxicity; body mass index (BMI) > 25 vs. ≤ 25 kg/m2 for hemoglobin; BMI ≤ 25 vs. > 25 kg/m2 for state of consciousness; elevated serum lactate dehydrogenase yes vs. no for pulmonary toxicity and infection; preexisting pulmonary disease yes vs. no for platelets, hematologic toxicity and state of consciousness; preexisting cardiovascular disease yes vs. no for cardiac function; preexisting cardiovascular disease no vs. yes for granulocytes and transaminases; therapeutic regimen (HD-MTX+ifosfamide vs. HD-MTX) for hemoglobin, leukocytes, granulocytes, platelets, hematologic toxicity, pulmonary toxicity, infection and cardiac function. The prognostic scoring systems demonstrated a risk to develop hematologic toxicity of 20%, 34.2% and 44.4% in the low, intermediate and high risk group. The respective percentages for infection were 17.5%, 27.1% and 45.3%. For the first time ever, valid prognostic factors were identified and prognostic scoring systems were developed for acute toxicities from HD-MTX-based treatment in PCNSL. These findings could be of high relevance for the development of individualized HD-MTX-based treatment approaches for PCNSL patients in the future.
