Background: Liver transplantation represents a curative treatment option for terminal liver disease. Due to the increasing age of recipients, orthopedic issues are expected to become increasingly relevant in the coming years. When dealing with immunosuppression of liver-transplant patients prior to elective surgery, however, it is unclear to what extent a preoperative change in the treatment regimen is advisable, due to a lack of evidence. The aim of this retrospective analysis was to determine the influence of immunosuppression on the postoperative outcome after orthopedic surgery in liver transplant patients and to derive a recommendation whether or not a preoperative change of the immunosuppression is advisable. Other possible influencing factors were also to be identified and investigated. Methods: The data of 152 patients in continuous care in the Charité liver transplant clinic between 2016 and 2017 were retrospectively evaluated. A total of 252 orthopedic trauma surgical events were included and examined in this study. Besides clinical parameters, the influence of immunosuppression, the intake of various osteoporosis-promoting and osteoporosis-prophylactic drugs, bone density and quality of life were evaluated and statistically analyzed. Results: Overall, most cases (80.6%) were without complications. At the time of the event, immunosuppression consisted in 102 (40.4%) cases of a monotherapy with a calcineurin inhibitor (CNI), in 12 (4.8%) cases of a CNI with prednisolone and in 138 (54.8%) cases of various other drug combinations. Examinations on the influence of immunosuppression on the postoperative complication rate, fracture frequency and quality of life showed no differences between the individual immunosuppression regimens (p>0.05). A preoperative change did not have a positive effect on the postoperative course (p>0.05). Considering further factors influencing the outcome, a higher patient age (p=0.03), the intake of proton pump inhibitors (PPIs) (p=0.04) and vitamin K antagonists (p=0.04) correlated significantly with a worse postoperative course. Conclusion: Neither the choice of drug combinations nor a preoperative change of immunosuppression could be associated with a better course. Neither could a correlation between the treatment regimen and fracture frequency be demonstrated. A change in immunosuppression before elective surgery is not considered advisable. However, especially a long-term intake of PPIs should be critically questioned and avoided in the absence of an indication.
Die Lebertransplantation stellt eine kurative Behandlungsmöglichkeit terminaler Lebererkrankungen dar. Aufgrund steigender Empfängeralter ist in den kommenden Jahren hier mit einer zunehmenden Relevanz orthopädischer Thematiken zu rechnen. Beim Umgang mit der Immunsuppression lebertransplantierter Patienten vor elektiven Eingriffen herrscht jedoch aufgrund fehlender Evidenz Unklarheit, inwieweit eine präoperative Umstellung des Therapieschemas ratsam ist. Ziel dieser Arbeit war es, den Einfluss der Immunsuppression auf den postoperativen Verlauf nach orthopädisch-unfallchirurgischen Eingriffen an lebertransplantierten Patienten zu erfassen und eine Empfehlung zur Frage einer präoperativen Umstellung für die Praxis abzuleiten. Weitere mögliche Einflussfaktoren sollten ebenfalls identifiziert und untersucht werden. Methodik: Es wurden die Daten von 152 Patienten, die sich zwischen 2016 - 2017 in der Lebertransplantationsambulanz der Charité in Nachsorge befanden, retrospektiv ausgewertet. Insgesamt wurden 252 orthopädisch-unfallchirurgische Ereignisse in diese Arbeit eingeschlossen und untersucht. Neben klinischen Parametern wurde der Einfluss der Immunsuppression, die Einnahme verschiedener osteoporosefördernder/-prophylaktischer Medikamente, die Knochendichte und die Lebensqualität evaluiert und statistisch ausgewertet. Ergebnisse: Insgesamt verliefen die meisten Fälle (80,6%) komplikationslos. Zum Zeitpunkt des Ereignisses bestand die Immunsuppression in 102 (40,4%) Fällen aus einer Monotherapie mit einem Calcineurininhibitor (CNI), in 12 (4,8%) Fällen aus einem CNI mit Prednisolon und in 138 (54,8%) Fällen aus verschiedenen anderen Wirkstoffkombinationen. Die Untersuchungen zum Einfluss der Immunsuppression auf die postoperative Komplikationsrate, Frakturhäufigkeit sowie Lebensqualität zeigten keine Unterschiede zwischen den einzelnen Immunsuppressionsschemata (p>0,05). Eine präoperative Umstellung wirkte sich nicht positiv auf den postoperativen Verlauf aus (p>0,05). In Anbetracht weiterer Einflussfaktoren auf das Outcome korrelierten sowohl ein höheres Patientenalter (p=0,03), als auch die Einnahme von Protonenpumpeninhibitoren (PPIs) (p=0,04) sowie Vitamin-K-Antagonisten (p=0,04) signifikant mit einem schlechteren postoperativen Verlauf. Schlussfolgerung: Weder die Wahl der Wirkstoffkombinationen, noch eine präoperative Umstellung der Immunsuppression konnten mit einem besseren Verlauf assoziiert werden. Auch eine Korrelation zwischen dem Therapieschema und der Frakturhäufigkeit konnte nicht nachgewiesen werden. Eine Umstellung der Immunsuppression vor elektiven Eingriffen wird nicht als sinnvoll erachtet. Jedoch sollte insbesondere eine langzeitige Einnahme von PPIs kritisch hinterfragt und bei fehlender Indikation vermieden werden.
