Geschlechtsangleichende Operationen (gaOP) nehmen heutzutage einen immer größeren Stellenwert ein. Das Radialispenoid hat sich dabei im Rahmen der gaOP Frau zu Mann (FzM) etabliert. Dafür existieren drei operative Varianten, welche sich im Hinblick auf die Formung der Harnröhre unterscheiden. Die älteren Verfahren nach Chang bzw. Gottlieb und Levine haben eine ulnar bzw. zentral gelegene Harnröhre. Das neuste Verfahren ist die Variante mit präformierter Harnröhre. Im Gegensatz zu den älteren Verfahren existiert bei letzterem ein erhebliches Erfahrungsdefizit mit nur spärlicher Literatur. Ziel dieser Arbeit war es, die Komplikationen der einzelnen operativen Teilschritte zu erfassen, sie nach Clavien-Dindo zu klassifizieren und sie mit der Literatur zu vergleichen.
Patienten und Methoden Als Studiendesign wurde eine retrospektive Single-Center-Studie gewählt, welche alle transidenten Patienten einschloss, die zwischen 01.01.2011 und 30.06.2017 ein Radialispenoid mit präformierter Harnröhre erhielten. Insgesamt konnten 39 Patienten eingeschlossen werden. In einem Nachbeobachtungszeitraum von sechs bis 80 Monaten (durchschnittlich 32 Monate), wurden die Komplikationen der operativen Teilschritte: Kolpektomie, Harnröhrenpräformierung, Penoidkonstruktion, Hodenprothesenimplantation und Glansplastik erfasst und nach Clavien-Dindo eingeteilt. Die Ergebnisse wurden mit der Literatur verglichen.
Ergebnisse Bei fünf von 39 Patienten (12,8%) verlief die Penoidkonstruktion komplikationslos. 22 Patienten (56,4%) erlitten eine Grad I, vier Patienten (10,3%) eine Grad II Komplikation. Eine revisionspflichtige Grad IIIb Komplikation trat bei 16 Patienten (41%) auf. Ein Patient (2,6%) erlitt eine Grad IV Komplikation. Bei 28 Patienten (71,8%) trat postoperativ eine urethrokutane Fistel auf. Allerdings mussten diese Fisteln nur in acht der 28 Fälle (20,5%) operativ verschlossen werden. In fünf Fällen (12,8%) trat eine Harnröhrenverengung auf, bei zwei Patienten mehrfach (insgesamt acht). Bei zwei Patienten (5,1 %) kam es zu einem Lappenverlust.
Schlussfolgerung Die Ergebnisse zeigen, dass das Verfahren mit präformierter Harnröhre den Varianten nach Chang sowie Gottlieb und Levine ebenbürtig ist und somit einen berechtigten Stellenwert hat. Jedoch fehlen in der bisherigen Literatur weiterhin wichtige Angaben der einzelnen Komplikationen, was einen echten Vergleich verhindert. Eine Klassifikation etwa nach Clavien-Dindo wäre dabei essentiell. Darüberhinaus konnte eine Reduktion der Rate an urologischen Komplikationen in der eigenen Arbeitsgruppe im Vergleich zu einer früheren Serie festgestellt werden. Dies könnte in der eigenen Lernkurve und einer technischen Modifikation im Sinne einer Visierlappenplastik unterhalb der Harnröhrenanastomose begründet sein. Die Serie dieser Arbeit liefert die bislang größte Datenbasis an klassifizierten Komplikationen und kann somit für künftige Vergleiche zu alternativen Techniken herangezogen werden.
Gender reassignment surgery (GRS) has been becoming increasing importance in recent years. The radial forearm flap has been established a standard for penile construction in GRS female-to-male (FTM). There are three surgical variants of the flap which differ in the formation of the urethra. The older techniques reported by Chang respectively Gottlieb and Levine have an ulnar respectively central urethra. The latest development is the technique with a prelaminated urethra. Compared to the older methods there is a lack of experience regarding this new technique. The aim of this dissertation was to assess the complications of the individual surgical steps, to classify them according to Clavien-Dindo classification and to compare them with the literature.
Patients and Methods A retrospective single-center study design was chosen. All transident patients undergoing a GRS using a radial forearm flap with a prelaminated urethra between 01/01/2011 and 30/06/2017. A total number of 39 patients were included. The complications of the surgical steps: colpectomy, urethral prelamination, phalloplasty, implantation of testicular prostheses and glans-plasty were assessed and classified according to Clavien-Dindo in a follow-up period of six to 80 months (32 months on average). The results were compared to the literature.
Results In five out of 39 patients (12.8%) the phalloplasty was uneventful. 22 patients (56.4%) suffered a grade I, four patients (10.3%) a grade II complication. A grade IIIb complication requiring surgical revision occurred in 16 patients (41%). One patient (2.6%) had a grade IV complication. In 28 patients (71.8%) a urethrocutaneous fistula occurred. However, these fistulas required surgical revision in only eight of these cases (20.5% of the total population). In five cases (12.8%) urethral stenoses were observed, in two of them multiple (eight in total). Flap losses occurred in two patients (5.1 %).
Conclusion The data show that the complication rate of the single surgical steps of the radial forearm flap with a prelaminated urethra is on same level or even lower compared with the elder Changs- or Gottlieb and Levines designs. Thus, the technique has proven to be standard procedure as well. However, important information about the severity of the complications is still missing in the previous literature allowing only poor comparison. A classification according to e.g. Clavien-Dindo would be essential. Additionally, a reduction in the rate of urological complications compared to a previous study was found in the current database. This finding could be due to our learning curve and a technical modification (visor flap at the site of the urethral anastomosis). This series provides the largest to date published database of classified complications using a radial forearm flap for GRS. Thus, it can be used for future comparisons to new techniques and innovations of existing methods.
