Bei nierentransplantierten Patient*innen wird die Transplantatfunktion aktuell durch Proteinurie und Serum-Kreatinin-Bestimmungen überwacht. Bei Verdacht auf eine Abstoßungsreaktion wird eine Nierenbiopsie durchgeführt, um die entsprechende Diagnose und weitere Pathologien feststellen zu können. Zur Entwicklung neuer, nicht-invasiver Monitoringmethoden für die Betreuung nierentransplantierter Patient*innen wurden in dieser Arbeit Immunzellen und Podocalyxin(PDX)-positive Zellen im Urin analysiert. CD4+-positive T-Zellen, Monozyten/Makrophagen und PDX-positive Zellen im Urin wurden mittels Durchflusszytometrie quantifiziert. Zellpopulationen wurden anschließend mit Biopsieergebnissen und Transplantatverläufen verglichen. Der Urin von 94 nierentransplantierten Patient*innen wurde in dieser Arbeit analysiert. Die Patientengruppen setzten sich aus einer Querschnitts-Biopsie-Kohorte, einer Längsschnitt-Studienkohorte an frischtransplantierten Patient*innen und einer Kontrollgruppe zusammen. Biopsiepatient*innen wurden in Patient*innen mit T-Zell-vermittelter Abstoßung (T cell-mediated rejection, TCMR), antikörpervermittelter Abstoßung (antibody-mediated rejection, ABMR) und Patient*innen ohne Hinweise auf Rejektion in der Biopsie (No rejection, No RX) eingeteilt. Patient*innen mit TCMR zeigten mehr T-Zellen im Urin als Patient*innen der Kontrollgruppe. Patient*innen mit Biopsie, jedoch ohne Rejektion in der histologischen Beurteilung, zeigten jedoch ähnlich viele T-Zellen. Makrophagen im Urin korrelierten mit der histologischen Beurteilung der interstitiellen Inflammation (p = 0.0004, r = 0.6978) und Tubulitis (p = 0.0088, r = 0.5562). Anhand der Kombination aus CD4+-T-Zellen und PDX-positiven-Zellen konnten Patient*innen mit Rejektion (TCMR und ABMR) von Patient*innen ohne Abstoßung deutlich voneinander getrennt werden (alle p<0.05, Area under the curve (AUC) 0.83). Prognosen bezüglich des Verlaufs konnten anhand der Analyse von Urinzellen nicht sicher getroffen werden. Die Analyse von Urinzellen mittels Durchflusszytometrie hat Potential, das Monitoring von nierentransplantierten Patient*innen zu optimieren. Der Quotient aus T-Zellen und PDX-positiven Zellen könnte zur Diagnose von Transplantatabstoßungen genutzt werden.
Creatinine and proteinuria are used to monitor kidney transplant patients. However, renal biopsies are needed to discriminate pathologies and diagnose renal graft rejection. In this study, we assessed whether the quantification of urinary immune cells and podocalyxin-positive (PDX-positive) cells would allow non-invasive detection of allograft rejection and outcome predictions in newly transplanted patients. Urinary cell numbers of CD4+ T cells, monocytes/macrophages, and PDX-positive cells were determined using flow cytometry and were compared to renal biopsy results and outcomes after transplantation. A total of 94 renal transplant patients were analyzed, consisting of patients with biopsy-proven T cell-mediated rejection (TCMR), antibody-mediated rejection (ABMR), patients whose biopsies showed deteriorating graft function without rejection (No RX), a control group of stable transplant patients (Ctrl) and a group of newly transplanted patients. Patients with transplant rejection had higher amounts of urinary T cells than Ctrl patients; however, patients who showed worsening graft function without rejection had similar numbers of T cells. Urinary macrophages correlated with the histological levels of interstitial inflammation (p = 0.0004, r = 0.6978) and tubulitis (p = 0.0088, r = 0.5562). Combining the amount of urinary T cells and PDX+ cells, yielded a significant segregation of patients with rejection (TCMR and ABMR) from patients in the No RX group (all p<0.05, and area under the curve (AUC 0.83). There was no meaningful significant segregation in newly transplanted patients regarding the analyzed urinary cell population. Urinary cell populations analyzed by flow cytometry have the potential to establish new monitoring methods for kidney transplant patients. The combination of urinary T cells and PDX-positive cells may allow non-invasive detection of transplant rejection.
