HINTERGRUND: Die Prognose des anaplastischen Astrozytoms ist trotz intensiver, multimodaler Therapie infaust. Das Gesamtüberleben Betroffener divergiert stark und die frühzeitige Unterscheidung zwischen Patienten mit besonders schlechter Prognose und jenen mit besseren Therapiechancen ist herausfordernd. Im Rahmen der vorliegenden Studie erfolgte die Identifizierung von Parametern, die signifikant mit dem Gesamtüberleben von Patienten mit anaplastischem Astrozytom assoziiert sind. Ziel dieser Arbeit war die Konstruktion eines klinischen Modell-Scores, der die Abgrenzung prognostischer Gruppen von Patienten mit anaplastischem Astrozytom zum Zeitpunkt der primären Diagnosestellung ermöglicht. METHODEN: Patientendaten wurden unizentrisch und zehn Jahre retrospektiv erhoben. Insgesamt konnten 108 Patienten identifiziert werden, die im Zeitraum von Januar 2010 bis Januar 2020 an der Charité Universitätsmedizin Berlin primär mit einem anaplastischen Astrozytom diagnostiziert und therapiert wurden. Weiteres zentrales Einschlusskriterium war der erfolgte Ausschluss einer 1p/19q- Kodeletion. Es folgte die Untersuchung sechs verschiedener Parameter auf Ihren Einfluss auf das Gesamtüberleben. Variablen, die sowohl in der univariaten, als auch in der multivariablen Analyse eine signifikante Assoziation mit dem Gesamtüberleben zeigten, wurden in dem Score berücksichtigt. Die Konstruktion des klinischen Prognosescores erfolgte anhand der 36-Monats-Überlebensrate.
ERGEBNISSE: Alter, Karnofsky-Index, Isozitratdehydrogenase-Status und Resektionsgrad zeigten sowohl in der univariaten, als auch in der sich anschließenden multivariablen Analyse eine signifikante Assoziation mit dem Gesamtüberleben. Der mittels dieser vier Variablen konstruierte Score ergab Gesamtwerte zwischen 20 und 33 Punkten. Anhand dieser ließen sich für die Studienkohorte drei prognostische Gruppen abgrenzen: A (20-25), B (26-29) und C (30-33 Punkte). Die 36-Monats- Überlebenrate lag in Gruppe A bei 23%, in Gruppe B bei 71% und bei 100% in Gruppe C. Das 5- Jahres-Überleben divergierte ebenfalls signifikant und lag bei 8% bzw. 61% und 88%. SCHLUSSFOLGERUNG: Das vorgestellte Modell eines klinischen Prognosescores für Patienten mit anaplastischem Astrozytom erlaubt die Zuordnung zu prognostischen Gruppen zum Zeitpunkt der Diagnosestellung. Angesicht der ausstehenden Überarbeitung der WHO-Klassifikation der Tumoren des zentralen Nervensystems sowie neuer molekularpathologischer Erkenntnisse und der Ergebnisse der CATNON-Studie werden eine Weiterentwicklung und Adaption des Scores nötig sein. Die Validierung durch eine unabhängige Kohorte ist Voraussetzung für eine klinische Anwendung.

BACKGROUND: Prognosis of patients diagnosed with anaplastic astrocytoma is poor, even when treated with intensive multimodal therapy. Overall survival time varies greatly within patients with anaplastic astrocytoma. Early differentiation between those with especially poor prognosis and those with better prospects would be helpful in choice of treatment regimen and patient consultation. This study aims to identify parameters significantly associated with overall survival and constructs a model of a prognostic score, suitable to identify different prognostic groups in patients with anaplastic astrocytoma. METHODS: All patient data was collected retrospectively. We identified 108 patients who had been diagnosed with anaplastic astrocytoma and received primary treatment at Charité Universitätsmedizin Berlin from January 2010 to January 2020. Tested absence of 1p/19q- codeletion was required for inclusion in the study. We evaluated a total of six parameters for their association with overall survival. Those variables that proved to have significant impact on overall survival in both, univariate and multivariate analysis, were then included in the scoring system. The final score was based on the 36-month survival rate. RESULTS: Age, Karnofsky Performance Status, isocitrate dehydrogenase status, and extent of resection all demonstrated significant association with overall survival in both, univariate and multivariate analysis. The prognostic score was constructed factoring in the 36-month survival rate and ranged between 20 and 33 points. We designated three prognostic groups: A (20-25), B (26-29) and C (30-33 points), which showed significant differences in overall survival. 36-month survival rate for group A was 23%, 71% for group B and 100% for group C, overall survival rate at five years was 8%, 61% and 88%, respectively. CONCLUSION: This anaplastic astrocytoma-specific model score allows the differentiation of prognostic groups at the time of initial diagnosis. Validation by an independent cohort is needed. Future adaptation will be necessary, especially given the expected update of the WHO classification of brain tumors, the practice changing results of the CATNON trial and our growing knowledge of molecular parameters.
