Einleitung: Nach den aktuellen ESC-Leitlinien spielen kardiale Troponine die
zentrale Rolle bei der Diagnostik eines NSTEMI. Auch in der
Risikostratifizierung und Prognostik von ACS-Patienten haben kardiale
Troponine als eine Variable des GRACE-Risk-Scores eine wichtige Funktion.
Kürzlich wurde eine neue Generation von hochsensitiven Troponin-Assays
entwickelt, welche eine neuartige Sensitivität aufweisen und dadurch die
Infarkt-Diagnostik verbessern. Es liegen jedoch bisher nur wenige Daten zur
Fragestellung vor, ob die hs Assays auch in der Risikostratifizierung und in
der Prognostik von ACS-Patienten Vorteile bringen. Das Ziel der BIC-2-Studie
ist daher, weitere Daten zum klinischen Nutzen von hs cTnI sowie zur
Wertigkeit von hs cTnI in der Prognostik bei Patienten mit akuten
kardiovaskulären Symptomen in der Notaufnahme hervorzubringen. Methoden: Es
wurden 537 Patienten mit den kardiovaskulären Leitsymptomen Brustschmerz und
Dyspnoe eingeschlossen. Von ihnen wurden bei der initialen Vorstellung in der
Notaufnahme klinische Daten gewonnen und Blutproben entnommen. Nach 90 Tagen
erfolgte ein Follow-Up mit telefonischer Nachbefragung hinsichtlich der
primären Endpunkte Rehospitalisierung und Tod. Die hs cTnI Werte wurden mit
dem ARCHITECT® STAT hs cTnI Assay der Firma Abbott bestimmt. Als erhöhte Werte
wurden hs cTnI Werte oberhalb der 99. Perzentile (23,72 pg/ml) einer gesunden
Population von 300 Charité-Mitarbeitern definiert. Ergebnisse: In der
BIC-2-Studie wiesen im Follow-Up rehospitalisierte und verstorbene Patienten
bereits in der initialen Messung signifikant höhere hs cTnI Werte auf. In
Subgruppenanalysen zeigte sich, dass rehospitalisierte Nicht-AMI-Patienten
bereits initial signifikant höhere hs cTnI Werte hatten als nicht
rehospitalisierte Probanden. Auch verstorbene Nicht-AMI-Patienten hatten
bereits in der anfangs erfolgten Messung signifikant höhere hs cTnI Werte als
überlebende Probanden. Im Vergleich des hs cTnI Studien-Assays mit dem während
des Studienzeitraums klinisch eingesetzten, konventionellen Assay zeigte sich
tendenziell eine höhere prognostische Wertigkeit des hs cTnI Assays. Die
Patientengruppe mit nach konventioneller Bestimmung normwertigen
Troponinwerten und nach hs Messung erhöhten Werten wies die höchste
Mortalitätsrate auf. Schlussfolgerungen: Der hs cTnI Assay wies tendenziell
eine höhere prognostische Wertigkeit auf als der konventionelle Troponin-
Assay, da er die Mortalität im Follow-Up-Zeitraum präziser vorhersagen konnte.
Insbesondere in der Nicht-AMI-Patienten-Gruppe wiesen bereits initial in der
Notaufnahme erhöhte hs cTnI Werte auf ein größeres Risiko für
Rehospitalisation oder Mortalität hin. Erhöhte hs cTnI Werte zeigen offenbar
eine vorliegende Herzbeteiligung einer Erkrankung an. Zukünftig sollte der
therapeutische Fokus daher auch auf die Nicht-AMI-Patientengruppe ausgeweitet
werden, um auch hier durch ein möglichst standardisiertes, evidenzbasiertes
Vorgehen die Mortalitätsrate zu senken.

Introduction: Current ESC guidelines recommend serial measurements of cardiac
troponin for the diagnosis of NSTEMI. In terms of risk stratification and
prognosis in patients with ACS cardiac troponin is integrated as one
prognostic variable into the GRACE-Risk-Score. Recently a new generation of
highly sensitive troponin assays has been developed, which seems to improve
the diagnosis of acute myocardial infarction (AMI). However, currently only
few data is available regarding the question whether the high-sensitivity
assays might improve risk stratification and prognosis in patients with ACS.
The objective of the BIC-2-Study is to show if high-sensitively measured
Troponin I has a clinical value in risk stratification and prognosis in
patients presenting with cardiovascular symptoms to the emergency department.
Methods: We enrolled 537 patients with the cardiovascular chief complaints
chest pain and dyspnoea. Blood samples were obtained at admission to the
emergency department. After a period of 90 days the primary endpoints
rehospitalisation and death were assessed via a telephone follow-up. Highly
sensitive troponin I (hs cTnI) levels were analysed by a prototype of the
Abbott ARCHITECT® STAT hs cTnI Assay. Elevated hs cTnI levels were defined as
above the 99th percentile (=23,72 pg/ml) of a healthy population of Charité
employees. Results: BIC-2-Study patients who were rehospitalised or died in
the follow-up period had initially significant higher hs cTnI levels compared
to patients without any endpoint. Subgroup analysis revealed that also non-AMI
patients who were rehospitalised or died in the follow-up period showed
significantly higher hs cTnI levels in the first blood draw. Comparing the
high-sensitivity assay to a contemporary standard assay used in the emergency
department, a better prognostic value for the hsTnI assay was shown in
survival analysis with the highest mortality in patients with a normal result
for conventional cTn and an elevated result for hs cTnI. Conclusion: Compared
to the conventional cTn assay, the hs cTnI assay showed a better prognostic
value, since the mortality in the follow-up period could be predicted more
precisely, particularly in the group of non-AMI patients presenting to the
emergency department with initially elevated hs cTnI levels. Thus elevated hs
cTnI levels can be a sign of cardiac effects of several diseases. In future
the medical focus should be broadened to the group of non-AMI patients in
order to establish a more evidence-based treatment and decrease the mortality
in these patients.
