A rise of the average age of a population makes the need for prosthesis, e.g., for hip or knee implants increasingly necessary. Because of wear, which elicits painful abrasion and inflammation, but also loosening and dislocation secondary replacements become more frequent. On the other hand implants become loose due to bone deformation which is caused by a change in stress distribution in the bone. This is why the optimal positioning of an implant needs to minimize the difference in stress distribution, the so-called stress shielding. Simultaneously one is to prevent locally high stresses because in case of exceeding a malign threshold value the spongy bone may break.
Therefore an elastomechanic contact problem that includes daily, but moreover all possible motions – trajectories in a relatively high dimensional load domain – is solved. The model of the Green-St. Venant stress tensor mandatory needs to include the geometric non-linearity as otherwise the anticipated appearance of rotations would annul the validity of the computed solution. In order to reduce the expected expensive computation and thus to enable the solution of the optimal control problem, the necessary application of efficient algorithms to sub-problems is studied in this work. As a first step the number of PDE solves is to be reduced as in case of a fine FE-grid the solution of the PDE constitutes the most expensive contribution. To this end Kriging interpolation is introduced and implemented. Kriging allows for sequential addition of test loads and the resulting load-response. Further the model-inherent variance can be utilized to test the precision of the current interpolating surrogate function and adaptively add training data from areas where the variance is maximized.
Additionally one exploits the marginalization of motions onto the load domain by means of a probability distribution. Here the measured loads from the open database OrthoLoad are combined with a patient-specific weighting of the motions such that the PDE is computed in a quasi-static way on the one hand, and on the other hand the probability distribution is made use of in the computation of the integral via Monte Carlo integration. The FE-discretization induces the order of error with which the accuracy of PDE solution, the Kriging interpolation and the Monte Carlo integration is controlled and, if necessary, adaptively refined.
Lastly, the derivative of the objective function is analytically evaluated wherein the implicit function theorem is applied. As implant position changes may lead to degenerate grids the re-discretization of the FE-grid becomes imperative. With that the FE-discretization error changes leading to discontinuities in the objective function calling for appropriate treatment thereof by the optimization strategy. This is here realized with a special line-search in the estimation of the step size.
The thesis concludes with a numerical evaluation of the interaction of the algorithms showing their efficiency and giving a proof of concept of the devised model simplifications and the optimization algorithm in total.
Durch steigendes Durchschnittsalter einer Bevölkerung werden Prothesen z.B. in der Hüfte oder am Knie immer notwendiger; und durch Abnutzungen, welche schmerzhaften Abrieb und dadurch Entzündungen, aber auch Lockerung verursachen, werden Revisionsoperationen immer häufiger. Da sich Implantate aber auch lockern, weil sich der umgebende Knochen durch die geänderte Druckverteilung deformiert, müssen Implantate so eingepasst werden, dass sie in einer optimalen Position die Druckänderung, das sogenannte stress shielding, minimieren. Gleichzeitig sollen aber auch lokal hohe Maximaldrücke vermieden werden, da sie durch das Übersteigen eines schadhaften Schwellwerts dazu führen können, dass der gegebenenfalls poröse Knochen bricht.
Hierzu wird ein elastomechanisches Kontaktproblem unter Berücksichtigung der täglichen bzw. aller möglichen Bewegungen, welche Trajektorien in einem relativ hochdimensionalen Lastraum darstellen, gelöst. Der Green-St. Venant Verzerrungstensor muss zwingend inklusive der geometrischen Nichtlinearität modelliert werden, da die beteiligten Körper in den Gelenken rotieren können. Um die zu erwartenden hohen Rechenanforderungen zu reduzieren und das Lösen des Optimalsteuerungsproblems überhaupt möglich zu machen, werden effiziente Methoden für Unterprobleme untersucht und angewandt. Dafür soll zunächst die Anzahl der Lösungen der partiellen Differentialgleichung (PDG) verringert werden, da bei fein diskretisiertem Finite Elemente-Gitter die Lösung der PDG den teuersten Anteil ausmacht. Hierfür wird Kriging als Interpolationsmethode vorgestellt und implementiert. Kriging erlaubt das sequentielle Einfügen von Testlasten und den daraus resultierenden Drucklast-Antworten. Außerdem kann die modell-inhärente Varianz ausgenutzt werden, um zum einen die Interpolationsgenauigkeit zu prüfen und zum anderen an Stellen, wo das Interpolationsmodell die größte Varianz aufweist, dem Modell adaptiv hinzuzufügende Lasten zu bestimmen.
Weiterhin wird genutzt, dass die Bewegungen auf den Lastraum marginalisiert und durch eine Wahrscheinlichkeitsverteilung charakterisiert werden können. Dazu werden gemessene Lastdaten der öffentlich zugänglichen Datenbank OrthoLoad mit einer patientenspezifischen Gewichtung der Bewegungen kombiniert, sodass einerseits die PDG quasistatisch gerechnet werden kann und andererseits die Wahrscheinlichkeit in der Berechnung des Integrals über den Lastraum mittels Monte Carlo Integration zur Anwendung kommt. Abhängig von der FE-Diskretisierung ergibt sich eine Fehlerordnung, anhand derer die Genauigkeit der PDG-Lösung, der Kriging Interpolation und der Monte Carlo Integration justiert und bei mangelhafter Genauigkeit adaptiv verfeinert werden können.
Zuletzt wird für das Optimierungsverfahren die nötige Ableitung des Zielfunktionals mithilfe des impliziten Funktionentheorems hergeleitet. Außerdem muss für die Schrittweitensteuerung eine spezielle Liniensuche eingeführt werden, sodass bei sich änderndem FE-Diskretisierungsfehler durch Verschiebung der Implantatsposition, die auftretenden Unstetigkeiten im Zielfunktionswert die Optimierung nicht zwecklos machen.
Das funktionierende Zusammenwirken der Algorithmen wird numerisch ausgewertet und nachgewiesen.
