Die Tiefe Hirnstimulation im Nucleus subthalamicus (STN-THS) ist eine gut etablierte Therapieoption bei Patient*innen mit dem idiopathischem Parkinson-Syndrom (iPS) zur Verbesserung motorischer Symptome und der Lebensqualität. Bezüglich des kognitiven Outcomes ist die Datenlage jedoch uneindeutig.
Unterschiedliche Studien legen nahe, dass die Volumina des Nucleus basalis Meynerts (Ch 4), dem Hauptkern des cholinergen basalen Vorderhirns, oder des gesamten basalen Vorderhirns (Ch 1-4) einen hohen prädiktiven Wert für die kognitive Entwicklung von Parkinson-Patient*innen ohne die Therapie mit einer Tiefen Hirnstimulation aufweisen.
Ziel/Fragestellung
Kann das im MRT ermittelte Volumen des Nucleus basalis Meynerts oder des gesamten basalen Vorderhirns als Prädiktor für das kognitive Outcome von Patient*innen mit dem iPS nach der Therapie mit einer STN-THS genutzt werden?
Methoden
Die präoperativen Navigations-MRT-Datensätze (T1-MPRAGE) von 55 Patient*innen (Alter: 61,4 ± 7,5 Jahre; Geschlecht: 39 männlich; Krankheitsdauer: 10,8 ± 4,7 Jahre) wurden analysiert (MATLAB 2019a Toolboxes SPM12 und CAT12). Anhand des Atlas von Zaborszky et al. (Anatomy Version 2.2b) wurden die Volumina des Nucleus basalis Meynerts und der Nuclei Ch 1-3 ermittelt. Die kognitive Entwicklung wurde anhand der relativen Veränderung von MMST- oder DemTect-Scores vor der Operation und 12 Monate danach berechnet. Zur Untersuchung des prädiktiven Wertes der ermittelten Hirnvolumina wurde eine multiple lineare Regressionsanalyse mit den Kovariablen intrakranielles Gesamthirnvolumen und präoperative L-Dopa-Äquivalenzsdosis durchgeführt.
Ergebnisse
Das mittlere kognitive Outcome nach der STN-THS war stabil mit einer leichten Tendenz zur Besserung. Allerdings zeigten die Patient*innen eine variable Veränderung nach der operativen Therapie (n = 15 Verbesserung; n = 24 keine Veränderung; n = 16 Verschlechterung).
Das Volumen des Nucleus basalis Meynerts konnte statistisch signifikant das kognitive Outcome der Patient*innen nach einer STN-THS voraussagen (F (3, 51) = 3,048; p = 0,037*; R2 = 0,152). Das kognitive Outcome (in %) wurde durch das Volumen des Nucleus basalis Meynerts (in cm3) signifikant beeinflusst (Nicht standardisierter Regressionskoeffizient B = 32,86; p = 0,017*).
Zusätzlich wurden die Volumina der Nuclei Ch 1-3 und Ch 1-4 als statistisch signifikante Prädiktoren der Kognition identifiziert.
Schlussfolgerung
Unsere Ergebnisse legen nahe, dass besonders das Volumen des Nucleus basalis Meynerts und unterstützend auch die Volumina Ch 1-3 und Ch 1-4 zur Abschätzung der kognitiven Entwicklung von Parkinson-Patient*innen nach einer STN-THS genutzt werden können. Übergeordnetes Ziel ist die bestmögliche Beratung hinsichtlich individueller Risikofaktoren, um Betroffene dem passenden Therapieverfahren zuzuführen und einen optimalen Therapieerfolg zu sichern.

Subthalamic deep brain stimulation is a well-established therapy for patients with idiopathic Parkinson’s disease to improve motor symptoms and quality of life(1). Data concerning cognitive outcome after subthalamic deep brain stimulation is still inconclusive(2).
Different studies suggest that the volume of the Nucleus basalis of Meynert (NBM), the main nucleus of the cholinergic basal forebrain (BF), or the volume of the entire BF provide a high predictive value for cognitive development in patients with idiopathic Parkinson’s disease without deep brain stimulation(3, 4).
Objective
Is it possible to use the volume of the NBM or of the entire BF (Ch 1-4), calculated from preoperative magnetic resonance imaging, as a predictor of cognitive outcome in patients with idiopathic Parkinson’s disease after subthalamic deep brain stimulation?
Methods
The preoperative magnetic resonance images with a high quality T1 MPRAGE sequence of 55 patients (age: 61.4 ± 7.5 years; gender: 39 male; disease duration: 10.8 ± 4.7 years) were analysed (MATLAB 2019a Toolboxes SPM12 and CAT12). The volumes of the NBM and the BF were calculated according to the atlas of Zaborszky et al.(5) (Anatomy Version 2.2b). The cognitive development was assessed calculating the percentage change of the scores MMSE(6) and DemTect(7) from pre- and 12 months postoperatively. To analyse the predictive value of the brainvolumes a multiple linear regression analysis adjusted for the total intracranial volume and the preoperative levodopa equivalent dose was conducted.
Results
The average cognitive outcome after subthalamic deep brain stimulation was steady with a tendency to improve. However, the patients showed a variable cognitive change after the therapy (n = 15 improvement; n = 24 no change; n = 16 decline).
The volume of the NBM could predict the cognitive outcome after deep brain stimulation statistically significant (F (3, 51) = 3,048; p = 0,037*; R2 = 0,152).
Additionally, the volumes of the Nuclei Ch 1-3 and of the entire BF were identified as statistically significant predictors of cognitive development after subthalamic deep brain stimulation.
Conclusion
Our results suggest that the volume of the NBM and additionally the volumes of the Nuclei Ch 1-3 and Ch 1-4 can be used to estimate the cognitive development of patients with idiopathic Parkinson’s disease after subthalamic deep brain stimulation. The overarching objective is to provide a best possible consultation for patients concerning their individual risk factors and thereby to ensure an optimal success of the therapy.
