Introduction: Limiting data exist concerning chemokine profile and new biomarkers of patients (pts) under belatacept (BEL) based therapy after renal transplantation (tx).
The evaluation of certain chemokines and biomarkers may offer an opportunity to explore the effect and the impact of different immunsuppressive regimen.
Methods: N = 18 pts were converted from a calcineurin inhibitor (CNI) or inhibitor of mammali-an target of rapamycin (mTORi) to a BEL-based regimen compared to control pts (n = 18) with continued CNI or mTORi therapy (matched for age, sex, and renal function). Serum and Plasma samples were collected for baseline (BL) and at month 3, 6, 9 (M3, M6, M9). 23 analytes analyzed by the Luminex-based multiplex technique. Neutrophil gelatinase-associated lipocalin (NGAL) and Calprotectin concentration were measured at BL and Mo3 by ELISA.
Results: After conversion to Bel pts that were converted from CNI express higher levels of CCL1 (BL: 5,9 vs. M6: 8,8; p = 0,005) and CCL8 (BL: 41,9 vs. M6: 56, p = 0,005). Secretion of CLL8 rises in Bel pts after conversion from mTORi (BL: 33,1 vs. M9: 42,5; p = 0,021). Pretreatment with CNI lead to a later increase of CCL15 (BL: 5377,7 vs. M9: 10901; p = 0,005) than with mTORi (BL: 8572,5 vs. M3: 9985,6; p = 0,011). CXCL12 expression increases under BEL treat-ment in mTORi converted pts (BL: 2791,7 vs. M3: 3397,1; p = 0,021) and in CNI converted pts between BL and Mo9 (BL: 3250,5 vs. M9: 4072,6; p = 0,037). Biomarker NGAL showed an increased level in pts converted from mTORi to Bel (BL: 166,3 vs. M3: 270,3; p = 0,038). The therapy regimen had no effect on Calprotectin after conversion.
Conclusion: Conversion to Belatacept led to an increase of several chemokines and NGAL in the patients who were converted from mTORi. This could be due to the different mechanisms of action of the CNI, mTORi and Belatacept to inhibit T-cell activation and proliferation. However, compared to the standard immunosuppression with CNI and mTORi, with its side effect profiles, belatacept represents an alternative treatment option for rejection prophylaxis in NTx patients. Due to its low specificity, calprotectin is not suitable as a predictive marker of kidney damage and the potential of NGAL still needs approval. Research into biomarkers for diagnosing kidney damage is necessary as part of further conversion studies.

Einleitung: Ziel der Studie war die Bestimmung von Chemokinen nach Konversion auf Belatacept
gegenüber der konventionellen Standardimmunsuppression mit Calcineurininhibitoren
(CNI) oder mammalian-Target-Of-Rapamycin-Inhibitoren (mTORi) nach einer Nierentransplantation
(NTx) sowie die Bewertung der Biomarker Neutrophilen-Gelatinase-assoziiertes Lipocalin
(NGAL) und Calprotectin als prognostische Prädiktoren der Nierenschädigung.
Methodik: Die Konversion des CNI- und mTORi-basierten Therapieregimes auf Belatacept
(n = 18) wurde der konventionellen Therapie mit CNI und mTORi (n = 18) gegenüberstellt. Es
erfolgte ein Matching der auf Belatacept konvertierten Patienten zu deren Kontrollpatienten hinsichtlich
des Alters, Geschlechts und der Nierenfunktion. Serum- und Plasmaproben wurden zum
Baseline-Zeitpunkt (BL), Monat drei (M3), Monat sechs (M6) und Monat neun (M9) aliquotiert.
23 Chemokinen wurden mittels Multiplex Technik genauer betrachtet. Zwei Biomarker, NGAL
und Calprotectin wurden zum BL und nach M3 mit einem enzymgekoppelter Immunadsorptionstest
(ELISA) gemessen (ng/ml).
Ergebnisse: Nach Konversion von CNI auf Belatacept zeigte die Konversionsgruppe eine stärkere
Expression von CCL1 (BL: 5,9 vs. M6: 8,8; p = 0,01) und CCL8 (BL: 41,9 vs. M6: 56, p = 0,01).
Die Sekretion des CLL8 nahm in den von mTORi auf Bel konvertierten Patienten zu (BL: 33,1 vs.
M9: 42,5; p = 0,02). Auch war ein Anstieg des CCL15 sowohl in der von CNI auf Bel konvertierten
Gruppe (BL: 5.377,7 vs. M9: 10.901; p = 0,01) als auch in den von mTORi konvertierten Patienten
(BL: 8.572,5 vs. M3: 9.985,6; p = 0,01) zu verzeichnen. Die CXCL12 Expression nahm
nach Konversion auf Belatacept von mTORi (BL: 2.791,7 vs. M3: 3.397,1;
p = 0,02) sowie von CNI zu (BL: 3.250,5 vs. M9: 4.072,6; p = 0,03). NGAL zeigte einen Anstieg
in der Konversionsgruppe von mTORi auf Belatacept (BL: 166,3 vs. M3: 270,3; p = 0,03). Währenddessen
hatten die unterschiedlichen Therapieregime keinen Effekt auf Calprotectin nach Konversion.
Schlussfolgerung: Die Konversion auf Belatacept führte zur Zunahme einzelner Chemokine und
des NGAL in den von mTORi auf Belatacept konvertierten Patienten. Dies könnte möglicherweise
auf die unterschiedlichen Wirkmechanismen des CNI, mTORi und Belatacept zur Hemmung
der T-Zellaktivierung und -proliferation zurückzuführen sein. Dennoch stellt das Belatacept
gegenüber der Standardimmunsuppression mit CNI und mTORi mit ihren ungünstigen Nebenwirkungsprofilen eine weitere Therapiemöglichkeit in NTx-Patienten zur Abstoßungsprophylaxe dar.
Das Calprotectin eignete sich aufgrund seiner geringen Spezifität nicht als prädiktiver Marker der
Nierenschädigung. Der Einsatz von NGAL als Surrogatparameter sollte weiter erforscht werden
sowie weitere Biomarker zur Diagnostik der Nierenschädigung im Rahmen von weiteren Umstellungsstudien.
